0016865584 2009-04-30 Orphan No Violations On 4/29/09 OEPA responded to an oil spill in Sandusky county, Woodville township, section 20. Oil was discharging from a field tile into a ditch along Township Road 16. On 4/30/09 inspectors Tom Benko and Ken Piispanen met with landowner Bob Bench and OEPA inspector Dave Schilt to attempt to find the well causing the discharge. New field tile was being installed in the farm field and a possible location where the ground had been ripped was checked because of the odor of oil and gas. Inspector Benko pinpointed a spot with a metal detector and the landowner excavated at the site with a backhoe. An open 8 inch casing was found approximately 4 feet below ground level but the pipe was below water. Gas and some oil was bubbling from the casing. A 12 inch diameter PVC pipe approximately 8 feet long was driven into the ground around the casing to contain the flow. Inspector Piispanen spoke to USEPA inspector Brian Kelly by phone. Well will be submitted for plugging with US Coast Guard funding since the ditch discharges to the Toussaint River approximately 1 mile away.
1084371599 2009-05-04 Orphan No Violations Checked site from oil spill last week on 4/29/09. Oil is contained in 12 inch PVC pipe over well. There does not appear to be a significant rise in fluid inside the pipe. Water in the open pit around the well has some oil on the surface apparently from leaking prior to riser placed over well. Absorbent boom was in pit. All booms removed from ditch except 1 boom laying along the edge of the bank. Noted oil film on water in ditch from water discharging from tile. Stake absorbent boom at tile outlet and retrieved another absorbent boom on top of bank further to the north. Coiled boom under tile outlet because oil was creating scum on top of water inside the staked boom. Called OEPA inspector Dave Schilt and left message.
1234762193 2009-06-10 Orphan No Violations Checked discharge at field tile. No discharge from tile. Absorbent booms removed from ditch and laying on ditch bank. Ditch and banks recently seeded and mulched due to ditch cleanout project.

1267553238 2010-02-18 Orphan No Violations Mike Coyer of Black Swamp Oil Field Services on site beginning clean out of well. Cable tool rig on site working on well. Tools reached depth of approximately 360 feet when hit obstruction. Could not break obstruction with tools on rig. Will bring in heavier tools (2600 lbs) to try to break obstruction. Water from cleanout passed to plastic lined pit. Only a small amount of oil noted. There was some gas in the fluid. Checked for H2S but no readings exceeded 10 ppm to trigger H2S monitor. Spoke to landowner Mr. Bench on site. He asked if well could be converted to a water well. This would not be allowed on this well due to contract specifications. Well is to be plugged full of cement.
1267734527 2010-03-04 Orphan No Violations Inspection with Jeff Fry and Sam Faust to review well head protection. Well head protection welded to casing. Valve allows well to release fluid directed to pit or be retained in well casing. System is acceptable and meets contract requirements. Cable broke on baler while baling well.
1271344159 2010-04-15 Plug / Plug Back No Violations Operator Mike Coyer reached depth of 925 feet during clean out. Continued effort at 925 feet was resulting in cave inside of the well bore. Operator made good effort to clean out well to this depth despite mud and obstructions encountered. Clean out process began with 1200 lb bit on cable tool rig with baler to depth 340 feet. An iron ball was encountered at this depth which is probably where the surface casing was shot and removed from the well. A 2400 lb bit was brought in to break the iron ball. The iron ball was broken and clean out continued to depth of 440 feet where the tools began hanging up in the well. A mud pump was brought in and water was pumped at approximately 700 psi to wash mud and debris from the bore hole. This was successful to depth of 925 feet but well bore was caving in and no further depth could be gained. Checked with ODNR supervisor Jeff Fry and permission was granted to plug well at this depth.
1272458258 2010-04-22 Final Restoration No Violations All equipmemt removed. Well has been cut off 4 feet below ground and ID plate attached. Pit backfilled. Due to amount of mud removed from well and washed into pit, the ground over the pit area was left higher than surrounding area to allow for settling. Farmer was already in field plowing/discing in preparation for planting. Final restoration acceptable.
1310439371 2009-05-20 Orphan No Violations Open pit around well has been backfilled. Noted gas still bubbling inside of open well casing. Absorbent boom still at tile outlet in ditch.
1663882321 2010-03-16 Orphan No Violations Rig on site with dozer and mud pump. Nobody on site. Spoke to Mike Coyer by phone. He will return this week when ground conditions improve to fish out baler. Plan to try to wash mud out of casing since cable tool not making good progress. Pit liner slipped on west side of pit and needs to be repaired.

1894588031 2009-04-30 Production Wells No Violations Contacted by Com Center. OEPA located suspected well. During installation of new tile, a tile was laid 3 inches from the 8 inch casing and cut off 2 feet below grade. Water, gas, and oil were releasing from borehole. Place 10inch diameter water line pipe over casing and push into ground to contain well head.
2002124641 2010-03-02 Orphan No Violations Cable tool rig on site. Nobody on site. Called Mike Coyer on phone. Discussed wellhead control for cable tool. He said he had device that could be attached to well for wellhead control. This should be installed before further activity on well. Also noted rip in pit liner above fluid level in pit. Mike will install new liner in pit. Clean out in well to depth of approximately 450 feet. Operator indicate he may try mud pump to clean out mud in well before resuming with tools.
