0094826428 2018-04-10 UrbanProduction Wells No Violations Status check inspection of this Clinton Sandstone well. My inspection found the wellhead fenced and locked, equipped with 2.375 tubing and rods, and operating with the aid of an electric powered pumping unit. This well produces to a diked, fenced, and locked tank battery to the east and contains one (1) 100 barrel tank, a 30 barrel poly tank, and a horizontal separator. The required ID is posted at the wellhead and at the tank battery. No violations.

1382558910 2013-10-23 UrbanProduction Wells No Violations I was on location at the request of Jeff Schonauer (JR Smail) to investigate possible contamination of a water well located by this oil and gas well. Jeff responded to the landowner the previous day (10-22-13) and his gas meter was triggered by running hot water in the shop restroom. His meter did not have the capability of distinguishing the source of the alarm and he contacted me asked if I could investigate. I met with Jeff and the landowner this morning to investigate. I calibrated the PHD6 gas meter prior to entering the premises. I spoke with the landowner and he indicated to me that the well water had a strong odor and was not sure if it was gas or oilfield substances and smelled like sulfur. He was not sure how long it has been like that and that he only notices it when he is not around the shop for awhile and returns. He said several customers have also noticed the smell. He took me into the restroom where he showed me the water softner, pressure tank, and sediment filter that were installed. The sediment filter appeared black and he told me he changes it once every week. There was an obvious sulfur odor around the equipment, but I did not detect any methane. I began running cold water in the sink and analyzing with the meter. I ran the water for several minutes and registered a reading of 0.05ppm on the CH4 (methane) gauge for several seconds and it returned to 0ppm. No LEL (lower explosive limit), H2S (hydrogen sulfide), or CO (carbon monoxide) was detected. I began testing the hot water and in about 30 seconds the CO levels were at 35ppm and triggered the alarm on the PHD6. The meter reading topped out at 63ppm of carbon monoxide and the LEL reached a reading of 2%. Methane was detected at a concentration of 0.10ppm and no H2S was detected during the hot water test. The hot water had an obvious sulfur smell during the test. I ran a low-level Quantab to test for chlorides and registered a 1.6 on the strip which equates to 43ppm of chlori
