1374584432 2013-07-22 Urban Drill / Deepen / Reopen No Violations I was on location to perform the required BOP test after the cementing of the 8.625 casing . Upon my inspection of the Drilling site I observed that the Blow Pit had a layer of oil on the surface of the water. I prevented the Drilling crew from unloading the fluid from the wellbore to the pit. I contacted Dave Matak ( Discovery O&G ) to inform him of the problem. The offending water hauler ( Pander) was contacted and remedied the situation by booming and skimming the pit till the oil was removed. Subsequently, the Drilling crew was allowed to unload the hole ( without incident ) and proceed to drill the cement. The well was configured as a Gas Plunger lift system and consisted of 4.5 casing , 1.5 tubing,
1571462881 2018-06-20 Preliminary Restoration No Violations The access road was graveled to the tank battery and wellhead. Several trees had recently fallen and were blocking the road to the wellhead. I will contact Discovery to remove the trees. The well was operated as a pumping well. A chain link fence surrounded the wellhead. I did not identify any leaks at the wellhead. The tank battery was surrounded with an earthen dike, a locked chain link fence. Urbanized signage was posted and legible. The equipment in the tank battery included two (2) 140 barrel stock tanks, one (1) 35 barrel poly brine tank and one (1) horizontal 2408 separator. The tanks had a lightning arrestor, pressure relief valve, carbon filter and pressure sealing hatches. The load lines were locked and plugged. No violations found during this inspection. The previously disturbed area was stable and vegetated. Preliminary Restoration was passed.
