0585129585 2007-06-14 Production Wells No Violations I met with Greg New (Dorfman), Reggie Brown (OEPA), and Rick Simmers (ODNR) at the location to observe the remediation. The 210 bbl stock tank and the separator have been removed. The location has been scraped and there are 2 stockpiles of oil saturated soil on the site. Greg will get the soil tested for disposal at American Landfill in Waynesburg. Dorfman will also flush the drainage tiles to clean out any residual free oil. The OEPA will agree to be out of the remediation process if Dorfman agrees to dispose of the oil saturated soil. Reggie, Rick, and I discussed the OEPA jurisdiction on oil and gas sites. A meeting with the OEPA will be scheduled in the future to discuss OEPA vs. ODNR jurisdiction.
0855432890 2007-06-18 Production Wells No Violations The location and the stockpiles are drying out. The siphon dam is nearly dry. There is no oil visible on the soybean field, siphon dam, or booms. The drainage tiles have not been flushed yet.
1181529851 2007-06-09 Production Wells Violations Noted I received a call at 2000 hrs on 6-8-2007 from Paul Shaw (Dorfman) regarding a lightening strike on a 210 bbl oil stock tank. 70 bbls of crude oil and 25 bbls of brine were released into a soybean field. Dorfman personnel and Reggie Brown (OEPA) were on the location and the clean up was in progress. I inspected the site on 6-9-2007. The stock tank stayed in place. The bottom of the tank was blown open and the top of the tank was blown off. Crude oil flowed east-northeast 1,150 along the ground surface and through the field tiles in the soybean field. Within that distance there was an area 200 long and 30 wide of burned vegetation and soil. No waterways were affected. A siphon dam, oil booms, and straw bales are placed in the surface drainage area in the soybean field. Oil saturated soil was pushed into a stockpile at the east end of the field. The clean up will continue on Sunday 6-10-2007. Clean up the site and remove the oil saturated soil. Rebuild the tank battery dike and replace the stock tank.
1373732045 2014-01-22 Production Wells No Violations I was on location to perform a status check on this production well. The tank battery contains a 210bbl production tank, a 50 barrel brine tank and a vertical separator. The tank battery contains proper signage and has a SPCC dike. The pumpjack has proper ID signage on the wellhead and house gas connection. The lease road is in stable condition. There are no violations at this time.
