1026568913 2014-02-20 Production Wells No Violations I was on location to follow up on complaint #4604. An employee of M D Body Corp stated that natural gas came into his paining room system from the air inlet fan and forced the shut down of the facility on Thursday February 13th. The fire department responded and used gas meters to test the area but no detectable levels were found. Dominion checked and tested the gas lines on location and also found no issues. The tank battery of concern is 900 from the inlet fan of the shop. The employee had a picture of the lid of the 210 bbl production tank open. During my inspection I found the lid to be closed and the valves on the vertical separator and the 100 bbl brine tank to be shut in. The well at this time is shut in. The pumpjack is in good mechanical condition at this time. I could not detect any gas with a PHD 6 gas detector at the shop, well or tank battery. I gave the employee my contact information and let him know that if the odor/leak occurs again to give me a call so that I can follow up on the complaint.

I could not detect any gas with a PHD 6 gas detector at the shop, well or tank battery. 1501:9-9-05 (d) of the OAC states that Tank hatch lids shall have a functioning seal and the hatch shall be secured at all times the well owner or his representative is not on-site. Enervest will be informed of this and the latch on the production tank shall be closed during production to prevent any possible escaping of gas from the tank battery.
