The flowline has been backfilled and the area graded to approximate original contour (AOC). The lease road has been dressed. I contacted Mrs. Beck and let her know the repairs had been completed. Complaint #5370 can be closed.

1463793291 2017-01-23 Temporary Abandoned Annular Disposal No Violations I conducted Temporary Abandoned Annular Disposal (TAAD) status check inspection on this well. There is no annular disposal in use at this well. The well is equipped with a pumpjack with an electric motor. The diked tank battery consists of one 210 barrel tank, three vertical separators. The brine tank is located outside the tank battery to the North. The tank battery is shared with well # 151-23500 & 151-24135. There are no compliance issues noted at this time.
1467897693 2016-07-06 Production Wells No Violations A complaint was received at 20:33 from Mrs. Kristen Beck concerning a flowline leak on her property located on Market Ave. just North of Diamond Road. I advised her I would be in route. I contacted PAW Oil and Gas answering service and they contacted the owner. I then contacted the following from ODNR Oil and Gas: Tom Hill (Regional Supervisor), Rick Anderson (District Supervisor), Terry Whitehair (Emergency Operations Manager), Fred Romeo ( Emergency operations) and the Internal Duty Officer Line. While in route PAW Pumper Ed Daniels contacted me and we met at the Paw #2 Well which was the well number I recorded from my conversation with Mrs. Beck - (34-151-23350-0000). The pumper shut the Paul Weber #2 in and we proceeded to the tank battery where Mr. and Mrs. Beck were waiting. Upon arriving at the tank battery it was noticed that the tank battery was shared with 2 other wells, the Paul Webber #1 and #7, with the flowline leak coming from the Paul Weber #1 rather than the #2. Mr. Daniels and I spoke with Mr. & Mrs. Beck briefly about the leak and they left for home. Due to the sandy condition of the soil it is difficult to estimate the amount of oil/ brine which leaked from the flowline. The release is estimated to be ~ 2 barrels. The Pumper proceeded to shut in the Paul Weber #1. Nine absorbent pads were placed on the area with standing oil/ brine. The leak on the surface was ~ 60 feet in length with an average width ~ 2 feet. I spoke with pumper Ed Daniels and he advised me he would be contacting SLA Energy Services, LLC to clean up the location and repair the well. I spoke with Mr. Daniels before leaving and advised him the contaminated soil will need to be disposed of at an approved solid waste facility. A follow-up inspection will be on 07-07-2016 to check the progress of the clean-up and repair.
1481213112 2016-07-13 Production Wells No Violations I conducted a status check inspection on this well to check the clean-up of a flowline leak. SLA Energy Servicea, LLC has cleaned-up the leak. The flow line has been replaced and a pressure test will be conducted this afternoon. I arrived back on location ~ 15:00 hours and Terry Whitehair (ODNR Oil & Gas) was also on location to witness the line pressure test. Tubing gas was used to pressure test the new flow line. The line pressure was 148 psi, the line was isolated for twenty minutes and the pressure dropped to 144 psi. There was no audible sounds of a leak in the flow line.
