0369992783 2014-10-23 Preliminary Restoration No Violations I was on location to check the status of equipment and the Wildcat drilling rig being removed from location. The frac tanks are being staged to be hauled from the location. The rig has been packed up and is going to be moved to a location in Portage county. They plan on solidifying the pits within the next 7 days.

Currently the pits have been closed and the well has been fraced. Six frac tanks are on location and the wellhead is currently shut in. When weather permits the equipment will be removed from the location.
0508221016 2014-10-15 Casing String Witnessed No Violations I was on location to witness the cementing of the 11.75 diameter surface casing installed to 466 by Wildcat drilling. A 15 diameter hole was drilled to 497. Petroset pumped 300sks of Class A 3% CaCl cement and displaced it with 55bbls of fresh water. A total of 15bbls of cement were returned to surface. The rig will wait on cement for 8hrs before drilling ahead.
0628437538 2014-10-16 Drill / Deepen / Reopen No Violations I was on location to witness PetroSet Cementing Services cement the intermediate casing. Wildcat drilled a 11.25 Hole #3 to 933 from rig floor (RF) and set 8.625, 23 lb/ft casing at 879 RF with 5 centralizers. The average joint length was 44.25. Rig Floor was 12 above ground level. PetroSet rigged up and established circulation. The operator mixed and pumped 200 lbs of gel and 50 lbs of cotton seed. Next, the operator pumped 300 sacks of Class A cement with a yield of 1.18. The cement was displaced to 859 using 55 bbls of water. I witnessed 18 bbls of cement returned to surface.
0668536186 2015-04-21 Preliminary Restoration No Violations I was on location to check the status of preliminary restoration for this well. The gate has been installed on the lease road and contains identification. The pumpjack well is installed but not producing at this time. The tank battery has been installed with a 140bbl production tank, poly brine tank, horizontal separator and sales line. The tank battery is surrounded by a fence and a identification sign is being installed. The well and tank battery are surrounded by a board on plank fence. The location has been graded but vegetation has not been established. At this time preliminary restoration has not been passed.
1070547766 2014-10-27 Preliminary Restoration No Violations I was on location to check the status of the closing of the pits. 6 loads of solidification material has been brought onto location and added into the back pits. The reserve/blow pit is not going to be solidified today, instead it will be partially solidified and dredged out to haul some of the material to a landfill due to the inability to properly cover the put with soil. Final solidification will occur tomorrow 10/28.
1412694655 2014-10-07 Surface Facility Construction No Violations I was on location with supervisors Tom Hill, Rick Anderson and inspector Josh Schiering to witness the digging of pit test holes to determine location stability. Ted Hill of Ohio Valley Energy (330-719-0100) was on location to explain the process of the pit digging and to locate the area of the proposed pits. An approximately 10 test hole was dug with a backhoe and the entire test hole consisted of a majority of sand and large rocks. At this time the steel pit requirement listed in the special permit conditions have been waived. They are only waived if the following steps are taken during the construction and use of the pits. 1) The pits and pit walls must be dug as to not exceed the angle of repose for the sand and gravel they are dug in. This will prevent the walls from collapsing into the pit. 2) The pits must have a bentonite layer applied to the bottom and as much of the pit walls as possible to form an additional layer of protection. 3) The pits must be double lined to provide an additional layer of protection to prevent pit waste from penetrating the liner system. A continue spill prevention apron must still be used on the flow ditch and discharge lines to the pits. The drilling rig shall also have secondary containment under the rig during operation to prevent any contamination. Construction will continue through the week and into next week. The well cannot be spudded until the water well results are submitted.
1500242045 2015-05-06 Completion Testing No Violations I was on location to check the status of this well being swabbed by a service rig. The well was loaded with water to kill it in order for the tubing to be reset in a new location in the well. The well is going to be initially run as a plunger lift well until it is no longer feasible. There are no violations at this time.
