0020619131 2018-11-06 UrbanProduction Wells No Violations I inspected the Carano Construction #1 as a follow up to a release of fluids due to a flow line failure. The flow line remained under pressure overnight with 120 psi on it without any pressure loss. The scrub brush was cut and removed. The disturbed area was graded, seeded and straw placed on it to prevent any erosion. I examined the catch basins and did not observe any free oil in the basins. I requested that Everflow Eastern ( Operator ) remove the old diapers and booms and replace them with fresh ones. I will observe the catch basins within a week to check on any migration of free oil. I met with Debra Fridley ( Property Manager ), Kristen Traxler ( Safety Manager for the property ) and Sarah Gross Zimmerman and explained the incident and the plan forward.
0601477006 2018-11-14 UrbanProduction Wells No Violations I checked the catch basins were oil had previously been caught by the absorbent diapers and booms and did not find any sign of oil or sheen. There was some brush left at the excavation site that requires removal. I spoke with George Strawn and requested that the brush be removed. George said that the sleeving of the flowline with plastic line will take place on 11/15/2018. After that is completed the well can be placed into production again.
0938757911 2018-11-19 UrbanProduction Wells No Violations After an attempt to push through a smaller plastic line stopped at ~ 90 Everflow excavated the old flow line at ~ 90 and found a 90 degree bend in the flow line. That section of flow line was cut out and the 2 plastic was pushed through the obstruction. Everflow Eastern will continue to push the plastic line to the wellhead an additional 500 feet.

2025658644 2018-11-05 UrbanProduction Wells Violations Noted I received a phone call from Fred Romero ( DOGRM Emergency Response ) regarding a flow line release which occurred at the Carano Construction # 1 at 8:33 am. I responded and was on location at 9:44 am. Everflow Eastern was the Operator of the well. George Strawn was the Operators representative on site. Fred Romeo was also on location. An uncoated two (2) inch flow line connecting the pumping well and the tank battery leaked and found its way into a catch basin approximately 10 away from the leak site. Once in storm sewer system the release traveled approximately 550 feet and was observed in five (5) separate catch basins in the Newport Landing Complex. Everflow put oil absorbent booms and diapers in the catch basins to confine the release. The leak site was excavated and two (2) pipe clamps were put on the line to stop further contamination. The clamps were not to be the permanent patch. George Strawn ( Everflow ) told me that they would slide plastic pipe inside the flow line for a permanent fix. The wells production cycle was for one (1) hour running per day with the fluid volume less one (1) barrel per day ( as reported in RBDMS 2017 Production Volumes ). The flow line was pressured up to 120 psi and observed for one (1) hour without pressure loss. The excavation was filled with clean soil. Repeated examinations was made of the catch basins and while oil had accumulated on the diapers and booms an estimation of the total release to the storm sewers could not be accurately made due to numerous intervening rainfall episodes. Everflow Eastern personnel did not utilize the Incident Notification System ( 1-844-642-2551 ) for the reporting of a reportable release in an Urban area ( OAC 1501:9:8-02-5 ( a ) within 30 minutes of becoming aware of the occurrence. Everflow personnel were notified by Debra Fridley ( Property Manager ) on 11/4/2018 at 2:30 pm. Akron Fire contacted OEPA at 11:58 pm on 11/4/2018.
