1129512040 2005-10-15 Production Wells No Violations At 23:15 on October 14, I received a call from Bill Kinney with Summit Petroleum reporting an explosion and fire at a tank battery on Barlow Road just east of the intersection with Terex Road. The explosion and fire resulted in the death of Mr. Charles Hott. Mr. Hott is believed to the son of the royalty owner. Mr. Kinney was on site at the request of the Hudson Fire Department. At approximately 21:30, Mr. Hott entered the tank battery, climbed the ladder and looked into the thief hatch to gauge fluid level in the 100 bbl tank. At this point he either looked in the tank with an open flame or lit a cigarette. The spark ignited vapors in the tank. A second person, injured in the explosion was transported to the hospital. Hudson Fire was on site to extinguish the fire and Hudson Police were on site to investigate the death. Moore Well Service was on site to drain the 100 bbl tank.
1129915240 2005-10-17 Production Wells Violations Noted Follow-up inspection after the fatal incident on 10/15. The affected tank battery must be dismantled and inspected, with equipment, valves and gauges replaced as necessary. The tank battery must be cleared of vegetation and the brine vault must be removed. Affected soil in and around the tank battery must be removed and properly disposed. The site must be inspected prior to turning the well back into production. The tank battery gate and thief hatch must be closed and secured at all times.

1394637998 2014-03-06 UrbanProduction Wells Violations Noted I inspected the Hott - Ahrens # 1 production facility on 3/6/2014. I found that while Pine Top has reported sporadic production since 1984, it is barely in compliance with O.R.C. 1509.062 ( Minimum Production Levels ). The tank battery consisted of a 140 Bbl. Stock tank and a Westerman 2408 mechanical separator. A Rockwell rotary meter was mounted on the separator to measure the natural gas volume produced from the well ( meter reading @ 4727 ). The load lines , on the tank, ( both oil and brine ) were not locked and unsecured ( by bull plug or flat plug ). The tank battery lacked legible required identification. The containment dike surrounding the tank battery is inadequate to contain any breached fluids. The well head consists of just 4.5 casing that is normally associated with a swab well configuration. The fence surrounding the well head was not secured. A gap in the fencing existed at the time of my inspection. The well head also lacked the required identification. A domestic house tap is present, but disconnected. To bring the Hott - Ahrens # 1 into compliance Pine Top Inc. must install the required Identification at both the tank battery and well head, repair the breach in the well head fencing, place locks and plug the the load lines on the stock tank, rebuild the containment dike surrounding the tank battery.
1554386645 2019-04-04 Production Wells Violations Noted My inspection found the compliance issues cited in the Compliance Notice issued on 03-12--inch pick-up line with a plug and lock at the production tank. The well is a swab well. The tank battery consists of a steel tank and a horizontal separator. A large tree branch fell on the west side of the tank battery fence. The well has zero production reported from 2014 through 2016. No Annual Production Reports have been submitted for 2017 and 2018. The well is idle. Consequently, the well is in violation of Revised code 1509.062 and shall be plugged in accordance with Ohio Revised Code section 1509.12 (B). The well owner/operator shall submit a written plugging schedule to Shawn Murphy at 3575 Forest Lake Dr., Ste. 150, Uniontown, Ohio 44685 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

--up line on the production tank is unsecure with no plug or lock. There are two (2) identification signs with no well information. The access road is stable. The RBDMS database shows Pine Top Inc. has filed zero annual production for 2014 and 2015. Photos were taken and are available upon request.

Pine Top Inc. still needs to: Post the proper identification sign. Repair the breach in the fencing at the well. Secure with fencing at the well and tank battery with locks. Rebuild the containment dike around the tank battery. Cut down all of the overgrown vegetation at the well and tank battery. -up line with a plug and lock at the production tank.
