0599870389 2018-10-17 Production Wells No Violations I did a compliant follow-up inspection for Complaint #5639. The well is equipped with a pumpjack with no belts attached to the electric motor. The switch in the electrical box is in the off position. The flowline valve from the tubing is closed. The flowline valve from the annulus is open. The tank battery consists of two steel production tanks, a steel brine tank and two separators. The well is legibly identified. Both the well and tank battery are fenced and gated with locks. I met with Mrs. Debbie AsFoura (Complainant), she advised me her complaint concerned a downed powerline going to the well which has since been corrected. I advised her I would like to close the complaint and she agreed with closing complaint. I gave her my contact information and I am recommending that complaint #5639 be closed.
1482498075 2016-12-22 UrbanProduction Wells Violations Noted The Baxter # 1 was completed in 7/84. The Operator was current in the reporting of production volumes. The lease road offered adequate access to the tank battery and wellhead. The tank battery consisted of two (2) 100 barrel tanks ( with charcoal filter ), two (2) vertical separators and one (1) steel 50 barrel brine tank. The tank battery was surrounded by locked fencing and an earthen dike. The equipment spacing was compliant. The load lines were secured with plugs. I did not identify any leakage or contamination during this inspection of the tank battery. The well was transferred to a new Operator effective 10/11/2016 and the I D signage did not reflect this change. This in violation of 1501:9-9-05 (10) OAC, which refers to the identification of producing leases. Compliance with this rule shall be met with the placement of signage reflecting the current information immediately. The well was operated with an electric pumping unit. The wellhead area was secured with locked fencing. I did not identify any leaks in the wellhead area. The electric line found to be either lying on or hanging a few feet off the ground for approximately 220. Several support poles had failed allowing the energized line to droop. At the wellhead the electric line was strung on the fence. This condition is in violation of 1501:9-9-05-E (5c) OAC which addresses, in part, equipment shall be maintained consistent with reasonably prudent operations for the assurance of public heath or safety. Compliance shall be met with the replacement of supporting poles and rehanging the electric line or other such method approved by the DOGRM Inspector immediately.
