0725899842 2011-11-02 Production Wells No Violations There were no violations noted at the time of inspection.
1549033013 2019-02-01 Production Wells No Violations Follow-up to Complaint #6065 concerning natural gas found in a nearby water well that caught fire on 1/31/19. I inspected this well with Inspector Dave Morrison. The inspection found the well equipped with 2.375 tubing and a plunger lift system. The gate valve is currently closed and the well is only supplying domestic gas at this time. The 8.625 surface casing ring is just above ground level and the annular port valve is fitted with a ball valve and bull plug at ground level. There are no gauges at the wellhead to assess pressure on the annulus or wellhead. I spoke with M & R Field Supervisor Rusty Morrison and requested that gauges be placed on the production casing and 4.5 X 8.625 annulus for assessment. Rusty said he would have this completed Monday Morning (2/4/19).
1549477472 2019-02-06 Production Wells No Violations Follow-up to Complaint #6065 in regards to an investigation of natural gas found in a domestic water well one quarter mile to the southwest. M&R Field Supervisor Rusty Morrison gauged the annular pressure between the 8.625 surface casing and 4.5 production casing on 2/4/19 and found 130psi on the annulus. The pressure on the 4.5 casing was found to be 250psi and there were no casing integrity issues. The annular pressure was found to be over pressurized by 7psi based on OAC 1501:9-1-08(N)(2), and I requested that M&R relieve the pressure on the annulus. Rusty notified me on 2/5/19 that the annulus was plumbed to the surface equipment to relieve the pressure and that it was down to 50psi. I requested that the annulus be left open to prevent building any pressure at the surface casing seat. My inspection today found the annulus plumbed to the flow line with a copper line at the wellhead. The well is currently shut-in (per Rusty) due to the domestic gas usage making plunger operation impossible at this time. The flow line valves are currently bypassing the separator and are open to the 100 barrel tank. I inspected the tank and vent stack and found no evidence of natural gas venting from the annulus indicating that the pressure has been relieved. No violations.
1550682773 2019-02-20 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1871575934, regarding the annular pressure on the 8 5/8 inch surface casing. At the time of this inspection, the well was equipped with a plunger lift assembly and not in production. The gate valve on the tubing and valve on the 2 inch flow line were closed. There was a domestic line plumbed off the 4 1/2 inch casing with a regulator, drip bottle and meter. The valve on the domestic line was closed. There was a valve plumbed off the 8 5/8 inch casing port with a pressure gauge. The gauge reading was 65 psi, at the time of this inspection. The diked tank battery was comprised of one 100 BBL tank and a mechanical separator. Valves on the separator were closed. Legible identification was located at the tank battery and the lease access was stable.
1556028800 2019-04-18 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1871575934, regarding the annular pressure on the 8 5/8 inch surface casing and complaint #6065 involving gas that caused a fire in a nearby residential water well. At the time of this inspection, I was on location with DOGRM - Environmental Assessment Team and consultants from Moody & Associates to collect gas samples from this well. The pressure on the surface casing gauge read 65 psi. The domestic line plumbed off of the 4.5 inch casing was open. A pressure reading was taken from the 4.5 inch casing and gauged at 80 psi. The well was not in production, during this inspection. Gas samples were collected by consultants from Moody & Associates and will be evaluated to determine if the gas from this well and gas present in the water well described in complaint #6065, are related. No other issues were observed at the well or tank battery location.

1828506900 2012-06-14 Production Wells No Violations I completed a production well inspection status check. My inspection found the well equipped with a plunger lift unit that was in good condition. The tank battery contained one, 100 barrel oil storage tank, a separator, and the proper identification. I noticed no visible spills or leaks. The lease road was in good condition. There are no violations at this time.
1871575934 2019-02-13 Production Wells Violations Noted Follow-up to Complaint #6065 concerning natural gas found in a water well located 0.25 miles to the southwest of this wellhead. This well that was found to be over-pressurized on the 4-1/2 production casing X 8-5/8 surface casing annulus on 2/4/19 by M&R Field Supervisor Rusty Morrison. The annular and casing pressures were not equalized at this time and it was assumed that the 4.5 production had integrity. The pressure on the annulus was relieved to the tank battery on 2/5/19. On 2/8/19, Rusty informed me that the pressure on the 4.5 production casing had dissipated after relieving the pressure on the annulus and that the landowners domestic gas supply (tapped to the production casing port) was dead. This condition has demonstrated that the production casing is defective and is communicating with annular space constituting a violation under 1509.12(A) of the Ohio Revised Code. My inspection today found that the annular port, bleed-off line has been disconnected and the valve is closed. The domestic supply has been plumbed to the 2.375 tubing at this time. I spoke with Rusty and requested that a needle gauge be installed on the annular port to monitor the pressure and that a Compliance Notice would be issued for defective casing. M&R Investments Ohio, LLC will be afforded 30 days from the discovery date of the defect to assess remedial options and submit a plan to 1) repair the defective casing, 2) plug and abandon the well, or 3) perform another activity approved by the Division. Failure to comply with this notice may result in further enforcement action.
