0156616016 2017-09-20 Final Restoration No Violations The asphalt around the P&A well site was cut in a 30 X 30 square and excavated down approximately six (6) preparing the area for repaving. The was not any sign of previous oil and gas activity. The tank battery equipment had been removed and all that remains were the concrete walls that surrounded the tank Battery. Ralph Gregory ( of Bass Energy )told me that the old owners and new owners of Canal Place are discussing the Landowner Waiver ( form 5 ). Not approved for final restoration during this inspection.
0335494592 2018-05-16 Final Restoration No Violations The wellhead location for the Canal Place # 1 has been paved over and was being utilized as a parking area for the Canal Place complex. The tank battery and wellhead were previously accessed on existing roads within the complex. The tank battery facility was removed and paved. I did not find any surface expression of previous oil and gas activities during this inspection. The flow lines were reported to be purged with fresh water. The Owners of Canal Place have submitted a Landowners Waiver to permit the flow lines to remain in place. The Canal Place # 1 was in compliance to meet the Final Restoration parameters. Final Restoration was passed.
0946638754 2016-02-18 UrbanProduction Wells No Violations The Canal Place # 1 well was completed in 1/94. the well construction was comprised of 360 of 8.625 surface casing, 3895 of 4.5 production casing and 1.5 tubing. A check of the RBDMS data base reflected that the Operator was current in the in the reporting of the production volumes. The well is operated as a plunger lift well. The well is secured with a locked privacy chain link fence topped with chain link fencing serving as a roof. Due to its location in a parking lot landscaping was not utilized. The I.D. signage was in place and current. The well head did not have any leaks or contamination. The well was in an on cycle during my inspection. The tank battery is shared with the Canal Place # 3 - D ( 34-153-2-2828 ). A concrete dike topped with privacy chain link fencing and barbed wire surrounded the tank battery. The fencing was secured with a lock. The tank battery consists of one (1) 140 barrel oil tank, one (1) poly 25 barrel brine tank and a 2408 mechanical separator. My inspection of the tank battery facility did not reveal any leaks or contamination. The I. D. signage was present, current and legible. No violations were identified during this inspection.

1569739551 2017-08-29 Plug / Plug Back No Violations I observed the following during plugging the Canal Place # 1. Nicks Well Plugging (NWP) was the contractor for the plugging of the well. I reviewed the plugging plan with Ralph Gregory ( Bass Energy Co., Inc. ) and NWP. The plan initially had the Clinton plug placed utilizing the 1.5 tubing as a work string. Since the Operator could not demonstrate that the 1.5 tubing was set sufficiently deep enough to be in compliance, I would not allow using that method of placement without confirmation of depth. After removing the 1.5 tubing and demonstrating the 4.5 casing integrity ( annular pressure differential ) I approved utilizing a Bullhead squeeze for placement. Mix water was 76 F and pH was 7. NWP mixed and displaced 38 sacks of Class A cement slurry through the interval 3748 to 3348 using 54 barrels of fresh water. The cement slurry weight was 14.7 ppg as verified by scale. Well was shut in overnight. No violations identified during this inspection.

1938050106 2016-07-29 UrbanProduction Wells No Violations I conducted a status check inspection of this Clinton Sandstone well. My inspection found the wellhead caged with screened, chain link fencing equipped with a plunger lift system. Gauges indicated 220psi on the tubing and was choked back to 73psi on the flow line. The valve on the annulus between the 8.625 surface casing and the 4.5 production casing was closed. The man door was locked and the proper ID and warning signage was posted on the fencing. The well produces to a tank battery to the north that is shared with the Canal Place #3D (PN#2828) and contains two (2) horizontal separators, two (2) 140 barrel tanks, and two (2) 35 barrel poly tanks. The battery has been constructed 4-5 below the grade of the parking lot and is fenced, screened, and secured with locks. The domestic hook-up for Canal Place has been disconnected. The proper ID and required warning signage are posted on the tank battery fence. No violations.
1983089038 2017-08-31 Plug / Plug Back No Violations I inspected the 8.625 surface casing after the previous plugging operations. I found the fluid level down approximately one (1) foot down from grade and static. The hard cement top was found 24 below grade and firm. The 8.625 surface casing and identifying well cap to be installed the week of 9/4/2017. No violations identified during this inspection.
