0125963636 2014-08-13 Production Wells No Violations I inspected this well to follow-up on Compliance Notice #1392820343 that required Hall & Horning Oil Field Services Incorporated to properly plug and abandon this idle (non-producing), Berea Sandstone well by 8/25/2014. My inspection today found that nothing has changed with the status of this well. As of the date of this inspection, Hall & Horning has failed to take any corrective action or perform an activity to remedy the issues outlined in the compliance notice issued on 2/27/2014. This compliance notice remains open. Note: During my inspection, I found a minor natural gas leak where the 2 flow line enters the ground west of the wellhead. The well is shut-in and the valves on the flow line are closed, so the natural gas is back feeding up the flow line from an undetermined source. I spoke with Dan Horning and he is going to correct the problem.

My inspection today found that nothing has changed with the status of this well. The required ID signage has not been posted and the well remains idle. As of the date of this inspection, Hall & Horning has failed to take any corrective action or perform an activity to remedy the issues outlined in the compliance notice issued on 2/27/2014. This compliance notice remains open.

1330300797 2017-05-10 Production Wells No Violations Follow-up to Chiefs Order #2016-68, issued on March 25, 2016, requiring Hall & Horning Services Incorporated to properly plug and abandon this Berea Sandstone well within thirty (30) days of the receipt of the Order. My inspection today (5/10/2017) found that the well has not been plugged and abandoned as required pursuant to Sections 1509.062(A) and 1509.12(B) of the Ohio Revised Code and is in material and substantial violation of Chiefs Order #2016-67 as defined under Section 1509.01(EE)(4) of the Ohio Revised Code.

1392820343 2014-02-04 Production Wells Violations Noted I inspected this well at the request of the Uniontown office to check the idle status of this Berea Sandstone well. The operator has never reported commercial production for this well that was drilled in 2005. I met with the landowner and he was unaware of any production from this well. A plugging permit was issued to Hall and Horning on 12/14/2009 and expired on 12/14/2010 without being executed. My inspection found 7 surface casing above grade and equipped with 1.5 tubing and a gate valve. The tubing was equipped with a riser, ball valve, and 90 degree elbow that was not connected to anything. All valves were closed. 2 flow line is run off the surface casing and flow line runs to the west. The is no ID posted at the wellhead as required by law and there is no associated production equipment on location. Based on the fact that no commercial production has been reported from this well since it was completed in 2005, Hall and Horning Oilfield Services Inc. will, within 180 days, obtain a plugging permit as required by section 1509:13 of the Ohio Revised Code and properly plug and abandon the well in accordance with section 1501:9-11 of the Ohio Administrative Code. Failure to comply with this notice may result in further administrative action including bond forefeiture.
1443450436 2015-08-13 Production Wells No Violations I inspected this well to follow-up on Compliance Notice #1392820343 that required Hall & Horning Oil Field Services Incorporated to properly plug and abandon this idle (non-producing), Berea Sandstone well by 8/25/2014. My inspection today found that nothing has changed with the status of this well. During my inspection, I found a small gas leak emanating from the flow line where it enters the ground. All the valves on the wellhead and the flow line were closed so the gas must be back-feeding from an unknown source. I spoke with Dan Horning about the flow line and he told me he would fix the leak. As of the date of this inspection, Hall & Horning has failed to take any corrective action or perform an activity to remedy the issues outlined in the compliance notice issued on 2/27/2014. This compliance notice remains open. *Note: I spoke with Dan Horning on December 15, 2014 (12/15/2014) concerning the status of the idle (non-producing) wells remaining on the bond of Hall and Horning Oil Field Services Incorporated (H&H Inc.) and the chances that they would be plugged in a timely manner by H&H Inc. Dan explained to me that H&H Inc. was defunct as a corporation and had no money to plug the remaining wells.

My inspection today found that the gas leak discovered during an inspection on 8/13/2015 has been mitigated. The idle status of this well has not changed and, as of the date of this inspection, Hall & Horning has failed to take any corrective action or perform an activity to remedy the issues outlined in the compliance notice issued on 2/27/2014. This compliance notice remains open.
