Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
-296066717 2016-5-19 0:00: PB No Violations
0000PL4321 Preliminary Restoration No Violations Preliminary Restoration done information from Data Point
0296066717 2016-05-19 Plug / Plug Back No Violations I inspected that cement top level in the 8.625 casing and found it to be 50 below the wellhead. The cement top was solid and static. No violations.
1463161384 2016-05-13 Plug / Plug Back No Violations Since the P&A operations are scheduled to begin on 5/I6/2016, I inspected the wellhead area of the JMJ Devolment #1 to identify any surface gas which is associated with the shallow shale formatios in the area. I did not detect any gas surrounding the wellhead. I have asked for the dirt around the wellhead to be excavated so that the 8.625 surface casing is exposed. The well was produced by an electric pumping unit. The wellhead area was secured a chain link fence and the man door was locked. The required I.D. signage was posted on the fence. The tank battery consisted of one (1) 140 barrel oil tank, one (1) 50 barrel poly brine tank, one (1) vertical separator and gas measurement station. The tank battery was surrounded by an earthen dike and secured by a locked chain link fence. The gated lease road was graveled to tank battery. The required I.D. signage was posted on the tank batterys fence. There was a 2 line laying on top of the ground, previously used to deliver solvent to the well. I told Keith Moore that would have to be removed prior to final restoration approval. No violations identified during this inspection.
1463492370 2016-05-17 Plug / Plug Back No Violations Moore Well Service was removing the 2.375 tubing from the 4.5 production casing in preparation for plugging and abandonment. The placement of the cement plugs was rescheduled for 5/18/2016. No violations identified.
