0319394027 2007-11-15 Production Wells No Violations On Saturday November 10, I received a call from Jerry Reeves with David Waldron reporting a spill at the tank battery. A valve failed and the stock tank was overfilled. The oil and brine spill was contained by the tank battery dike. The initial response included a back hoe and a vac truck to isolate and recover free product. A NOV will not be issued if the remediation response continues in a timely fashion.
0409218898 2013-03-01 Production Wells No Violations I received a phone call from Ted Cutright (Waldron & Associates) about an oil spill at this location. Ted told me that the flow line salted off and overpressurized the stuffing box on the pumping unit causing oil to seep out at the wellhead. I arrived on location and met with Dennis Kingsberry. There was oil standing in the dike around the wellhead and pumping unit. At the northwest corner of the dike there was a small area where it was apparent that oil had seeped through the containment. We walked the location to survey the extent of the release. The spill had channelized to the northwest and to the northeast. The area to the northwest was impeded by fallen brush and trees. Some of the oil had made its way around the impediment and just into the edge of some brush and trees that was very wet. The area to the northeast was contained almost entirely in a channel 20-30 feet behind the well head. I went into the brush and wet area just past here and found a minor amount of sheen about 10 into the brush. I instructed them to get booms and absorbent pads in place before disturbing the affected areas to prevent any further migration of the spill. They were going to construct a small, lined pit as a holding area for the affected soil where they could mix it with lime and peat. I estimate that 1.5-2 barrels of oil have been released outside of the containment affecting an area of approximately 2000 square feet. The crew began placing booms and absorbent pads around the perimeter and I was satisfied that the spill had been contained on the perimeter. I will check back on Monday to monitor the progress.
0469475576 2013-03-19 Production Wells No Violations I was on location to check the clean-up status of an oil spill that occurred on 3/1/2013. I walked the location and found eight areas with minor amounts of free oil ponded. The ponded oil was in tracks and footprints in the mud and will need to be soaked up and removed. I will notify Ted Cutright (D.A. Waldron). The affected soil pile was covered and no oil was found outside the booms on the perimeter. This clean-up continues.
1145996289 2006-04-19 Production Wells Violations Noted I received a call from Jerry Reeves with Waldron & Associates, reporting an oil release at the Camp Asbury #3 well. The flowline apparently salted off with the resulting pressure buildup causing a failure at the stuffing box. The oil ran downgradient from the wellhead to the edge of a wetland. The oil was trapped by both vegetation and deadfall and did not impact more than ten feet into the dry portion of the wetland. The total affected surface area was approximately 1500 square feet. Waldron personel and a BDB Energy vacuum truck were on site when I arrived and were in the process of stockpiling impacted vegetation and soil and recovering any ponded oil and/or brine. The impacted soil and vegetation will be removed and properly disposed. The affected area will be seeded and mulched.
1251881757 2013-04-03 Production Wells No Violations I was on location to check the clean up of this oil spill from 3/01/2013. The base of the pumping unit has been sufficiently steamed out. The site is still not dry enough to get in and haul out the affected soil. The booms are still in place and there is no oil running of location. This clean up continues.
1283706674 2013-03-07 Production Wells No Violations I was on location with Ted Cutright (D.A Waldron) to inspect the progress of the oil clean-up on this location. We walked the location and examined the perimeter of the location. Most of the affected soil had been stockpiled in the lined dike behind the pumping unit. There were several areas inside the contained area that had free oil and they were going to use peat and absorbent pads to remove the oil. I spoke with Ted about the ultimate disposal of the affected soil and explained to him that I would like it hauled away to a proper disposal facility and not remediated on location. We agreed that this would be done as soon as the site dries out enough to allow the trucks access. I inspected the area around the pumping unit and requested that they dig a sump in front of the unit and steam the rock pad supporting the pumping unit and inside the dike. They would be able to pump the free oil out of the sump and into a water truck for disposal. Ted agreed and I will check back on the progress next week.
1362758137 2013-03-04 Production Wells No Violations I was on location to follow up on the progress of the oil spill clean-up from 03/01/2013. Waldrons crew was on location spreding peatsorb to soak up any free oil on loctaion. A small lined pit had been constructed and the affected soil was being placed in the pit and mixed with lime and peat. I walked the perimeter of the affected area and it appeared that the booms and pads were containing the release on the location. This clean-up continues.
1363622003 2013-03-13 Production Wells No Violations I was on location to inspect the clean-up status of an oil spill that occurred on 03/01/2013. Two of Waldrons crew were on location and searching for any free oil seeping out following the recent rain. They found several minor areas of residual oil and were using pads to soak it up. The booms were still in place around the perimeter and there was no free oil getting past the booms. The site was very muddy and it will not be possible to get any equipment in until it dries out. The lined pit containing the affected soil was covered and not leaking. This clean-up continues.
1365073278 2013-04-17 Production Wells No Violations I was on location to check the clean up status of an oil spill that occurred on 3/1/13. The contaminated soil has been removed removed from the location. The pit liner has been removed and the pit area has been graded. Booms are still in place on the north side of the location and there is no free oil near the booms. Several areas of standing water showed a slight sheen and I saw several small globs of oil. I will call Ted Cutright and let him know that it needs to be soaked up with pads. This clean up continues.

1815581226 2017-12-15 Production Wells No Violations Follow-up inspection to see if a damaged poly tank has been removed from the tank battery that was found on 10/2/17. My inspection today found that the tank has been removed. The battery is fenced and locked, the well remains shut-in, and the required ID is posted. No violations.
1857391342 2017-10-02 Production Wells No Violations The flow line valves on the tubing and 4.5 production casing are currently closed and the well is shut-in. The well produces to a diked, fenced, and locked tank battery to the west and contains two (2) 100 barrel tanks, a 35 barrel poly tank, and a horizontal separator. The top of the poly tank has been damaged and is cracked, open and full to the point that a gallon of oil has leaked out the cracks and onto the ground inside the containment area. I called Pin Oak Operations Manager Ted Lawver and informed of my findings. I asked him to have the tank emptied and removed. He said he would take care of the tank within the week. I will check back later to see if the tank has been removed.
