Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0352092246 2015-10-29 Preliminary Restoration No Violations I was on location to check the status of preliminary restoration. The lease road is gravel and stable. The tank battery is properly identified and contained within a dike. The tank battery contains a 210bbl production tank, 100bbl brine tank, vertical separator and chart box. The well is identified and is an electrically driven pumpjack well. The well and tank battery contain all proper gauges and locks. The location has been graded, seeded and vegetation has been established. The tank battery, roadway and wellhead all conform to spacing requirements. The leak on valve on the backside of the 210bbl production tank has been cleaned up and the valve replaced and the dike rebuilt At this time preliminary restoration has been passed.

The location has been graded, seeded and vegetation has been established. The tank battery, roadway and wellhead all conform to spacing requirements. The valve on the backside of the 210bbl production tank has a small visible leak with a small accumulation of salt crystals and oil. I have contacted Greg New of Dorfman indicating that the valve must be replaced and the impacted soil removed from the location and properly disposed of. He stated the valve will be replaced and the drain removed from the dike within the week. I will follow up to confirm the leak is stopped and the dike is sealed tight.
