0188430917 2016-03-08 Production Wells No Violations I conducted an inspection on this well as a follow-up to a request from the Stark County engineers office. The wellhead had a pumpjack on it and the valves on the wellhead were open. There was water around the 8 and 5/8 inch casing and the water had a thin layer of oil on it and was bubbling indicating a leak from the wellhead. The tank battery consisted of a 100 BBL production tank, a 50 BBL steel brine tank and mechanical separator. The well identification sign was legible and mounted at the tank battery. The access to the tank battery and wellhead was stable. I contacted G&O Resourcces Ltd who was sending out the pumper, Mark Shubert to remedy the leak at the wellhead. I will reinspect this well in two weeks.
1230458774 2014-01-17 Production Wells No Violations I was on location to follow up on a compliance notice for this well regarding complaint #4577. The Stark County Engineers Office called to report that while one of their bridge engineers were inspecting a bridge, he discovered an exposed oil and gas line leaking gas. The line crosses a creek and the creek is bubbling where the line was leaking. The well pumper Dave Widduck shut in the tubing and the leak stopped. They are also going to be planning to plug this well. We have agreed to have the flow lines removed from the well to the tank battery, which will no longer provide a pathway for gas to enter the stream if gas flow was to occur again. They are going to be removing the flow lines to ensure that the well cannot contaminate the creek at this time. The compliance notice can be now passed at this time as the contamination and leaking from the well has stopped. I will be following up on this well when the flow lines are removed when the weather improves and the well is plugged. There are no violations for this well at this time.
1389885554 2014-01-15 Production Wells Violations Noted I was on location to check on complaint #4577. The line crosses a creek and the creek is bubbling where the line is leaking. He wasnt sure if that was the well owner. I was able to identify this well as the well providing the gas leak. I contacted Chuck Will (330-805-0181) and he said that the well had been shut in and that it could not be leaking. I insisted he send someone to the well to check on it. The pumper Dave Widduck confirmed that the well was shut in, but the tubing was not shut in. This was causing gas to still flow through the line, which was under the bridge in the creek. They shut in the tubing and the leak stopped. They are also going to be planning to plug this well. We have agreed to have the flow lines removed from the well to the tank battery, which will no longer provide a pathway for gas to enter the stream. They are going to be removing the flow lines this week and ensure that the well cannot contaminate the creek at this time. A compliance notice has been given to the operator and will be passed when full remediation occurs. Dave Widduck will contact me when the flow lines are removed so that I can verify that the work has been done and the compliance notice passed.
1493809226 2019-04-15 Production Wells No Violations My inspection found the well is not plugged as requested by the Compliance Notice issued on 01-09-2019. The well is shut in. The water ponding around the wellhead shows it is leaking gas below ground level. The application history shows no Plug and Abandon permit being issued at this time. I phoned 330-253-0620 (G & O Office) and left a message for a return call. The tank battery consists of a steel production tank, a steel brine tank, a vertical separator and a meter.
1547124833 2019-01-09 Production Wells Violations Noted My inspection found the well is equipped with a pumpjack. The well is shut in and the flowlines are disconnected. The well last produced in 2013 for 47 barrels of oil. The well has zero production reported from 2014 through 2017. The tank battery consists of steel tank, a vertical separator and a meter. The well is legibly identified. I called 330-761-3508 and it went to a fax machine. This well is idle. Consequently, the well is in violation of Revised code 1509.062 and shall be plugged in accordance with Ohio Revised Code section 1509.12 (B). The well owner/operator shall submit a written plugging schedule to Shawn Murphy at 3575 Forest Lake Dr., Ste. 150, Uniontown, Ohio 44685 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

2092763925 2015-08-03 Production Wells No Violations I was on location to perform an inspection of this production well at request of the landowner. The lease road is stable. The tank battery contains a 100bbl production tank, 50bbl brine tank, vertical separator and has new identification posted. The electrically driven pumpjack wellhead is currently shut in and overgrown. The electric box is disconnected. A new bridge is being built near the access road and the flowlines for the well have been removed while the construction is being completed. I have contacted Bill Lavin of G&O and they are going to evaluate the well to either plug or put the well back into production following the site construction. This well produced 47bbls of oil in 2013.
