0335877345 2017-06-05 Production Wells No Violations Follow-up to Complaint #5159 concerning an oil release at this wellhead that occurred on 10/27/15. The contaminated soil was removed to Kimble Landfill on 11/30/15. My inspection on 12/3/15 found that all the contaminated material had been removed from location and was rough graded. My inspection today 6/5/17 found no issues pertaining to the original oil release and vegetation is well established around the location. This complaint has been satisfied. No violations.
0375479914 2015-03-18 Production Wells No Violations I received a call from Joe Teppirt (Enervest) to report that a flow line leak had occurred at the tank battery of this well. I arrived on location and Scott Miller Contracting was on site preparing to begin clean-up operations. I inspected the location and found that the flow line had formed a leak 3 north and outside the containment dike. The impacted area was along the lease road and was 10 wide and stretched 70 to the east. The release followed the ruts in the lease road and was under some snow cover in areas. 5-10 gallons of oil were released and did not impact any waterways or field tile. I had Scott Miller construct a siphon dam just east of the extent of the release and they began steaming the impacted areas along the lease road and flush the oil to the collection area in front of the dam. I inspected the tank battery and it was fenced, diked, and contained 2-210 barrel tanks and a horizontal separator. The proper ID was posted at the tank battery. The wellhead is located 0.25 miles due north of the tank battery and is equipped with an electric pumping unit. The unit is currently turned off and the well is shut-in. The proper ID is posted at the wellhead. I will check back later on clean-up progress.
1231039832 2015-03-18 Production Wells No Violations I was on location to follow-up on a flow line leak that occurred earlier this morning. Scott Miller Contracting was flushing the affected area with steam and hot water and collecting the product in front of a siphon dam that was constructed. Clean-up is progressing and will continue tomorrow. The siphon dam will be left in place to collect any residual oil and I will continue to monitor the location for any issues.
1403660905 2015-10-27 Production Wells No Violations Follow-up to Complaint #5159 concerning an oil leak at this wellhead. The Uniontown Office notified me of this spill at 11:38am and Joe Teppirt (Enervest) contacted me at 12:38pm to notify me of the release. I was on location at 1:00pm with Enervest personnel and Scott Miller Contracting (SMC). Apparently the timer on the pumping unit failed and the well pumped all weekend. The cause of the leak was a failure of the stuffing box seal. The pumping unit had settled/shifted causing the polished rod to rub and wear out the seal. 2-3 barrels of oil had leaked from the stuffing box and ran down grade from the wellhead affecting an area of 30 X 10. The oil did not enter waters of the State. Due to the forecast of heavy rains this evening and tomorrow, I advised SMC to build a diked containment area and had Joe Teppirt get a pit liner to line the containment. I had SMC excavate the affected soil and place it on the liner inside the containment. The liner was folded over the material and secured to keep any rain from leaching out the soil pile. SMC constructed a siphon dam with one (1) 4 and one (1) 6 PVC pipe at the north side of the affected area (down grade) to catch any residual oil washed out by the rain. The affected soil will be disposed of at an approved landfill when site conditions allow its removal. I will continue to monitor the location for any issues.
1450361594 2015-11-03 Production Wells No Violations I was on location to follow-up on Complaint #5159 in regards to an oil leak that occurred on 10/27/15. The siphon dam is functioning properly and the water propagating through the dam drainage tubes is clear. The water on the containment side of the dam has a minor amount of sheen and absorbent pads are collecting any residual oil on the waters surface. The contaminated soil is wrapped and contained inside a pit liner. I will continue to monitor the location until the soil is disposed of and the siphon dam is removed.
1624165987 2015-10-30 Production Wells No Violations Follow-up to Complaint #5159. The affected soil is wrapped in a pit liner and is surrounded by a containment dike. There is nothing leaching out of the covered soil. The siphon dam is functioning properly and pads have been placed on the leached oil behind the dam. I found no oil or sheen past the siphon dam. The site is saturated with rain water and small amounts of residual oil have leached out around the wellhead. The site will need to dry out before the affected material can be removed. I will continue to monitor this location.
1662250252 2015-12-03 Production Wells No Violations I received a call from Joe Teppirt (Enervest) on 11/30/15 to inform me that the contaminated soil has been removed from location and disposed of at Kimble Landfill. I inspected the location today and found that the soil has been removed, the siphon dam has been taken down, and a dike has been built around the front of the wellhead and pumping unit. The pumping unit has been re-spotted and leveled. The north side of the location where the siphon dam was constructed has been roughly graded and is very muddy and water saturated. The contamination issues at this location have been resolved. I will continue to monitor this location for any residual oil.
