My inspection found that the electric pumping unit has been installed and is currently in operation. The valve on the 4.5 production casing port is closed and the sales line is currently being installed to the tie in point at the Sekel A #s 1 & 2 tank battery located 800 to the northeast. Cary Leshock (Operations Supervisor) is on location and adjusting the Baird valves for optimal pressure. Identification has been posted at the wellhead and at the tank battery. No violations.
0457359156 2019-02-19 Work Over Reconditioning No Violations I was on location to inspect workover operations on this horizontal Clinton Sandstone well. Moore Well Service was rigged up on location and running 2.375 tubing into the 4.5 production casing. They are currently above the curve and circulating the hole with air to remove any sand or paraffin. All returns are being circulated to a steel tank and the wellhead is equipped with a blow out preventor. They are going to continue to trip tubing through the curve and into the lateral to clean out the casing. There were no violations at this time.

Moore Well Services was rigged up over the hole and were flowing the well back to a steel tank with 2.375 tubing spotted at 3000. They are also currently hauling in freshwater and filling the pits and tanks in preparation to complete the #2H (PN#24472). I found no violations at this time.

My inspection found U.S. Energy completing a bit trip. They have changed out the 6.175 PDC bit and swapped out the downhole motor. They are currently tripping in at a measured depth of 5148 and the bottom of the current hole is at a measured depth of 5480. The pits are in good condition and have plenty of freeboard. No violations.

My inspection found that the frac tanks used during completion operations have been removed from location. Moore Well Service is on location and hauling water from the pits and transferring the water to the Frame pad for use in completion operations. I spoke with Harry (U.S. Energy) and the pits will be solidified and closed in the next two (2) weeks and Preliminary Restoration can be approved after this area is reclaimed and stabilized. Moore Well Service informed me that they would be moving in next week to workover this well. No violations.
0632106438 2018-08-28 Completion Testing No Violations Status check inspection of this drilled, horizontal Clinton Sandstone well. My inspection found the rig crew moving the drilling equipment to the west side of location to make room for completion operations scheduled to begin next week. 14 frac tanks have been spotted along the north side of the pad on the south side of the drilling pits and are currently being filled with freshwater by Moore Well Services. Pit water (brine) is being pumped into half of the frac tanks and being treated with a biocide. The pits are in good condition and have plenty of freeboard and the wellheads are secured with jersey barriers. No violations.
0669254623 2018-09-27 Drill / Deepen / Reopen No Violations I was on location to witness the fracing of the above well, Producers Service Corp was on location performing the operation. All annuals were monitored. M&R ( Diversified ) has 4 well in the area that they are also monitoring the 8-5/8 and 4.5 casing with 2-pen records. I witnessed stage #5 of the 15 stages. There was no safety or environmental issuse at that time.

U.S. Energy ran 21 joints of 23lb/ft. casing and 74 joints of 20lb/ft. casing to a depth of 4156, measured from the kelly bushing (KB +10), into an 8.75 open hole with a total depth of 4181 (KB). The casing was equipped with a guide shoe and 19 bow spring centralizers. Petroset Cementing conditioned the hole and cemented the casing with 300 sacks (63 barrels) of Class A cement pre-mixed with 3% calcium chloride (CaCl2). The mix water was tested at 75 degrees Fahrenheit with a pH of 7 and having a chloride concentration of less than 125ppm. Cement density was verified by scale at 15.4lbs/gallon. U.S. Energy ran 7 joints of 36lb/ft. casing to a depth of 270, measured from the kelly bushing (KB +10), into a 17.5 open hole with a total depth of 306 (KB). The casing was set open ended and equipped with three (3) bow spring centralizers. Petroset Cementing conditioned the hole and cemented the casing with 350 sacks (73.5 barrels) of Class A cement pre-mixed with 3% calcium chloride (CaCl2). 15 barrels of cement was circulated to surface during the job. The mix water was tested at 73 degrees Fahrenheit with a pH of 7 and having a chloride concentration of less than 100ppm. Cement density was verified by scale at 15.4lbs/gallon. 0988032355 2018-11-16 Surface Facility Construction No Violations Status check inspection of surface production equipment construction and installation that will service this well and the Sekel Unit #2H (PN# 24472). Moore Well Services has spotted and plumbed together three (3) 210 barrel tanks on the north side of the location. They are currently moving the fire-heated, gas production unit into position east of the tank battery. The tank, wellhead, and separator spacing meets the requirements outlined in ORC 1501:9-9-05. The wellhead has been completed with 2.375 tubing and rods and is ready for the pumping unit to be installed (not currently on location). The pad and lease road are snow covered and in stable condition. No violations.
1063471409 2018-06-13 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of U.S. Energy. They are currently drilling a 8.75 hole on air at a depth of 3187 and are dusting through the top of the Big Lime at a rate of 42/hour. The blow line is chained to cement blocks and the pits are in good condition. They are expected to reach total depth on this vertical section of the hole by early tomorrow morning (6/14/18) and cement 7 casing later in the afternoon. No violations.

Moore Well Services was rigged up and pulling the flow back tubing from the hole. They said there was 200 of sand fill-up at the end of the lateral and will not be cleaning it out at this time. There plan is to trip the tubing and run the production tubing and rods. The pits used for drilling and completion are mostly empty and in good condition. No violations.

Moore Well Services was rigged up and running pumping rods into the 2.375 tubing. The tubing was set at measured depth of 4554 (138 joints) and about 30 degrees into the curve with a full length mud anchor and a perforated pup joint. They are running a total of 182 pumping rods. Keith Moore said they should be moving the service rig out Monday (7/12/18) and his roustabouts are going to start building the tank battery. No violations
1526319426 2018-05-14 Drill / Deepen / Reopen No Violations Status check inspection of pad construction for this horizontal, Clinton Sandstone well. My inspection found the lease road and apron completed and stable. I met with Dave (site construction) and he said that the rain has prevented any further construction of the pad. I inspected the pad and it is approximately 200 X 100 and the southwest quarter of the pad has been stoned. The cellars for this well and the 2H (PN#24472) have been installed and the bottoms have been sealed with cement. I took GPS readings of the cellars (wellhead locations) and they agree with the submitted location plats. There are two (2) bull dozers, a trac-hoe, and a roller on location. I will check back later on construction progress. No violations.

1528120940 2018-06-04 Drill / Deepen / Reopen No Violations Status check inspection of this horizontal, Clinton Sandstone, drilling operation. U.S. Energy set 17 of 20 drive pipe on 5/31/2018. They are currently rigged up and waiting on parts for the air hammer. I met with the tool pusher and he said they will probably not start drilling until some time on 6/5/18. I inspected the surface casing on the racks and there are eight joints of new, 36lb/ft., ASTM A500 STC casing on the racks. The pits are lined and fenced and have not been filled. I found no violations at this time.
1528738075 2018-06-11 Drill / Deepen / Reopen No Violations I was on location to check on the drilling status of U.S. Energy Rig #1. They have drilled the 12.25 intermediate hole to a depth of 525 measured from the Kelly bushing (KB) which is 10 above ground level (GL). The discharge line is venting low pressure gas from the top of the Berea Sandstone around 405 (KB). The driller indicated that water was encountered in the formation. They are currently tripping drill pipe out of the hole. No violations.
1529522998 2018-06-20 Drill / Deepen / Reopen No Violations Status check inspection of drilling operations at this horizontal Clinton Sandstone well. The U.S. Energy rig is tripping out of the hole with the motor and drill bit. I spoke with the tool pusher and he said they were getting tight and got hung several times while trying to do a 20 slide. They are currently 200 from landing the curve at a measured depth of 4835 and an angle of 58 degrees. They recovered the 6.175 PDC bit and the bit was in gauge and missing several buttons but should have been able to drill according to the company man. They made up a new bottom hole assembly (BHA) with a new motor and drill bit and began tripping back into the hole. My inspection of the drilling pits found them in good condition. No violations.
1529681193 2018-06-22 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of U.S. Energy. My inspection found the rig currently shut down and the 3.5 drill pipe bent and split in half above the rig floor and below the top head drive. The rig had drilled a 17.5 hole to a depth of 306 measured from the kelly bushing (KB +10). The tool pusher told me they were cleaning up the hole and something plugged of or caved in and forced the drill pipe out of the hole and split the pipe. There were no injuries or releases caused by the incident. They cut the bent drill pipe off and are getting a grapple to get ahold of and retrieve the cut off pipe. There were no violations at this time.

U.S. Energy has landed the curve with a 6.175 bit at a measured depth of 5039. They are currently drilling on fluid and back reaming the curve. They should begin drilling the horizontal leg later today. The drilling pits are in good condition and have plenty of freeboard. No violations.
1536324721 2018-09-06 Completion Testing No Violations Status check inspection of completion operations on this horizontal Clinton Sandstone well. Producers Service Corp. was the fracturing contractor and SureFire Wireline provided the crane, lubricator, pump down guns, and kill plugs. I met with the U.S. Energy company man on location and they have just completed stage 13 of 16 total stages. They are currently shut down and waiting on acid from Zanesville. The operations were discussed and there were no reported issues or anomalous annular pressures encountered during fracturing operations. The location is in good condition and all fluids are being contained in steel tanks. No violations.
1536590657 2018-09-10 Completion Testing No Violations Status check inspection of completion operations. Moore Well Services is rigged up over the hole and is drilling out the last frac plug at the end of the horizontal leg of the well. The wellhead is equipped with a blow out preventer and stripping head that are functioning properly to prevent the escape of flow back water. All fluids are being circulated to a steel tank. They drilled through the last kill plug and are circulating the hole clean. No violations.
1537195680 2018-09-17 Completion Testing No Violations Status check inspection of this horizontal Clinton Sandstone well. Moore Well Services is currently on location and flowing the well back to a steel tank. Tubing is currently spotted at 3216. The well is gassing and flowing back fluid. Two (2) more joints were run to a depth of 3276 and the well continued to flow back. There was no detectable LEL inside the doghouse. No violations.
1545926299 2018-12-27 Preliminary Restoration No Violations Status check inspection for preliminary restoration of this horizontal, Clinton Sandstone well location. My inspection found the well is in production and is equipped with 2.375 tubing and rods, and operating with the aid of an electric powered pumping unit. Gauges indicate 250psi on tubing and 390psi on 4.5 production casing. A gauge indicates 75psi on 4.5X 7 annulus and it is equipped with a 250psi pop-off valve. The 7 X 13.375 annulus is plumbed to surface and open. The stuffing box is fitted with a pollution control device that is plumbed to a 55 gallon drum. This well produces through an indirect, fire-heated, horizontal separator and into three (3) 210 barrel tanks. All the surface production equipment is within secondary containment and is shared with the Sekel #1H (PN#24473). A 500 barrel frac tank has been spotted within secondary containment and is being used to transfer brine from the 210 barrel tanks for disposal. The required ID is posted at the tank battery and the wellhead is properly identified. There are currently 12-500 barrel frac tanks on location that were utilized for completion operations. The tanks have been emptied and will be moved to the Frame Pad for completion operations in the next month. The drilling pits remain open and contain cuttings and freshwater. I have spoken with U.S. Energy concerning the pits and have extended the time frame for closure until the water can be transferred to the Frame Pad for use in fracturing operations. The liners are intact and the pits have plenty of freeboard. Drilling was completed on this pad on 7/10/18 and preliminary restoration is due by 1/10/19. This time frame has been extended into the spring when a good seed bed can be established. No violations.
1717347536 2018-07-03 Drill / Deepen / Reopen No Violations Status check inspection of this horizontal Clinton Sandstone drilling operation. U.S. Energy Rig #9 has drilled a 6.175 hole to a total measured depth of 6557. Note*: They attempted to cement the 4.5 casing on 7/1/18 and could not break circulation. The casing was pulled and the drill string was tripped back to bottom to clean up the hole. The rig is currently running the casing back into the hole and should be cementing this afternoon. I found no violations at this time.
1748059657 2018-11-26 Surface Facility Construction No Violations Status check inspection of production equipment installation. My inspection found the wellhead tied into the fire-heated, horizontal separator shared by the Sekel #2H (PN#24472). The dump line to the tank battery has been installed and there are three (3) shared, 210 barrel tanks. The dike around the tank battery is partially constructed. The electric powered pumping units have not been installed and the power supply has been set and the lines are trenched in. All equipment spacing meets the requirements of OAC 1501:9-9-05(A). There are currently 15-500 barrel frac tanks on location that were used for completion operations. Much of the ancillary equipment used for the drill out and flow-back remains on location. The drilling pits are still open and need to be emptied, solidified, and closed. I will speak with U.S. Energy about their time frame for getting the pits closed. There are currently no violations.
1757191232 2018-06-11 Casing String Witnessed No Violations I was on location to witness the running and cementing of the 9.625 intermediate casing string. U.S. Energy ran 12 joints of new, 29lb/ft., A500 Grade B&C casing into the hole and landed the casing at a depth of 510 measured from the Kelly bushing (KB +10) with a 12 landing joint. The bottom of the casing was fitted with a guide shoe and was run with six (6) bow spring centralizers. The casing was cemented with 63 barrels (300 sacks) of Class A cement slurry (verified by scale at 15.3lbs/gal.) and 14 barrels of good cement circulated to surface prior to landing the plug. Refer to the casing ticket for more details on this job. No violations.

1844260855 2019-02-04 Production Wells No Violations Status check inspection of this horizontal, Clinton Sandstone well. My inspection found Field Supervisor Cary Leshock on location with a roustabout working on this pumping unit. They lowered the rods and down-hole pump four (4) inches to try and improve well performance. I talked to Cary about the drilling pits and he said they would begin hauling the water to the Frame pad next week to prep for completion operations that are scheduled to begin on 2/18/19. No violations.
