0056765487 2017-04-21 Production Wells No Violations On location to follow-up on a phone call received 04/20/2017 from Mr. Brok Plymale complaining of excessive noise and leaking oil from the pump jack motor associated with this well, complaint #5108. At the time of my inspection Terry Whitehair DOGRM Emergency Response Coordinator, Steve Hillard (Hilco Well Services, Inc) contract pumper and Brok Plymale were on site. The pumping unit was shut down. Steve stated that it should have been running but had shut down. The pump had been running the day before; Steve gauged the tank and found that the well had not made any fluid. Steve indicates there may be an issue with the downhole pump. Inspection of the motor found that the bolts holding the exhaust manifold to the motor had broken off. The exhaust system on the motor consisted of steel tubing coming off the motor ran into a metal drum. The seal behind the flywheel had fresh motor oil on it. The operator had attempted to seal the leak with a motor sealant; the repair work was not successful in completely stopping the motor from leaking oil. There were absorbent pads under the motor, that where saturated with engine oil. I asked Steve Hillard to start the pump, which he did. The sound produced from the motor can be considered a nuisance. Brok Plymale indicated that he utilizes the area of woods around the well for recreational activities and the noise from the motor interferes with his enjoyment of the land. Steve Hillard was instructed to shut down the motor until repairs to the motors seals and exhaust system were made. Steve was informed that the motor could be run for a short period of time for evaluation, only during day light hours. Steve will inform the owner of the well of the instructions. I gave my contact information to Steve and informed him to have the owner call me if they have any questions. The well can continue to produce gas as long the motor is not ran.
0078976074 2016-03-23 Production Wells No Violations I was on location to follow-up on Compliance Notice #1457977852 and Complaint #5108. Mike Ingalls was on location and running the unit to test the downhole pump. He has placed absorbent pads under the motor to catch the oil leaking behind the bull wheel. The mulch they have placed around the unit is breaking down and generating a slight organic sheen. I discussed with him again about building a dike around the wellhead and that they still need to address the leaking motor to be in compliance. He said they will only run the pump when someone is on location and I asked him to keep any leaked oil from hitting the ground. This violation continues.
0374246387 2016-03-17 Production Wells No Violations I was on location to follow-up on Compliance Notice #1457977852 and Complaint #5108. Everything around the pumping unit looks in good order and there is no noticeable sheen on location. I had spoken with Mike Ingalls (KMFAD) on 3/16/18 and asked him to build a dike around the wellhead. I spoke with him today and he said he was going to bring in a load of stone and dirt on Saturday (3/19/16) to complete the work. The pumping unit motor still needs to be repaired prior to operating the unit. This violation continues.
0459932792 2016-09-21 Production Wells No Violations I was on location to follow-up on Compliance Notice #1457977852 and Complaint #5108. I received a call from the landowner (Brock Plymale) that the pumping unit was running, leaking oil on the ground, has no exhaust and is very loud. My inspection found the pumping unit in operation and leaking oil onto the rock below the motor and onto the pumping unit base. A two (2) foot diameter area below the motor is wet with oil. The muffler and exhaust pipe are lying on the ground and the noise being emitted from the manifold is unacceptable and a nuisance to the landowner. I called Carrie Moore and told her to shut the pumping unit off immediately. I told her that KMFAD may not operate the pumping unit until the leaks on the motor have been properly fixed and an appropriate muffler has been installed to mitigate the engine noise. The area under the motor will need to be cleaned again. This violation continues.
0676947689 2016-10-26 Production Wells No Violations I was on location to follow-up on Compliance Notice #1457977852 and Complaint #5108. I met with the pumper, Mike Ingalls, and he installed a new breather on the pumping unit motor which he said should solve the leak issue. He has also installed a new exhaust line and drum muffler on the motor. I told him he could run the unit, but if it continues to leak, it will need to be shut off and further repairs will need to be made. The rock below the motor still need removed as well as any contaminated material. I will follow-up later to see if the leak has been mitigated. This violation continues.
0822944525 2018-02-21 Production Wells No Violations Follow up to Compliance Notice #1457977852 and Complaint #5108. I received a call from the landowner that the pumping unit was in operation and that it was running throughout the night. I arrived on location and found the pumping unit in operation. Northeast regional Supervisor Tom Hill previously told Carrie Moore (KMFAD) not to operate the pumping unit until the leak is properly repaired. There are currently two (2) containers under the unit containing several gallons of leaked oil from the motor. The exhaust remains a nuisance and has not been repaired. The Murphy switch-gauge is no longer leaking anti-freeze. I sent Carrie a text message (2/22/18) as she has not been responsive to Supervisor Hills phone calls and messages. I asked her to shut the pumping unit off and to not run the unit until it is properly repaired. This violation continues.

The inspection found that the unit was not running. There are currently two (2) trays under the unit motor that have caught the oil leaking from the motor. The front tray is nearly full (~2 gallons) and is one inch from the top of the tray. The front and top of the motor housing and behind the bull wheel are wet with oil and it does not appear that the motor or leaks have been repaired. This violation continues.

1232180922 2018-06-29 Production Wells No Violations Follow up to Compliance Notice #1457977852 and Complaint #5108. The old motor that was the source of the Compliance Notice and Complaint was replaced in April of 2018. My inspection today found that the replacement motor is continuing to function properly without leaking oil and the stone below the motor is clean and dry. The required ID is posted at the wellhead and at the tank battery with a valid emergency contact number. The nuisance noise complaint has also been resolved with a properly functioning muffler. Northeast Regional Supervisor Tom Hill previously took several soil borings from around the pumping unit and did not find contamination that would require moving and excavating below the pumping unit. I spoke with the Complainant, Brock Plymale, on 6/28/18 and told him that the issues had been resolved and that the Complaint and Compliance Notice would be closed out. Mr. Plymale did not state any objections. This Compliance Notice is resolved and Complaint #5108 can be closed. No violations.
1444826331 2015-09-02 Production Wells No Violations Follow-up to Complaint #5108 concerning motor oil leaking out of the gear box of the pumping unit of this well. I spoke with the landowner, Mr. Plymale, at 11:23am and he told me he had shut the pumping unit off and called the emergency number on the ID sign. He said the company has not called him back. I told him I would be out to investigate this afternoon. I arrived on location at 12:45pm and inspected the area. My inspection found the wellhead equipped with a natural gas-powered pumping unit. There was less than 0.5 gallons of fresh motor oil on the ground under the base of the unit motor. The oil was mostly contained on the ground inside the base of the unit. Older, dried oil staining was found on the ground 2-3 outside the southeast corner of the pumping unit base and requires some minor clean-up. The well produces to a diked tank battery to the north and contains one (1) 210 barrel tank, one (1) 35 barrel poly tank, a vertical separator, and the associated meter run. There is a domestic hook-up in the tank battery connected to a drip bottle and a 1 supply line. ID is posted at the wellhead and the tank battery. I called the emergency number on the ID sign and left a message requesting a call back from KMFAD to discuss the issues with the leaking motor. I will wait to hear back from the company. This complaint continues.
1445404173 2015-11-03 Production Wells No Violations Follow-up to Complaint #5108 concerning motor oil leaking from the engine of a natural gas powered pumping unit. The operator has placed clay litter beneath the motor to absorb the oil and the motor continues to leak small amounts of product. The area around and below the unit motor still needs to be properly cleaned up and the leak needs to be fixed or properly contained. I will speak with Carrie Moore about addressing the issues. This complaint continues.
1457977852 2016-03-08 Production Wells Violations Noted Follow-up to Complaint #5108 which has been outstanding since 9/2/2015. I have spoken to the well owner, Carrie Moore, on numerous occasions about cleaning up the ground around the Abbe #3 pumping unit that has been affected by oil leaking out of the pumping units motor and fixing the source of the leak. As of the date of this inspection, no satisfactory clean-up efforts or repairs have been made or undertaken by KMFAD Oil and Gas, LLC. Kitty litter has been placed under the units motor to soak up the oil and a tray is now under the motor and has 2-3 gallons of oil inside of it and is in danger of overflowing. I found a small amount of oil in front of the wellhead that has leaked from the stuffing box and placed several pads on the affected area. Kitty litter has also been placed around the wellhead to soak up oil. KMFAD will immediately cease production operations of this well until the following issues have been addressed: 1) the leak on the pumping unit motor is repaired or mitigated, 2) the affected material under and around the outside of the pumping unit is removed and disposed of properly, 3) a containment dike is constructed around the wellhead (and around the pumping unit if needed) to contain any future leaks at the motor or wellhead (OAC 1501:9-9-05 (9)), and 4) a valid emergency contact phone number needs to be posted on the ID signage as required.
1466723574 2016-12-28 Production Wells No Violations I was on location to follow-up on Compliance Notice #1457977852 and Complaint #5108 with Inspector John Fleming. My inspection found that the pumping unit continues to leak oil onto the ground below the motor. I notified KMFAD that the pumping unit must remain off until the leak is fixed. This violation continues.
1518787881 2018-02-14 Production Wells No Violations Follow up to Compliance Notice #1457977852. My inspection today found the pumping unit in operation and continuing to leak oil from the packing behind the bull wheel. A plastic, automotive oil container has been placed under the unit to catch the leaking oil. Clean stone has been placed under the unit motor. I could not determine if the impacted stone and material below the motor has been removed due to the unit running at the time of my inspection. I found antifreeze leaking out of the Murphy Switch Gauge (a small drip every two (2) seconds) onto the rock below the unit. Also, the muffler has not been properly reattached to the bottom of the manifold and a piece of metal has been wedged under the exhaust line in an attempt to close the gap. The noise from the exhaust continues to be a nuisance. This violation continues.
1519407268 2016-02-12 Production Wells No Violations Follow-up to Complaint #5108. I met on location with KMFAD well pumper Mike Ingalls to go over what needs to be done to address the issues at this well. I told him that the contaminated material under the motor and around the back of the unit needs to be dug up and disposed of properly. I told him he may run the well if he contains the leaking oil to a tray and that it may not get on the ground. He will notify me when they are going to remove the contaminated material. This complaint continues.
1519674709 2018-01-17 Production Wells No Violations I was on location with DOGRM Regional Supervisor Tom Hill to meet with the owner of the well, Carrie Moore and the well tender, to discuss the operation of the well. At the time my inspection the well was in production and the pump jack motor was running. According to Carrie, modifications had been made to the muffler to reduce the amount of exhaust noise that is created from the operation of the pump motor. The crank case was not leaking at 11:45 am this morning. The well tender stated that they had corrected the issue with blue sealant. Supervisor Tom Hill requested that Carrie Moore fix the issue with the exhaust manifold correctly and have the seals on the pumping units motor repaired to prevent lasks. Tom indicated that the well may operate as long it is not leaking. Tom Hill and I returned to the well site at 3:00 pm. At that time, we found the well leaking oil and antifreeze. Supervisor Hill called Carrie Moore and left a message, informing her that the motor was leaking and requsted that she shut down the unit until repairs are made. 1519675634 2018-01-18 Production Wells No Violations I was on location to follow-up on the call I made to Carri Moore, to shut down the pumping unit until the motor can be repaired. At the time of my inspection the unit was running and leaking oil and antifreeze onto the ground. The well motor operation continues to pollute. The following needs to be completed: 1. The pumping unit needs removed, and any soil contaminated with oil and antifreeze shall be removed, and disposed of at an approved disposal location. 2. The packing on the motor needs to be repaired to prevent the unit from leaking oil. 3. The exhaust manifold needs to be repaired to reduce excessive noise generated from the operation of the motor. 4. The anitfreeze leak needs corrected. 5. The pumping units motor shall be shut off until all repairs and clean-up are completed.
1519840525 2018-02-28 Production Wells No Violations I received a call from the landowner, Brock Plymale, that the pumping unit at this well was turned on yesterday (2/27/18). My inspection today found the pumping unit operating in non-compliance with the Divisions request to not operate the unit until the leak in the motor and exhaust were fixed. Oil continues to leak and is being captured in a plastic oil pan. The exhaust discharged line has been wedged against the bottom of the manifold with stone, a wedge, and a hammer union, but still is not sealed at the manifold connection. This violation continues and a Chiefs Order will be requested to shut the unit down until properly repaired and the area under the unit is remediated. This violation continues.
1520433084 2016-03-15 Production Wells No Violations I was on location to follow-up on Compliance Notice #1457977852 and Complaint #5108. Owner Carrie Moore and pumper Mike Ingalls were on location and they began digging up the contaminated material around and under the pumping unit motor using shovels. They filled three (3) five (5) gallon buckets for removal and stockpiled the remaining material on a tarp for removal when it dries out. They brought in a load of mulch and placed the mulch around the unit and wellhead. A valid emergency contact number has been posted. The leak on the motor still needs to be addressed prior to running the pumping unit. This violation continues.
1523464098 2015-12-16 Production Wells No Violations Follow-up to Complaint #5108 concerning oil leaking from the motor of this natural gas powered pumping unit. My inspection found that oil has continued to leak out of the motor and that the ground below the motor has not been properly cleaned up. There is a streak of oil in front of the wellhead that was discharged from the tubing blow-down valve and I placed an absorbent pad on the oil. This Complaint continues.

The pumping unit is currently not in operation and there is no oil on the ground under the motor. There is an organic sheen being generated by the mulch around the wellhead and it is not oil. A load of stone has been delivered to location and I have not been notified that any repairs have been done to the pumping unit motor. This violation continues.

1731295485 2018-01-11 Production Wells No Violations Follow-up to Compliance Notice # 1457977852 and Complaint #5108. My inspection today found the natural gas powered pumping unit in operation. The requested repairs to the pumping unit motor have not been achieved and the unit continues to leak oil behind the bull wheel and is dripping on the rock below the unit motor. The muffler connection at the exhaust manifold continues to be an issue and is not properly connected/secured to the manifold. The noise from the exhaust is a nuisance to the landowner and I have asked KMFAD to not operate the well on numerous occasions until the leak and exhaust have been repaired. Attempts to contact the owner Carrie Moore on her cell phone and the listed number on file with the Division of Oil & Gas found that neither number was currently a valid contact for the company. I left a message with the well tender, Mike Ingalls, to shut the well off, and he responded on Friday evening that it would be shut off on 1/13/2018. He also gave me a new contact number for Carrie Moore (740-610-8981). The emergency contact number on the ID signage needs to be updated again. This violation continues.
2059096955 2015-10-26 Production Wells No Violations Follow-up to Complaint #5108 concerning oil leaking from the motor of this natural gas powered pumping unit. My inspection found that no activity has been undertaken to clean up the area below the unit motor and around the unit base since speaking with Carrie Moore on 10/23/15. The area in front of the wellhead is sloppy and there is a minor amount of oil on the ground that I placed an absorbent pad on to soak up. This Complaint continues.
2146966210 2018-04-13 Production Wells No Violations Follow up to Compliance Notice #1457977852 and Complaint #5108. I was notified by Supervisor Tom Hill that KMFAD owner Carrie Moore had informed him that the pumping unit motor at this well had been replaced. My inspection today found that the motor of issue has, in fact, been changed out. The old, leaking motor was on a trailer and parked in front of the Abbe-Lowell #1 to the north. I observed the motor in operation and there was fresh, standing oil on top of the crank case around the fill plug. I wiped the oil off and did not find anything leaking at this time. The replaced motor has a functioning muffler that is properly attached to the manifold and is no longer a nuisance noise issue. I will check back later to confirm that the replaced motor is not leaking oil.
