0092692138 2015-02-05 UrbanProduction Wells No Violations I was on location to follow up to the odor complaints #4872 and #4870. The well has had rods and tubing installed and a pumpjack installed. The well is pumping 24hrs a day into the production tanks. The production tanks have been installed with charcoal filters and closing latches to prevent and capture odor. There was no odor or gas detected on location using a Sperian PHD 6 gas meter. At this time both complaints can now be closed. There are no violations at this time.
0158324344 2014-10-09 Casing String Witnessed No Violations I was on location to witness the cementing of the 8.625 diameter intermediate casing installed by Pep Drilling to 1201 with 8 centralizers. A 11 diameter hole was drilled to 1222. Universal pumped a 290sk 50/50 6% gel and 2% CaCL lead cement and a 100sk Type 1 3% CaCl .25lb/sk flake tail cement slurry and displaced it with 75bbls of fresh water. Circulation was obtained during the entire job and 40bbls of cement were returned to surface. There are no violations at this time.
0285266561 2015-05-05 Production Wells No Violations I was on location to check the status of the Chipco LLC service rig making a pump change on the well. The pump in the well has become full of sand and inoperable. The pump is going to be pulled from the well and replaced with a new one from Sutton Supply. The well will be put back into production when the pump change is complete.
0333280684 2014-10-07 Drill / Deepen / Reopen No Violations I was contacted by Enervest regarding the permit depth of the 11.75 diameter surface casing. The permit calls for 710 (50 below the base of the Big Injun). Off set wells #25552, 25258, 25351, 25259, 25217, 25357 and 24584 in Stark show surface casing points ranging from 400 to 575, which as listed is 50 below the base of the Big Injun. The offset wells for Enervest #25351, 23870 and 25393 show the base of the Big Injun at approximately 350 based off of logging equipment. It has been determined that setting 504 of casing will properly seal of the Big Injun in this area while avoiding cross contamination from brine in the Berea Sandstone.
0399427849 2014-11-06 Drill / Deepen / Reopen No Violations I was on location to check the status of the Chipco LLC service rig running cementing tools and packer into the well through the Weatherford wellhead. Currently the operation is shut down because Weatherford did not load the proper wellhead and flanges onto the hot shot trailer for shipment to location. The proper equipment is being gathered and loaded but will not arrive until later tonight. The cementing of this well will now be pushed back to 11/7/14.
0410522213 2015-05-08 Production Wells No Violations I was on location to check the status of trying to free the stuck rods in the well. Appalachian Well Services was on location to try to determine a free point in the 2.375 diameter tubing. There was no definitive free point found during the bond log. A tight spot was found in the tubing at approximately 2500. The Chipco service rig is going to run .875 diameter rods into the well to attempt to break free the mud anchor in the well. Work will continue Monday to try to free the rods in the well.
0465277482 2015-10-16 Production Wells No Violations I was on location to perform an inspection regarding complaint #5145. The electrically driven pumpjack well was producing during inspection. The annular pressure on the 8.625 diameter and 4.5 diameter casings were showing 0 psi on the annular pressure gauge. The tank battery and wellhead do not have any visible leaks or issues. The well and tank battery are properly identified and contain all required locks and plugs. There were no violations at the time of inspection.
0653201591 2014-11-25 Completion Testing No Violations I was on location to check the status of the flow back for this well. The well is being flowed through a gas buster into 500bbl frac tanks. There are some gas shows during the flow back. The location has Enervest representatives on location and have the flow back under control. There are no violations at this time.
0783416250 2014-10-23 Drill / Deepen / Reopen No Violations I was on location to check the status of Pep drilling. Currently the rig is tripping out of the hole to stand back the drill pipe. A 6.5 diameter roller reamer is on its way to location to be run in the hole and ream the hole clean and remove the dogleg present in the lateral. Depth of the well is currently 6695 of total 7500.
0852005740 2014-11-07 Casing String Witnessed No Violations I was on location to witness the cementing of the tapered 5.5/4.5 diameter production casing by Universal Well Services. A cement port collar was installed in the casing at 4000 and 2 3/8 diameter tubing was stabbed into the port collar. Universal pumped 425sks of Unifill Light with 3%CaCl and 6% Gel cement and 100sks of 50/50 2% gel, 10% salt tail cement. The cement was displaced with 15.6bbls of freshwater. A reverse circulation of 100bbls of brine and 10bbls of fresh water were used to remove any residual cement in the casing after the cement port collar was closed shut. The frac stack will be installed and the well prepared for stimulation. A track hoe was on location solidifying the reserve pit at the time of the cement job. The pit was properly solidified and had no free liquid and was in a firm solid state. The working pits are going to be closed and solidified tomorrow. There are no violations at this time.
1042559616 2015-12-09 Production Wells No Violations I was on location to check the status of working over this well. The Chipco rig is pulling the rods from the well after they parted in the wellbore. A new set of rods are being installed and are made of a heavier gauge steel to prevent future rod parting.
1058627506 2014-09-30 Surface Facility Construction No Violations I was on location to witness the installation of the water tight cellar for this well. A cellar was dug and a 5 diameter corrugated pipe was installed in the cellar. A flow ditch was dug, lined and flow pipe was installed. The cellar was backfilled with gravel and the inside of the cellar filled with cement. The pipe and backside of the corrugated pipe were also filled and topped off with cement. The flow pipe will be buried once the pits have been dug. There are no violations at this time.
1121070433 2014-12-08 UrbanProduction Wells No Violations I was on location to follow up to complaint #4872 regarding the strong smell of natural gas coming from this well location. A 5000bbl per day separator has been brought onto location and installed. The separator is going to flow all the oil to the production tanks, equipped with charcoal filters, and the water to the open top tanks. No oil is currently being flowed back yet, it will take several hours for the jet pump to catch up. There is no noticeable smell of gas at this time. The well will be monitored for 24 hours and the location will be inspected on 12/9/14 to check the status of odor control.
1159784868 2014-11-04 Completion Testing No Violations I was on location to check the status of pit closure for this well and the preparation for cementing of the production casing and staging of frac tanks for stimulation. Currently the blow pit has been solidified and covered with topsoil. The location is be leveled out and frac tanks are arriving on location and being loaded with water for the frac. They will be cementing the 5.5 diameter production casing tomorrow 11/5/14 with Universal Well Services. There are no violations at this time.
1220195318 2014-12-09 UrbanProduction Wells No Violations I was on location to follow up to complaint #4872 regarding the strong smell of natural gas coming from this well location. The new separator has been installed and the oil is being produced into the production tanks. Only water is flowing into the open tank, and there is no noticeable odor on the location or the surrounding area. The well will be monitored until the flow back is done.
1226313074 2014-11-05 Drill / Deepen / Reopen No Violations I was on location to check the status of the Chipco LLC service rig running cementing tools and packer into the well through the Weatherford wellhead. Currently the operation is shut down because Weatherford did not bring the proper wellhead and flanges to location. Because of this the cementing tools and packers could not fit through the wellhead and be installed in the wellbore. The proper wellhead and flanges are going to be loaded and shipped to location by Weatherford. They expect the tools to arrive and cement the job late 11/6/14.
1412864435 2014-10-08 Casing String Witnessed No Violations I was on location to witness the cementing of the 11.75 diameter surface casing installed by Pep Drilling to 504 with 4 centralizers. A 17.5 diameter hole was drilled to 73 and a 14.5 diameter hole was drilled from 73 to 525. Universal pumped 377sks of Type 1 3% CaCl .25lb/sk flake cement slurry and displaced it with 59bbls of fresh water. Circulation was obtained during the entire job and 25bbls of cement were returned to surface. There are no violations at this time.
1414765350 2014-10-30 Drill / Deepen / Reopen No Violations I was on location to check the status of Pep Drilling installing 4.5/5.5 diameter production casing into the wellbore to 7580. The rig has currently landed the casing at bottom and is circulating on the well to clean it up. There were no tight spots while running the casing. The possibility of encountering tight spots was a concern due to challenges maintaining a straight and level lateral while drilling. The rig is currently rigging down and will be moving to the Marks 2S location.
1415121831 2014-11-03 Preliminary Restoration No Violations I was on location to check the status of pit solidification for this well location. The pit is currently solidified but does have some free water on top of it. Another load of solidification material will be brought onto location and used to fully solidify the blow pit. The working pit will be closed following the cementing of the 5.5 production casing on 11/5/14. There are no violations at this time.
1417787410 2014-12-04 Completion Testing No Violations I was on location to follow up on complaint #4870. The complainant reported a heavy gas smell in the neighborhood. The odor is coming from the well near the intersection of Joliat and Easton. Complainant is also concerned that her neighbors had their water wells tested by the company prior to drilling, but the company did not approach her to have her water tested. The well if currently being flowed back through a gas buster and into frac tanks on location. The flow back is going through a separator and the natural gas is being sold through the sales line. The smell of natural gas is coming from the off gassing of the oil while it is flowed into the frac tanks. Enervest believes that this well could still need to be flowed back for a few weeks. The complainants water well was not tested because the residence is approximately 2250 away from the wellbore, outside the testing distance. The gas odor did not register on a Spherian PHD6 gas meter and is not an explosion hazard at this time.
1418046934 2014-12-05 UrbanProduction Wells No Violations I was on location to follow up to complaint #4872 regarding the strong smell of natural gas coming from this well location. The well is currently set up with a jet pump in the well to flow back the well. The jet pump is set in the well and running at up to 80 barrels per minute to create a vacuum on the formation to pull production oil and frac fluid into the wellbore. The flow back is pumped through a three stage separator. The gas from the separator is sold through the sales line and the flow back is going through a gas buster and into an open top frac tank. Oil and frac fluid, along with residual gas enters this frac tank and off gasses into the atmosphere causing the natural gas smell that the complaint cited. It was discovered that the current separator is not large enough for the capacity being processed. A larger three stage separator has been found and will be brought onto location and installed. This separator will send the gas to the sales line, and the oil from the well to the production tanks which are equipped with charcoal filters to capture the vapors from the oil. This separator will be installed on 12/8 and evaluated for its ability to prevent and stop the odor complaints.
1421242433 2015-01-13 Production Wells No Violations I was on location to follow up to complaints #4870 and #4872 regarding the odor complaints for this well. The jet pump is still running as the well is still flowing strong. The jet pump is going to be on the well for at least one additional week before the well is rodded up and the pumpjack installed on the well. The three phase separator is selling the gas from the well, sending the oil to the stock tanks with charcoal filter and the flowback water to the open top gas buster. There is no noticeable odor at this time on location.
1445862383 2015-10-26 Work Over Reconditioning No Violations I was on location to witness the workover on this well. The rods and 2.875 diameter tubing was pulled from the well and a 150bbl gel sweep was pumped into the well. The gel sweep was used to pickup any residual sand in the wellbore and force it back into formation. A new pump will be installed on 10/26 and the well will be put back into production.

1653869619 2014-10-29 Drill / Deepen / Reopen No Violations I was on location to check the status of Pep Drilling this well. The well has been drilled to total depth of 7580 but is currently being reamed. The rig is going to make several reamer runs to be able to remove any edges and smooth out the doglegs present in the well. They will ream until they are confident that casing can be run and installed in the well without it getting tight.
1866156413 2015-01-29 Production Wells No Violations I was on location to check the status of installing the pumpjack on this well. Currently tubing and rods have been run into the well and a crane is on location installing the pumpjack. The pumpjack will be installed and operational within the next 2 days and will begin resuming production as soon as soon as the work is completed.
1927327439 2015-02-02 Production Wells No Violations The pumpjack has been installed and is fully operational. The annular space on the well was missing a pressure gauge, Enervest installed one and also closed the open hatch on the 210bbl production tanks. The frac tanks are being unloaded and moved from location. Work on removing the frac tanks has slowed due to the several inches of ice on location. The well pad will be restored fully when the ice thaws and the location can be seeded.
