0228078133 2013-06-24 Plug / Plug Back No Violations An inspection of this plugged and abandoned well was conducted to check status of cement level and potential leakage. During the inspection, it was noted that the cement was down approximately 5 with no observed gas bubbling within the fluid.
0973722782 2012-04-04 Orphan No Violations Inspection to update GPS and well status for a potential Orphan Well Package. At the time of my inspection the well had 8conductor casing, 6 surface casing with casing head, 2 tubing on a casing head ring and rods with a stuffing box still inside tubing. The well is in a cow pasture and is surrounded by smaller trees and brush.
1370956214 2013-06-11 Plug / Plug Back No Violations An inspection of plugging operations today revealed that the contractor fished all of the remaining parted steel tubing and wooden rods from the wellbore. The contractor utilized the same overshot tool, but equipped only with (1) 10 joint of 4.5 instead of (3) 10 joints that were utilized on 6/7/2013. The tool had initially set down on a rod at approx 340 however, the contractor was unable grab at this location. Additional joints of tubing were then added to the fishing string to allow for the retrieval of wooden rods at a deeper location. Note: Natural gas was smelled and felt through the tubing during this procedure. The contractor latched onto the rods at a depth of 440, but his clutch began to slip and was unable to pull with rig. The contractor consulted with his partner, Mr. Mike Coyer, and mobilized another rig to the location for the retrieval of this fish. In summary, all wooden rods, steel tubing, and the working barrel excluding the mud anchor have now been removed from the wellbore.
1371752753 2013-06-19 Plug / Plug Back No Violations I witnessed plugging operations today. The wellbore was filled solid with approx 7.8 yards of 9 sack grout mix from a total depth of 1230 to the surface. Prior to cementing, the contractor bailed the hole dry on 6/18/2013 and 6/19/2013. The contractor was instructed to leave the wellbore open to allow for settling and to ensure no leakage had occurred.
1374062072 2013-07-16 Final Restoration No Violations An inspection of the Final Restoration revealed compliance with Ohio Revised Code 1509.072. The well site was situated on a location designated as pasture field. At the conclusion of the inspections conducted on Permit Numbers 60214 and 60215, the results were discussed with the landowner, Mr. Tim Sautter, at his residence. Mr. Sautter confirmed that he was satisfied with the restoration work on this property and the additional wells also plugged and abandoned but located within field crop.
