On 10-31-2008, the top section of the 8-5/8 diameter casing was cut off and a new collar was welded on. The top section of the 4-1/2 diameter casing was cut off and a new belled nipple was welded on. A new 4-1/2 diameter wellhead was installed. On 11-4-2008, I observed and photographed the new Parmaco packer. The packer was run in on a used string of 2-7/8 diameter external upset tubing. I observed the placement of the annular inhibitor (5 gallons of annular inhibitor to 35 barrels of fresh water pumped down the annulus). It was too dark to continue, so we shut the well in for the night. On 11-5-2008, annular inhibitor (5 gallons of annular inhibitor to 70 barrels of fresh water) was pumped down the 2-7/8 diameter tubing. The packer was set in tension at 4947.40. I performed the MIT. Pressured up 1380 PSI on the annulus; dropped to 1370 PSI after 15 minutes; pressure drop of less than 1%; MIT PASSED.

They are circulating the csg with a Hondo trash pump. It appears that cement from the annular space between the 8-5/8 csg and the open hole dropped into the 4-1/2 csg while it was unscrewed. After screwing back onto the 4-1/2, they were unable to get the tbg down to the required setting depth inside the 4-1/2 csg. They tripped out the tbg and went back in with a bit on the tbg and started drilling.

I checked the cement brine storage pits and observed that they were pumped down approximately five feet below ground level. I didnt check the injection pressure during this inspection because the needle on my UIC gauge broke. The injection pump was running while I conducted my inspection, but I couldnt check the pump because the door to the pump house was locked with a chain and combination lock. No trucks arrived on site while I was conducting my inspection. My inspection revealed no violations at this time.

There is an identification sign posted on the pump house. I called Bill Magrell from Petrox and left a message for a return call. At 10:35 am, Mr. Magrell returned my call. I told Mr. Magrell to install locks to secure the fencing around the open pits and a follow-up inspection will be conducted by 11/27/2017. Mr. Magrell stated the fencing would be secured immediately.

A follow-up inspection will be conducted.
