0273211623 2012-11-14 Production Wells No Violations I inspected the Frank 2H (34151257350000) site on November 14, 2012; I was accompanied by Tom Hill. We met Tom Vosick, Enervest Operating LLC, to check the pressure on the cemented annulus of the 8 inch casing string. Tom Vosick opened the valve on the annulus and there was zero pressure. The 5.5 inch casing had 535 psi and the tubing registered 2069 psi. Tom Hill and I walked the site to check restoration and drainage. My inspection showed sedimentation migrating off site on the south side of the well pad. There is a grated drain south of the wellhead outside the fenced well pad that does not have waddles around the drain, allowing sediments to enter the drain. The silt fencing and straw bales on the south side of the site are inadequate to control erosion. The south slope from the chain link fence to the grated drain is slumping and does not have vegetation and erosion control in place. The north slope, to include the north central and northeast corner, from State Route 619 to the chain link fence is slumping and does not have vegetation and erosion control in place. All slopes need to be stabilized to prevent slumping and erosion. Straw waddles need to be used to isolate the grated drain on the south side of the site. The south side of the site requires additional erosion control to prevent sediments migrating off site, such as straw waddles, silt fencing, and vegetation. No violations were issued. I called Tom Vosick to discuss the inspection items and he referred me to Chris Besley (304-590-7707). I called Chris and discussed the issues with him and he scheduled a contractor to be on site Monday November 19, 2012 to address the listed items. I will meet with the contractor next week to discuss the inspection.
0433435591 2014-01-27 Preliminary Restoration No Violations I was on location to perform a production inspection and to pass preliminary restoration. The lease road is in good condition, graveled, and does not show signs of erosion. The tank battery consists of twelve 210 stock tanks, six for condensate and six for brine, and one mechanical separator. The 210 bbl stock tanks have proper identification sign attached. The dike has good walls and containment capacity. The drain lines on the 210 bbl stock tanks are closed and capped to prevent an uncontrolled release. There is a 50 barrel sand tank on location as well. All equipment on site is new and in great condition. The wellhead does not show signs of oil, gas, or saltwater leaks. The fencing surrounding the wellhead and entire location is in good condition. The well has three casing strings and each is cemented to surface with no annular space. The 5.5 inch production casing has 235 psi at the time of my inspection. Erosion controls are in place and working as designed during my inspection. There are no violations on this well site and preliminary restoration can be passed at this time.
0928137512 2012-11-29 Production Wells No Violations I inspected the Frank 2H (34151257350000) site on November 29, 2012. My inspection was to follow up with the meeting I had on November 19, 2012 with Joe Little regarding erosion control around the well site. Joe Little Excavating installed straw matting on the northern and southern slopes of the site to control erosion. Straw bales have been installed on the hillside south of the well pad and surrounding the metal storm grate to prevent erosion and sedimentation migrating off site.
1818585851 2012-11-05 Production Wells No Violations I inspected the Frank 2H (34151257350000) site on November 5, 2012 due to a request from Kyle Tharp regarding the water well compliant at 12308 McCallum Avenue in Lexington Township. The lease road is in good condition, graveled, and does not show signs of erosion. The tank battery consists of twelve 210 bbl stock tanks, six for condensate and six for brine, and one mechanical separator. The 210 bbl stock tanks have proper identification sign attached. The dike has good walls and containment capacity. The drain lines on the 210 bbl stock tanks are closed and capped to prevent an uncontrolled release. All equipment on site is new and in great condition. The wellhead does not show signs of oil, gas, or saltwater leaks. The fencing surrounding the well head is in good condition. I called Tom Vosick, Enervest Operating LLC Utica Well Division, to discuss the completion report for this well. According to Tom, the well was in production from August 16, 2012 to September 12, 2012 and since September 12, 2012 the well has been shut in. The well has three casing strings and each is cemented to surface with no annular space. The 5.5 inch production casing has 450 psi and the tubing has 2025 psi at the time of my inspection.
1969037274 2012-11-19 Production Wells No Violations I met with Joe Little from Joe Little Excavating at the Frank 2H (34151257350000) well site on November 19, 2012. Joe and I discussed erosion control surrounding the well site. We walked the site and I showed Joe the areas of erosion and the need for control to prevent sedimentation migrating off site. Joe will have a crew install straw matting on the north and south slopes of the site. Isolate the grated drain south of the wellhead outside the fenced well pad and improve silt fencing. The entire site will be reseeded in the spring of 2013 to establish permanent erosion control.
