1037536013 2008-08-15 Production Wells No Violations CASE 07-001 is closed. The well is operated by the Church for their own use.
1171461533 2007-02-08 Production Wells No Violations CASE 07-001 The Ohio EPA received an anonymous complaint reporting an oil spill at this well site. The release estimate was 200 gallons. Reggie Brown responded to the complaint. The spill originated at the wellhead and sprayed oil on the pump jack, wellhead and stock tank. The ground was snow covered and the extent of ground affectment could not be determined. A small creek located 20 feet north of the well did not appear to be impacted. RBDMS indicates that the well has an unknown owner and was plugged and abandoned. The well is in production and is owned by St. Barnabas Catholic Church. The church has owned and used the well for domestic gas for many years. I explained the remediation requirements to the maintenance man. I also asked them to post the required identification and will provide them with required forms to obtain an owner number and transfer the well to them. They do have a contact with D.A. Waldron who will assist them with repairing the well.
1175174080 2007-03-23 Production Wells No Violations Met on site with Bob, the head of maintenance for the parish to review the spill remediation and to discuss getting the well into the churchs name. The spill has been cleaned up and a containment dike has been installed between the well/tank battery and the nearby creek. A blank ID sign was on site and I provided some of the required information. I gave Bob the application for an owner number and the transfer form to get the well into St. Barnabas name.
1264016417 2010-01-13 Production Wells No Violations Status check inspection found no violations. The equipment is in good operating condition with no signs of any leaks or spills.
