Inspections Performed

-431927424 2014-10-10 0:00 PB No Violations 8.50
0431927424 2014-10-10 Plug / Plug Back No Violations I was on location to witness J-Well plug the well for K and R Realty. J-Well removed the remaining 1.5 tubing from the well. Next, the operator rigged up to pull stretch on the 4.5 casing and found the casing had parted. J-Well decided to removed the 4.5 casing and set the remaining plugs with the tubing. The total length of 4.5 casing removed from the well was 2,818 with an average joint length of 38.10. The operator set the tubing at 2,755 for the Big Lime plug. Plug #2 was set from 2,755 to 2,455. PetroSet rigged up and established circulation. The operator mixed and pumped 75 sacks of Class A cement with 2% gel. The cement was displaced with 6 barrels of water. J-Well pulled 70 joint of tubing. Plug #3 was set from 421 to 221. PetroSet established circulation. The operator mixed and pumped 50 sacks of Class A cement. J-Well pulled 7 joints of tubing. Plug #4 was set from 200 to surface. PetroSet established circulation. The operator mixed and pumped 95 sacks of Class A cement. I witnessed good cement returned to surface. J-Well removed the final 6 joints from the well and PetroSet topped off the well with cement. After 20 minutes, the cement held static at surface. A cement sample was taken and weighed.
1412941127 2014-10-09 Plug / Plug Back No Violations I was on location to witness J-Well plug the well for K and R Realty. PetroSet was contracted as the cementing contractor. First, J-Well ran 4,333 of 1.5 tubing into the well to set the bottom plug. Plug #1 was set from 4,333 to 3,966. PetroSet rigged up and established circulation. The operator mixed and pumped 25 sacks of Class A cement with 2% gel. The cement was displaced with 10 bbls of water. Next, J-Well removed 60 joints of tubing to clear the cement. The well was shut-in for the night.
1418049066 2014-12-08 Final Restoration No Violations I was on a location for a final restoration. During my inspection, I found the site graded and well vegetated. There was no production equipment associated with the well. No pits were used to plug the well; a half round container was used for all returns. All debris was removed. The lease road was restored. Final restoration passed.
