0079266683 2018-03-05 Production Wells No Violations I met on location with Mr. Louis Showalter to observe erosion in the bean field that he claims is a result of the oil and gas activity on this property. Mr. Showalter does not own the property but has been farming this land for many years. He said he installed five (5) inch drain tile years ago to drain the upper field and that the he believes that the drainage tile was damaged or plugged up by the activity. He said the erosion has been occurring for over a year. There is a slope break from the upper field into this field 350 east of the well. It appears that water has discharged from the slope break and incised drainage channels into the bean field over one (1) foot in depth and varying width. A channel has also been incised along the slope break running north and south. The channels merge at the culvert under the lease road and have discharged silt and sediment into the adjoining field to the west. The drainage tile emerges in a ditch in the adjoining field and was discharging clear water. I could not make a determination at this time if the oil and gas activity is related to this erosion. The lease road (owned by Cedar Valley & Northwood) has issues, is unstable, and is causing sedimentation to occur in the adjoining field. I will revisit the location with Supervisor Tom Hill to determine what may need to be done.

1311352373 2011-07-22 Drill / Deepen / Reopen No Violations I witnessed the cementing of 8 5/8 casing by Petroset on Poulson rig #1. There were no violations at this time.
1523364476 2018-03-23 Production Wells No Violations I was on location with Regional Supervisor Tom Hill to follow-up on Complaint #5816 and to assess the erosions issues in the agricultural fields on the property. The complainant has claimed the issues in the fields were caused by the oil and gas drilling and producing activities of this well. I reviewed the area of concern with Tom. It was determined that the rill and gully erosion that starts approximately 560 feet east of the well, up slope, and runs west and south of the well, is not caused by the production activities. Reviewing of aerial photography confirmed our findings. A photo taken October 27, 2011 clearly shows that the more pronounced erosion starts approximately 360 feet east of the edge of the area cleared for drilling operations. Review of aerial photographs taken April 13, 1994 and December 31, 2003 clearly show that the erosional features in the agricultural fields predate any oil and gas activity on the property.

Minor rill erosion approximately 6 feet in length was found to be coming off the well site, southwest of the well. The owner will be contacted to repair the erosion, stabilize, and seed. The access road north of the well is showing signs of erosion. There is a low spot in the access road in the same area as where drainage exits the fields. The operator of that well will be contacted to address the access road. The access road appears to have been recently graded. The grading resulted in stone and soil to be furrowed along the access road. This material needs to be graded out and seeded. The rockier material could be used to repair the access road. The following is recommended: Traffic on the access road should be limited until the frost is out of the ground, to limit rutting that can cause erosion issues. The property owner is advised to contact Portage Soil and Water for assistance in addressing erosion issues on the property.

2065340932 2015-05-27 Preliminary Restoration No Violations I received a request from Columbus to check the status of this well as it was hydraulically fractured in 2011 and Hopewell Oil & Gas had yet to report any production. I also conducted a preliminary restoration inspection that is due for this location. My inspection found that the well is in production and equipped with a gas fired pumping unit. A gauge indicates 90psi on the 4.5 casing and 70 psi on the 2.375 tubing. The wellhead is located in an agricultural field and the site is graded and planted. The tank battery is diked and contains one (1) 100 barrel tank, one (1) 210 barrel tank, and a horizontal separator with a meter run. The chart was changed on 5/26/15 and the proper ID is posted at the tank battery. I contacted Steve Opritza in Columbus and he informed that the production report, was in fact, submitted, and at that time of his inquiry, the numbers had not been posted in the RBDMS Database. Preliminary restoration is approved. Ownership of the well was transferred to Siberian Solstice Energy on 5/29/15. No violations.
