0047938502 2012-03-22 Drill / Deepen / Reopen No Violations I spoke with the driller, Wesley Clark, and he advised me they spudded the well on 03-21-2012. The geolograph is at 350. The pit liners are intact and the fluid levels are acceptable. No violations are noted at this time.
0069762463 2016-03-02 Production Wells No Violations I was on the Seeman #3 tank battery pad to observe the remediation after a oil release reported on 2/29/2016. Barnett Excavating, Inc. were the contractors to remove all the contaminated gravel and subsoil and haul the material to Countywide Landfill in East Sparta, Oh. With the equipment removed the previous day the gravel was completely removed and loaded into the trucks for disposal. The subsoil was removed ( where contaminated ) and placed into trucks for disposal . A total of 34.5 tons of contaminated material were removed ( weight tickets were supplied by Operator ). After reviewing the excavation for remnant contamination I approved the clean up. New gravel was brought into and the base of the tank pad was reconstructed. A 2 X 3 sump was left open to monitor any migration of contaminated fluids. The equipment was replumbed and pressure tested. Approval was given to place the Seeman # 3 back into production.
0413000849 2015-05-04 Preliminary Restoration No Violations I was on location to perform an inspection of this production well. The lease road is graveled and stable. The tank battery is shared with well #25084 and contains a 210bbl production tank, 100bbl production tank, two (2) 50bbl brine tanks, two (2) horizontal mechanical separators, a dike and proper identification. The well site contains the required locks, plugs and gauges. The gas driven pumpjack well is in good mechanical condition, contains proper identification and is fenced in. This well is listed presently as Drilling. This well is in production, but no production numbers are available. I have contacted Rusty Morrison of Atlas to check into why the production numbers have not been submitted for the well, or if the current status of the well is incorrect in the state database.
0914724896 2016-03-01 Production Wells No Violations I was on the Seeman #3 location to review the progress of the remediation of the the oil spill confined in the tank battery. Atlas personnel had disconnected one 210 barrel oil tank, one (1) 100 barrel oil tank ( Seeman #2 well ), two (2) 50 barrel brine tanks and two 2408 mechanical separators, all the connecting piping and manifolds. Activity was shut down until the next day when excavation would begin.

1456932500 2016-02-29 Production Wells No Violations I received a phone call at 0750 hrs. from Lisa Raffle ( Atlas Energy, L.P., Regulatory Compliance, 724.579.2320 ) notifying me of an oil release at the Seeman # 3 tank battery. The well was completed in 3/2012 as an electrified pumping well. The timer failed to shut down the pumping unit at the end of its scheduled cycle on 2/25/2016 and was discovered to have over flowed the 210 barrel oil storage tank on 2/29/2016. The well tender gauged the tank ( 2/25/2016 ) at 12 10 and was discovered at 0700 hrs. on 2/29/2016 to have breached the 210 barrel oil tank. It was estimated that 11 - 12 barrels of fluid were spilled out of the 210 barrels oil tank. The tank battery encompassed a 40 X 80 area and was surrounded by an earthern dike. The clayish soil under the tank pad was topped by 4 to 6 of pea gravel. The tank battery consisted of one (1) 210 barrel oil storage tank, one (1) 100 barrel oil storage tank, two (2) 50 barrel brine tanks and two (2) 2408 mechanical separators. This is a common tank battery with the Seeman #2. I arrived at 0840 hrs. and Sutton Pump and Supply was steaming the tanks and equipment down toward a sump hole. I requested that they stopped steaming in order to have a water truck suck out the fluid to determine if a dike drain was present. I did not see any evidence of a dike drain. Drainage in the area was to the Southwest and I followed the field tile system to its outlet to see if any fluid had reached the tile. The tile outlet was free from contamination. A mini excavator dug a deeper sump hole to determine the level of downward migration. It was determined that the oil contamination was confined to the gravel - soil interface. I requested that the Operator dismantle the equipment in the tank battery, set on plastic, excavate the contaminated gravel down to clean soil. I requested that the electric breakers for the electric sevice be pulled so that the well could not restart. Operations were halted in order to mobilize equipment and personnel
1464144142 2016-03-03 Production Wells No Violations I reviewed the tank battery for the Seeman # 3 after the clean up and reconstruction of the tank pad. The sump, that was left open to monitor, did not have any contamination contained within. All piping and equipment was leak free. No further corrective measures will be required at this time.
