0237643511 2013-02-21 Plug / Plug Back No Violations Pep Drilling Rig # 10 incurred a Downhole fire while drilling at a depth of 5251 KB as determined by loss of penetration rate and borehole samples. Upon removing the drillstring from the wellbore it was determined that the bit, bit sub and two drill collars were lost in the hole. Operator made the decision to set a Kick-Off plug at 4777 KB and sidetrack the damaged wellbore. Pep Drilling picked up the drillstring and went back into the wellbore to a depth of 5177 KB. Universal Well Services broke circulation and pumped 15 Bbl of gel sweep followed by 115 sacks of intermediate kick plug cement ( 1.18 yield, 16.0 lbs/gallon density. The resulting plug was set from 4777KB to 5177 KB. Water temp was 38 and pH was 6.5. Pep Drlg tripped back into wellbore and tagged the top of the kick-off plug at 4784 KB. DDC was the directional contractor used. Utilizing a Timed drilling and Sliding method the wellbore was drilled a total depth 5434 KB on 3 / 1 /2013. The running and cementing of the 4.5 production string followed with no incidents.
0274459795 2018-03-05 Production Wells No Violations I did a status check inspection for this well. The well is equipped with a pumpjack. The tank battery consists of a steel production tank. A plastic brine tank, a vertical separator and a meter. The tank battery and brine tank are shared with the tank battery for well #151-25826. The well is legibly identified. No compliance issues are cited at this time.
1737316020 2015-12-31 Preliminary Restoration No Violations I was on location to perform a preliminary restoration inspection of this well. The lease road is graveled and stable. The electrically driven pumpjack well is identified, contains required gauges and does not have any visible leaks. The tank battery is shared with well #25826 and is identified and surrounded by a dike. The tank battery contains two (2) 210bbl production tanks, two (2) vertical separators, poly brine tank and two (2) chart boxes. The location has been reclaimed and vegetation has been established. The wellhead, tank battery and roadway all conform to spacing requirements. At this time preliminary restoration has been passed.
