1154299944 2013-02-11 Orphan No Violations Uniontown office received a call from Jim Levin ( MRT Properties ) concerning a Gas Odor reported by one of his Tenants ( Roto-Rooter ) coming from a catch basin behind their building. DOM/EOG were called and had cleared their lines and meters from any leaks. Mr. Levin acknowledged a oil and gas well ( E & L Realty # 1 , 34-153-2-0681 ) being P & A at 887 Hampshire Suite A on 9/ 24/86 by KST Oil and Gas. I arrived on location @ 1417hrs. and met Mr. Jim Levin who showed me where he thought the well had been and the offending catch basin ( approx. 200 away ). I detcted gas in the catch basin, but not in the other catch basins that closer to the plugged well location. The gas I smelled at the catch basin behind Roto-Rooter had an odorant smell to as was confirmed by Norburt Lowder ( who assisted ). I did not detect any gas odor around the suspected P & A well, but this could have been caused by the frozen ground conditions.With the gas diminishing and no indications of gas odor inside the building at 887 Hampshire Road Suite A. I left location @ 1500hrs.. I was to return on 2/12/2014 to probe deeper around the suspected area.
1165570295 2014-02-12 Orphan No Violations As a follow inspection to the previous days activities, I was on location @ 0845hrs. and met with Mike Poulson ( Leak Detection, DOM/EOG ) to gather more gas readings in order to determine source of the gas odor. Starting with the catch basin @ Roto-Rooter he obtained a reading of 350 - 529 ppm, all other catch basins were cleared of any natural gas readings. I used a probe to get below the frost line in order to sniff any gas around the suggested location of the plugged E & L Realty #1 well. In the probe hole a reading of 941 ppm natural gas was read on DOM/EOG meter. I read on my meter a reading of CH4-1.0, LEL- 7. Mr. Levin showed me a letter dated 11/15/1989 from KST that they observed gas perculating at a small rate from the casing. A PVC cap was placed on the casing with slots cut into to allow venting with the further recommendation that the well be connected to a vent system rising above the proposed building roof line with a small vault containment. At 1100hrs. the Leak Detection personnel located a leak in the DOM/EOG distribution line servicing the building and determined that was the source of the gas odor reported. With that issue corrected, I advised Mr. Levin that the venting well will need to be addressed. Either by replugging the well or vaulting the well and running vent lines above the roof line. The area of the wells location is located in a bermed landscaping bed and is approximately located 27 from the SE corner of the building located at 887 Hampshire Road. There has not been reported gas odors in that building nor has the Mr. Levin reported any noticable scalded or dead vegetation in the vicinity. He asked if the excavation and exposing of the well could wait until the ground thaws. I advised this would be agreeable, but if the situation worsens, ie: gas odor in building, outside and general deterioration of the situation, in the opinion of DOGRM, immediate measures would be prescribed.
1424820559 2015-11-04 Final Restoration No Violations I conducted a final restoration inspection of this site. The wellhead and tank battery equipment have been removed. The wellsite has been restored and had no erosion issues. The lease access has also been restored. There is a plug report on file in the state dateabase that shows the well to have been plugged in 1986. Final restoration can be passed.
