2015-05-05 H2O 0 149609 Gal 0 Producers Service Corporation A Producer's acid pump truck was used to achieve a break on this well at 1341psi. 600 gallons of 15% acid was used to achieve break and the well was treated with 37.6bbls of fresh water. Three Producer’s Frac trucks were on location to treat the well. The average treatment rate was 15.9bbls/min at 3780psi. A higher treatment rate could not be achieved. It was believed that the well was not completely dry when the acid was dropped and was diluted limiting the breakdown. The full 149,609 gallon

0066161424 2018-11-15 Production Wells No Violations I inspected this well. The well is equipped with a pumpjack. The tank battery consists of two steel production tanks. Two plastic brine tanks, a horizontal separator and a meter. The tank battery is shared with well # 151-25873. The well is legibly identified. No compliance issues are cited at this time.
0128550114 2015-05-28 Preliminary Restoration No Violations I was on location to check the status of restoration for this well. A contractor was on location performing dirt work and reclaiming the drilling location. A pumpjack has been set on the well today. I explained to the operator that a mechanical pressure relief valve and pressure gauge must be installed on the 8.625 diameter surface casing and production casing annulus to monitor pressures. Flowlines for the well have been installed and are waiting to be covered by topsoil. I will follow up when reclamation is complete to pass preliminary restoration.
0455731570 2018-11-15 Preliminary Restoration No Violations My inspection found the pits have been filled and the land restored around the well and tank battery locations. There are no signs of active erosion at either location. The preliminary restoration is compliant with Ohio Revised Code 1509.072(A). The preliminary restoration is passed.
0899356619 2014-11-12 Surface Facility Construction No Violations I was on location to check the status of building of the drilling location for this well. Palmer & Sons are on location with a bulldozer and a backhoe stockpiling topsoil and leveling the location. Poulson Drilling will move to this location following the completion of the RTB 2 well.
1197771195 2014-11-14 Drill / Deepen / Reopen No Violations I was on location to witness the drilling of this well by Poulson Drilling. The well is currently being drilled on fluid with a 14.75 diameter tricone bit at 250. 15sks of starch had to be added to the drilling fluid to prevent fluid losses while drilling. The rig will drill ahead to a target depth of approximately 675. There are no violations at this time.
128550114 2015-5-28 0:00: PL No Violations 1.00
1416860406 2014-11-24 Drill / Deepen / Reopen No Violations I was on location to witness the testing of the blow out preventer (BOP) for this well. The BOP was pressured up to 300psi and held for 30 minutes. The pressure fell to 275psi which is an acceptable amount. The rig will now drill ahead on air with their 7.875 diameter drilling bit.
1416945450 2014-11-25 Drill / Deepen / Reopen No Violations I was on location to check the status of Poulson drilling this well. Currently the rig is drilling their 7.875 diameter hole at 2993. The rig will drill to the top of the Big Lime at approximately 3300 and shut down for Thanksgiving. The pits and rig will be fenced off and locked during the stoppage of drilling.
1684449301 2014-11-22 Casing String Witnessed No Violations I was on location to witness the cementing of the 11.75 diameter surface casing by Universal Well Services. Poulson drilled a 14.75 diameter hole to 655. Poulson installed 640 of the casing with 6 centralizers. Universal pumped 400sks of Type 1 cement with 3%CaCl and .5lb/sk flake. The cement was displaced with 74bbls of fresh water. A total of 18bbls of good cement returned to surface. The rig will wait on cement for 8 hours before breaking off their head.
1817634415 2014-12-02 Casing String Witnessed No Violations I was on location to witness the cementing of the 4.5 diameter production casing. 5466 of 4.5 diameter production casing was installed with 10 centralizers in a 5530 7.875 diameter hole. Universal pumped 235sks of Unitropic 10-4-2 cement slurry and displaced it with 86bbls of fresh water. Circulation was obtained during the entire cement job. Poulson Drilling will now be moving out of the county.
455731570 2018-11-15 0:00 PL No Violations 0.50
