0007037873 2013-04-29 Plug / Plug Back No Violations I was on location to witness the plugging of this oil and gas well. Nicks Well Plugging is the contractor doing the plug job for Summit Oil. Nicks stripped the rods and tubing out of the hole, rigged up, and set the Clinton plug through the 4.5 casing with 25 sacks of cement and 50 pounds of cottonseed hulls. The well pressured up to 1500 psi and was shut-in until the following morning. No violations.

1342626274 2012-07-18 Preliminary Restoration No Violations On 7/18/2012, I completed a production well inspection status check. My inspection found the well was a pumping unit that was in good condition. The tank battery contained two 100 barrel oil storage tanks, with the proper identification posted, one 50 barrel brine storage tank and a separartor. I noticed no visible spills or leaks. The lease road was in good condition, there are no violations at this time.
1532130042 2014-07-09 Final Restoration No Violations I was on location for a final restoration. I found all production equipment removed. The site was graded. Vegetation was well established. All debris has been removed. The pits are filled and closed. A lease road waivers was obtained from the landowner.
