0031782378 2011-01-06 Drill / Deepen / Reopen No Violations An inspection revealed work being peformed on drilling pit. No drilling at this time. Observed fluid being pumped from the pit into tanks. Four water trucks have been scheduled to haul water from the location. The company will allow existing liner to dry then install new liner and address pit walls. The blowline measured 72 to the pit and will be re-positioned prior to the commencement of drilling. The well owner, Mr. Gary Hopkins was in attendance for this activity.
0060728069 2011-01-28 Drill / Deepen / Reopen No Violations Mr. Mike Williams, Geologist/DMRM Central Office, received a complaint today from Senator John Careys office alleging that oil was spraying onto Ms. Melody Longs house. Arrived at the well location at 1:15 pm (snow cover w/ slight wind to the east away from Ms. Melody Long property). Mr. Jon Scott, Mineral Resource Inspector 2 accompanied me on this complaint. The drilling rig has now moved off the location and company working towards removing the remainder of equipment. The well site showed no evidence of oil discharge. The drilling pit contained no oil or evidence of spraying. Crude oil was smelled around an approx 250 bbl frac tank used for brine and half full (to the east & downwind) located on edge of pit. Bottom valve contained a small drip (1 drip approx every 10 secs. All valves on well were observed open on one side (4.5 csg half-open). No detection with gas meter. The trees located along the embankment of intermittent stream were stained only from dusting and/or release lines (dirt+air). This area measured 79 in length. Walked the intermittent stream and found no evidence of oil. At 1:40 pm, an interview was conducted with Mr. Lewis, the landowner for Permit 58. Mr. Lewis states that he never heard any sounds or observed any discharge. This complaint will be closed.

An interview was also conducted with Ms. Melody Long at 3:00 pm, prior to the inspection of her house (interview ended at 3:04 pm). Ms. Long states no oil complaint was filed with Senator John Careys office because, her concern is with the methane gas in her sons bedroom. She states that Senator Careys office got it wrong and must have mixed up her previous message left about the air quality. Ms. Long states that she left them the same message she left me. I informed Ms. Long that I did not receive her message at the office until the following Thursday. I emphasized for her to call my cell phone with any problems because, we are in the field about 90-95% of the time. I

No mechanical separator on line from well to tank. Seed needs to be applied to bare areas. 100 bbl steel tank is venting gas.
0346929980 2011-02-11 Preliminary Restoration No Violations Pit solidification. Pit water being hauled off by Huffman-Bowers. Water is being transported to next drill location. Water is being stored in frac tanks. King Excavating on site with a trac hoe and dozer. Pit had 2 ice on surface. Excavator broke the ice, calcium was added to ice to melt it. The water was pumped off cuttings, then 2 tear drop truck, bulk loads of cement and fly ash was added to cuttings and mixed with the trac hoe. A good mortar consistency was achieved. Accompanied by Josh Garland MRI 1. Gary Hopkins was on site for most of the work. Pit to be filled in on 2-12-11.
0681627264 2011-01-13 Drill / Deepen / Reopen No Violations A Static Water Level test was conducted on water well located at 2107 Piney Creek, a neighboring property to this well location. The complainant had alleged disruption prior to the spudding of Permit 58. Mr. Jon Scott, Mineral Resource Inspector 2 assisted with this activity. Sanitized the probe before placing into wellbore. The measured static water level showed 8 8/10. Cobwebs and grass cuttings were observed inside the well and under cap. It was also noted that the cap was not tight. Snow cover at time of inspection. An inspection of Permit 58 well site was also performed. The depth at time of inspection was 2820. The drill bit may potentially be plugged, as circulation has now been lost. A deviation tool had been hung in the hole, but later retrieved. However, lost circulation would follow shortly thereafter. The tool may have opened up the string float therefore, allowing the stabilizer to be plugged. The drill pit and blow line have been addressed and are acceptable.

0868472360 2011-12-21 Production Wells No Violations Accompanied by Jon Scott on inspection. Identification in good order. There was an area 20ft wide by 90 long that had no vegetation established. Small wisps of gas were being vented from the 100 barrel production tank. A call was placed to Gary Hopkins at 2:55 pm. He stated that the bare area will be getting gravel on it. He also stated that after the first week of January, he will be making critical decisions about the fate of the well. He then agreed to talk to the landowner about house gas to alleviate the gas venting problem. Another solution to the gas venting problem that he made mention of was installing a generator to sell electricity to the power company.

1297914151 2011-01-04 Drill / Deepen / Reopen No Violations An inspection of the well site was conducted with Deputy Chief, Tom Tugend. The intermittent stream, blow line from drilling rig and pit were reviewed. The stream impact measured 96 (length) x 25 (approx width of pooled area). During the inspection, it was noted that rig crew had placed a section of new liner over old liner containing on the backside. The drilling pit and fluid level will need to be addressed. The blowline will also need to be repositioned, as it was facing downward in pit and not staked and/or chained. Mr. Tugend will contact the well owner, Mr. Gary Hopkins to discuss the inspection results. The Division will attempt to consult with the Army Corps of Engineers and Ross County Soil and Water regarding the intermittent stream.

Witnessed two more air samples taken inside long residence after frac/during flow back. No LELs on meter inside Long Residence or out side residence. Entire perimeter was walked with gas meter and no LELs were observed.

Andy Thomas and I arrived on the well site at 2:20 am on 4-3-2011. Air sample JTS 11-22 was taken at Frack tanks GPS N39.35804 / W082.78544. Gas meter readings at frack tanks 0 LELs , 0 H2S. Wellhead is shut in, gas meter readings at wellhead 0 LELs, 0 H2S. Wellhead has pool of rain water around it. No bubbles were observed in water around wellhead. Andy Thomas and I inspected all 34 frack tanks on location. No leaks were observed. Air sample JTS 11-23 was taken inside the Long residence master bedroom located off of the living room. Gas meter readings inside house, 0 LELs, 0 H2S.

1315395388 2011-08-17 Preliminary Restoration Violations Noted An inspection of the preliminary restoration conducted with Jon Scott revealed an area that lacked vegetation, no identification on the well head or tank battery, and gas was being vented from the 100 bbl. tank in the battery. Called Gary Hopkins at 9:56 am and left a voicemail. He returned my call at 10:06 am. We addressed the situation, and he stated that the well was not doing very well, and he was going to give it a couple more months, and then possibly plug it. He said he would get identification on it as soon as possible. He also stated that the landowners had been doing the re-seeding on the property for the restoration, and he or his pumper would talk to them about getting vegetation established.
1350999145 2011-01-22 Drill / Deepen / Reopen No Violations Complaint # 2984 investigation and sampling with Kyle Tharp. Kyle took air samples and I monitored the Gas meter and recorded the readings at each sample point with in the house and at intervals from the house to the well. Was unable to take a water sample due to outside frost proof hydrant being tilted backwards and frozen solid. This hydrant is the only source of well water that is not routed through the water softener. Melody Long and her son were present during the investigation.

1369658726 2011-01-17 Administrative Inspection No Violations Plug-back to the Queenston formation today, due to a dry hole. Discussed formation tops and plugs with the well owner, Mr. Gary Hopkins via telephone. The activity was not witnessed because of a Holiday.
1451417036 2015-12-29 Production Wells No Violations Production well inspection. The well consists of a pumping unit and 2-100 barrel tanks. The pumping unit was in operation during my inspection. The last reported production was in 2013.
1453639311 2010-12-21 Drill / Deepen / Reopen No Violations Witnessed surface job. Good returns were observed at the surface. The hole was drilled on air. The cement weight was verified with scale and showed 17 ppg. A wet cement sample was obtained from this job. Walked the perimeter of location and discovered that a nearby intermittent stream contained a dam approximately 100 in length. Hopco Resources representative Mr. Greg Boyd and Mr. Randy Rainer, the toolpusher for G&L Drilling were in attendance. Mr. Rainer stated that the dam was constructed for water containment, due to lack of available water trucks. Further stating the containment did not allow for much water and is not being used at this time. Mr. Boyd and Mr. Rainer were instructed to correct this deficiency immediately. Management will be notified of the inspection results. Left the location and cement was at the surface. Furthermore, rig personnel were starting up dozer to begin work on the stream. Note: snow cover at time of inspection. Water well complaint on neighboring property alleging disruption prior to spudding.

1847093205 2010-12-30 Drill / Deepen / Reopen No Violations Witnessed the cementing of 7 casing (intermediate string). The cement was weighed and a wet sample was obtained for records. Informed the well owner, Mr. Gary Hopkins, in the field, that fluid level appears to have exceeded a section of pit liner located next to stockpiled soil. The fluid level along with tapered edge of pit and stockpiled soil does not allow for clear confirmation of liner top. Mr. Hopkins stated that he thought the liner was located up higher and just buried under stockpiled soil. The intermittent stream has been addressed and no longer contains a dam. Instructed Mr. Hopkins to address the fluid level inside the pit.
1882810828 2011-01-10 Drill / Deepen / Reopen No Violations A field review of the intermittent stream was conducted today with Mr. Melvin Garrison, Ross County Soil & Water and Mr. Gary Hopkins, the well owner. Mr. Garrison stated that impact to the stream was a non-issue. He recommended the installation of straw bales for wet weather and to establish vegetation. Mr. Garrision further stated to call, in the spring, when work is being performed. No drilling at time of inspection, due to mechanical problems with rig. Snow cover at time of inspection.
