0123325720 2015-06-16 Production Wells No Violations I was on location to follow up to a compliance notice issued for contamination. The spill around the wellhead has been cleaned up and clean vegetation is growing. The tank battery has been cleaned up and the oil soaked rags and gravel around the tanks have been removed. The water inside the dike was field tested with Quantabs and did not show any elevated chloride levels. At this time the compliance notice has been closed and the there are no violations.
0650178474 2014-08-01 Production Wells Violations Noted I was on location to investigate complaint #4759. The complainant stated that around the week of July 6 he noticed the ice cubes made with his well water were salty. This well is about 850 from the residence. The tank battery is shared with well #21733 and contains two 100bbl production tanks, a vertical separator, a poly brine tank and I.D. signage. There has been a spill in the dike with adsorbent pads and booms covered with oil and the rock inside the dike also covered with oil. Will Mack (330-465-3872) was on location to dig out the 8.625 OD casing head. The 8.625 casing was set to 1041 and through the Berea. The casing head has been covered and surrounded by a steel barrel and buried. The barrel was cut back but could not be removed. No oil was flowing from the 8.625 casing but was bubbling gas. The wellhead at this time could not be accessed due to the steel barrel. Next week a bobcat and equipment will be brought on location to cut back the steel barrel and remove it from the well. The wellhead will also be excavated further so that the 8.625 casing can be accessed. This well will be followed up upon for the complaint and the oil spill in the dike and around the wellhead must be cleaned up before compliance notice will be passed.
1220022862 2014-03-18 Production Wells No Violations I conducted a status check inspection of this well while confirming database GPS coordinates. RBDMS plots this well 3.6 miles due east on the north side of Georgetown Road and is incorrect. The well is operating with the aid of an electric pumping unit. The ID at the wellhead indicates that this is PN#21733 (Miller #2) which is 1150 NE of this location and needs to be corrected. This well produces to the same tank battery as the Miller #2 (PN#21733). See inspection report for #21733 for battery inspection. I will notify the Stark County inspector of the issues I found at this well.
