0440006522 2012-11-07 Drill / Deepen / Reopen No Violations I inspected the drilling operations at the Rummell Farms 3 (34151257840000) site on November 7, 2012. The drilling contractor for this well is PEP Drilling Inc., rig number 10. My inspection found the access road is graveled from Rummell Avenue to the well site. The working pit and blow pit are located to the west of the wellhead; soil berms are being utilized to isolate the pits. The working pit has 4 feet of free board and the blow pit has 7 feet of free board. The pits are cordoned off with safety fencing. Silt fencing has been installed along the eastern and northern perimeter of the well site to prevent migration of sediment into the small pond directly east of the wellhead. The silt fencing is in place and in good condition. PEP drilled an 11 inch borehole to a depth of 952 feet and was setting 8.625 inch casing and cementing to surface. The blow out preventer and rotating air head were not in place, but PEP did use an air body attached to the 11.75 inch conductor casing to drill on air and fresh water. Jeff is the driller on duty. A copy of the permit is located in the dog house.

The tank battery consists of a two 210 BBL production tanks, a 50 bbl poly brine tank, and two vertical mechanical separators. The tank battery is shared with well #25827 The proper identification is located on the tank battery. The lease road is in stable condition. There are no violations.

The tank battery consists of two steel production tanks, two vertical separators, a plastic brine tank and a meter. The tank battery is shared with well #151-25827. The well is legibly identified. No compliance issues are cited at this time.

The working pit and blow pit are located to the west of the wellhead; soil berms are being utilized to isolate the pits. The working pit has 7 feet of free board and the blow pit is empty. The pits are cordoned off with safety fencing. Silt fencing has been installed along the eastern and northern perimeter of the well site to prevent migration of sediment into the small pond directly east of the wellhead. PEP drilled a 17.5 inch borehole to a depth of 136 feet and was setting 11.75 inch conductor casing and cementing to surface. The blow out preventer and rotating air head were not in place; PEP is set up to drill on fluid. Herb is the driller on duty. A copy of the permit is located in the dog house.
1354559989 2012-11-29 Completion Testing No Violations I inspected the hydraulic fracturing operations at the Rummell Farms 3 (34151257840000) site on November 29, 2012. My inspection found Superior Well Services was contracted by Enervest Operating LLC to complete the hydraulic fracturing of the well. The job started at 12:43 and ended at 14:18 with an average operating pressure of 2732 psi. The average slurry rate was 47.4 bbls of water per minute. A total of 170,781 gallons of water was used to complete the operation.

The drilling contractor for this well is PEP Drilling Inc., rig number 10. My inspection found the access road is graveled from Rummell Avenue to the well site. The working pit and blow pit are located to the west of the wellhead; soil berms are being utilized to isolate the pits. The working pit has 4 feet of free board and the blow pit has 6 feet of free board. The pits are cordoned off with safety fencing. Silt fencing has been installed along the eastern and northern perimeter of the well site to prevent migration of sediment into the small pond directly east of the wellhead. The silt fencing is in place and in good condition. PEP installed the blow out preventer (BOP) and rotating air head and was preparing to test the BOP. The BOP held 400 psi for 30 minutes without a loss of pressure. Jeff is the driller on duty. A copy of the permit is located in the dog house.
1542373066 2018-11-15 Preliminary Restoration No Violations My inspection found the pits have been filled and the land restored around the well and tank battery locations. There are no signs of active erosion at either location. The preliminary restoration is compliant with Ohio Revised Code 1509.072(A). The preliminary restoration is passed.
