0917059576 2010-10-05 Plug / Plug Back No Violations In the spring of 2003, the tubing in this well failed at the surface and went down the hole. In the process, the rods and the production casing parted and also went down the hole. Since then PEP has done nothing to repair the well and has recently decided to plug and abandon. For the past two weeks, CHIPCO has been onsite repairing the wellhead, reconnecting the 4-1/2, and recovering rods and tubing. To date they have recovered approximately 3400 of bent 2-3/8 tubing and rods and do not feel that additional tubing or rods can be recovered. PEP requested and was granted permission to set the bottom plug in the top of the Big Lime. Plugging will begin tomorrow when Appalachian Well Service will be on site to cut the 4-1/2 casing above the Salina.

1117481216 2003-07-15 Production Wells No Violations The well is no longer leaking gas. However, the well is still idle and PEP has not proposed any corrective action to repair the well. A plugging permit has been issued.

1224661546 2013-07-10 Final Restoration No Violations My inspection showed no evidence of the well, tank(s), separator or line associated with this permit number. The area around the well has been graded and vegetation has been re-established. The lease road is being maintained with limestone as agreed upon verbally by the landowner. Final restoration passed.

1637682983 2010-10-12 Plug / Plug Back No Violations The last plug stayed up and the well has been covered over. Mark Smith was on site this day with the backhoe to start reclamation operations. The landowner requested the lease road stay and the limestone from the tank battery.

2109130317 2010-10-06 Plug / Plug Back No Violations Appalachian Well Surveys was on site to cut the 4-1/2 casing off in order to begin plugging operations. The cutter sat down at approximately 2100 which was approximately 900 higher than anticipated. The cutter was pulled and there was parafin on the bottom and sides of the tool. The decision was made to run tubing in the hole to circulate out the parafin. Activity will resume tomorrow. The violation is resolved.
