0150689981 2003-04-01 Production Wells Violations Noted Status check inspection - the well operation is causing pollution around the wellhead. The cause may have been stuffing box leak. There does appear to have been some attempt to clean up the spill. There is still impacted soil north and east of the wellhead. The tank battery for this well is in good condition. I contacted Buzz West with Orion to discuss the operation of this well.
0209490726 2017-09-25 Production Wells No Violations I was on location with DOGRM District Supervisor Steve Ochs to follow-up on Compliance Notice #-1942129359. A load of clean fill dirt has been dumped southwest of the tank battery. The two (2) 50 barrel, steel tanks have been steamed and are lying on their sides inside the tank battery. The 210 barrel tank has been steam cleaned and the vent stack has been removed. The well continues to sell natural gas and the chart at the sales meter was changed today. There is currently no activity on location. This violation continues.

I extended the compliance date to 9/11/17 on 8/28/17 to accommodate the time required for soil testing to determine the proper disposal procedure. My inspection today found the location stable and no further migration of brine or oil away from containment. I will check back next week to see if remedial operations have commenced. This violation continues.
0553393502 2017-10-20 Production Wells No Violations Follow-up to Compliance Notice #-1942129359. My inspection found the tank battery has been rebuilt. The dump line from the separator has been plumbed to a 50 barrel tank. The dike walls on the west and north side of the battery have been rebuilt and the scalded areas outside of the dike have been remediated and seeded. New grass is beginning to re-establish in these areas. All the affected material has been removed and disposed of at Waste Managements American Landfill. This violation has been resolved.
0572267958 2017-08-28 Administrative Inspection No Violations I spoke with Pin Oak Operations Manager Ted Lawver today (8/28/17) concerning this well location and the time line for remedial activities. Ted told me that Lakeside would be taking soil samples this week for analysis to determine proper disposal procedures. He said that it would probably be 1-2 weeks before they receive the test results at which time they would proceed with clean-up operations. I have extended the remedy required date to 9/11/17.

1449524817 2017-09-26 Production Wells No Violations Follow-up to Compliance Notice #-1942129359. Lakeside Oilfield (Ron Hammen) was on location continuing with clean-up operations. The good 50 barrel tank was spotted on the other side of the lease road for later use. The bad 50 barrel tank was placed in the back of a dump truck and the 210 barrel tank was strapped to the trailer. Both tanks were removed for disposal at a scrap yard. They are going to steam the rock in the tank battery and push it to a sump where it will be vacuumed out tomorrow. This violation continues.
1506531218 2017-09-27 Production Wells No Violations I was on location to oversee clean-up operations in reference to Compliance Notice #-1942129359. Lakeside Oilfield was on location and dug a sump at the west end of the battery and steamed the rock with 250 gallons of freshwater. J. Hawk Water Hauling was sucking the fluid out of the sump with their water truck. I tested the water in the sump with a Quantab and found it was over 1000ppm chlorides. I requested that steaming continue with the remaining 250 gallons of water. After flushing again, I tested the water and found it below 300ppm chlorides and excavation and removal of saturated materials began in and outside of the containment dike. Lime was spread on all of the excavated areas to buffer any residual chlorides and clean top soil was replaced where the saturated materials had been removed. The dike walls were rebuilt and the foot print of the battery was shrunk down. I will check back later on progress. This violation continues.

1525877287 2018-05-09 Production Wells No Violations Status check inspection of this Rose Run Sandstone well. My inspection found the wellhead equipped with 2.375 tubing and rods and a natural gas powered pumping unit. The tubing valve is currently closed and the 4.5 production casing valve is open. This well produces to a diked tank battery to the north that was remediated in the fall of 2017 and vegetation has been well established around the perimeter of the battery that was previously impacted by salt water. The battery contains one (1) 50 barrel tank, a vertical separator, and an associated meter run. There is a line heater between the wellhead and tank battery that is no longer in use and a domestic tap and meter is plumbed to the flow line at this point. A gauge on the line heater indicates a pressure of 140psi. The required ID is posted at the tank battery. No violations.

1904592371 2017-09-19 Production Wells No Violations Follow-up to Compliance Notice #-1942129359. My inspection today found that remedial activities have not yet been undertaken by Pin Oak Energy. The situation is static and there has been no further migration of the brine scald or oil inside containment. I spoke with operations manager Ted Lawver on Friday 9/15/17 and he told me that they finally received the results of the material testing and that they would begin remedial operations today or Wednesday 9/20/17. I will check back tomorrow to see if activities have started. This violation continues.
1942129359 2017-08-22 Production Wells Violations Noted Status check inspection of this Rose Run Sandstone well. My inspection found the wellhead equipped with a natural gas powered pumping unit. The well produces to a diked tank battery that contains one (1) 210 barrel tank, two (2) 50 barrel tanks, a vertical separator, drip bottle, and the associated meter run. There is a line heater between the wellhead and tank battery that is not in use. My inspection of the tank battery found an area measuring 18 wide and 30 long inside the central and north sides of the containment area that has been impacted by crude oil and brine. The rock is stained red and black and the staining reaches one (1) foot up the interior dike wall. There is coagulated crude oil sludge throughout and around the interior perimeter of the containment area. The western side of the containment area has standing water and thick, coagulated sludge. I ran a high range Quantab (Lot #5243) on a sample of the water. The Quantab unit value indicated 4.8 which corresponds to a chloride level of 1037ppm. Brine has migrated through and under the containment wall and impacted a 20x6 area on the north side, and 10x6 areas on the northwest and southwest corners. These areas have been scalded and are primarily devoid of vegetation. I spoke with Mark Van Tyne (Pin Oak Energy Partners), at 2:10pm, and informed him of my findings. I requested that the contaminated material be excavated and removed for disposal and the impacted areas be remediated. He said he is sending someone out to assess what remedial work will need to be done. Based on the appearance of the material inside the containment area and impact of the brine scalding damage, this was not a recent release and has been impacting the area in and around the tank battery for some time. DOGRM Emergency Response Coordinator Fred Rome arrived on location to assist me with the assessment. It had just rained and a high range Quantab was run in the standing water of a deer footprint in the 20x6 impact
1955951383 2017-09-20 Production Wells No Violations Follow-up to Compliance Notice #-1942129359. My inspection today found that remedial activities have not yet been undertaken by Pin Oak Energy. Ron Hammen from Lakeside Oilfield called this morning at 7:30am about starting clean-up operations at this location and the call was lost. They excavator is currently on location and the lease road has been graded and widened. I spoke with Ron again at 2:10pm and he will be in with the water truck in the morning to get the tank steamed out before moving it. I will check back on operations tomorrow (8/21/17). This violation continues.
