0001537717 2006-09-12 Production Wells Violations Noted The Uniontown office received a call from Mr. Dan Bailey on Nola Drive (330-877-6747 work) reporting that the well was leaking. I inspected the site with contract pumper Steve Hilliard and found that the well was leaking gas around the tubing head. The noise from the leak could be heard 300 feet away. The wellhead pressure was 920 psi and we could not safely tighten the tubing ring. I called Mr. Bailey and told him that the safest thing to do right now is to let the well vent to atmosphere. I will monitor the site and note any change in pressure or noise. Environmental Exploration and/or George Kelly still cannot be located.

0483130028 2016-10-14 UrbanProduction Wells No Violations I inspected the St. Nickolas Byzantine Church # 1 to access the severity of the corrosion on the 4.5 production casing. The well is located approximately 600 S W of the City of Akron # 77 ( 34-153-2-3121, completed 1/2/2011 ) This is an active hornizontally drilled Solution Mining well. The lateral is generally in the NWN direction. To the East there are residences within 530 and to the South there are residences 360 away. The well is situated in a grassy field at the end of Rowena Ave. There was evidence of previous scalding events but none recently. A natural gas odor and audible hissing could be detected approximately 30 to 40 away. The well head was fenced and locked. My inspection of the well head indicated a build up a petroleum based material on the 8.625 X 4.5 casing head and iron staining on the equipment and ground.. The 4.5 casing appears to be 30% eroded away. The 1.5 tubing is corroded, but does not appear to leaking during this inspection. Access could not be gained ( locked and rusted closed ) to test for Chlorides.

The well head continues to vent natural gas through the corroded 4.5 casing at the tubing head. The wellhead is fenced and locked. A pressure determination was not made due to lack of gauges. Ground contamination was evident. I will contact the Orphan well program as to the status of this well.
0866364292 2014-12-10 UrbanProduction Wells No Violations I inspected the St. Nicholas Byzantine Catholic Church # 1 with Rick Anderson on 12/10/2014. The well is operated as a plunger lift well and has not reported production in RBDMS since 1996. There is an outstanding Chiefs Order 2006-122 and a previous Notice of Violation # 1827587203 issued. The wells construction is 8.625 surface casing, 4.5 production casing with 1.5 tubing. The tank battery consists of a 100 barrel oil tank, 2408 horizontal separator and is shared offsite with the Boldry Unit well ( 34-153-2-2408 ). The 8.625 to 4.5 wellhead is badly corroded and leaking allowing gas, brine and oil to escape and cause ground contamination. The well is located in an open grass field. There is evidence of previous scalding episodes around the wellhead. The contaminated area encompasses a circular area of 50 to 75 surrounding the wellhead. The wellhead is positioned 400 from an unnamed tributary to the Tuscarawas River. Upon reviewing the previous inspections, it was reported that the gas leaks originated from the 1.5 tubing. Now the 4.5 casing is the major source of leakage and contamination. Attempts to contact Mr. George Kelly and / or any successor to Environmental Exploration have been unsuccessful. The well requires plugging and abandonment.

1418657099 2014-01-13 Orphan No Violations I inspected this well with Mike Schumacher ( Cargill Inc. ) to discus possible Cargill participation in the P & A of this well. Cargills interest in participating stems from their desire to protect their Solution Mining Project # 4. The St. Nicholas Byzantine Church # 1 ( SNBC #1 ) is located 630 southwest of the City of Akron # 77 ( 34-153-2-3121 ). The SNBC # 1 has not reported production in RBDMS since 1996. The tank battery is shared with the Boldry Unit # 1 ( 34-153-2-2408 ) well to the west. The well was operated as a plunger lift operation. The construction of the well is 8.625 surface casing, 4.5 production casing with 1.5 tubing. The production casing was cemented with 100 sacks of cement ( class and yield are unknown ) thus giving a possible cement top around 3465 . The Salina D unit ( which is actively being solutioned mined in the area ) can be found locally at approximately 2800 GL. The Salina interval is not covered in the SNBD #1 well and open to communication from offset operations. The surface surrounding the well head has evidence of past and recent scalding of vegetation. The affected area is approximately 30 to 50 away from the well in various spots. The source appears to be a badly corroded well head. A hissing of gas could be heard during this inspection. Attempts to contact Environmental Exploration personnel have been unsuccessful, as the owner George Kelly is out of state. Mr. Schumacher ( Cargill ) expressed that their interest would be limited to isolating the Salina section to the top of the Onondaga Limestone.
