0073944028 2015-06-29 Production Wells No Violations Follow-up to Compliance Notice #1435687915. I was on location and met with Field Supervisor Ted Cutright (DA Waldron). We discussed issues on this location and what needs to be done to clean up the tank battery. Ted is going to start taking the fence down so the equipment can start being pulled from the tank battery. I will check back later on progress. This violation continues.
0273368632 2015-07-16 Production Wells No Violations Follow-up to Compliance Notice #1435687915. I was on location with Ted Cutright (Waldron) to check on clean-up progress of the tank battery. We discussed the disposition of the two (2) brine vaults and how to decommission them. Each of the vaults is a one-piece concrete box and the tops contain rebar. It will require a jack-hammer and chop saw to cut the lids off. We decided that the residual liquid in the bottom of the vault would need solidified. Ted poured a bag of portland cement and a bag of sand in the vault. After thirty minutes, the concrete began to set. Ted will bring in a load of ready mix next week and solidify the bottoms. I tested the water that has been collecting in the excavated hole with a Quantab and the range was between 500-600ppm chlorides and can be discharged to dry the location out for reclamation. This clean-up continues.
0336463523 2015-07-27 Production Wells No Violations Follow up to Compliance Notice #1435687915. I was on location to check the status of clean-up operations. The brine vaults have been properly removed from service and buried. The area of the former tank battery has been graded and is dry and clean. The affected soil pile is on containment and has been covered with lime prior to removal. I will close the compliance notice once the contaminated soil is disposed of properly. This violation continues.

The fencing and posts have been removed and the two (2) horizontal separators have been pulled out of the battery. Ted Cutright (Waldron) excavated a hole inside the battery and found a layer of clay 2 down that had isolated the contaminated soil above it. The soil and gravel towards the tanks (east) is more saturated than the excavated material to the west where the separators were pulled out. The impacted soil will be placed on a liner and be disposed of properly. The company is going to work on getting the tanks pulled out and continuing to excavate. This violation continues.

0940938254 2014-04-02 Annular Disposal No Violations I inspected this well at the request of the Columbus Office for annular disposal. The well was operating with the aid of an electric pumping unit. I found no evidence of annular disposal at this wellhead. No violations.
1025150410 2015-07-01 Production Wells No Violations Follow up to Compliance Notice #1435687915. I was on location to inspect the clean-up progress at this tank battery. Waldron employees were on location and have loaded both horizontal separators onto a flatbed trailer and are strapping them down for removal. Both of the 140 barrel tanks have been pulled from containment and spotted to the southwest. I spoke with Ted Cutright (field supervisor) and he informed me that in lieu of rebuilding the tank battery and replacing equipment, the company was going to remove all equipment, remediate the affected area, and plug both the Scott #s 1 & 2. This clean-up continues.
1188805817 2009-04-29 Production Wells No Violations A small amount of oil sludge has accumulated behind the siphon dam. Otherwise the spill appears to be remediated. I will check one more time before closing the file.
1239627395 2009-04-06 Production Wells No Violations Ted Cutright with Waldron & Associates called and reported a crude oil spill he attributed to a salesline leak south of the Scott tank battery. The leak point is approximately 230 south of the tank battery. The impacted area extends approximately 1400 to the east following an ephemeral stream bed. Crude oil collected at various points along the way behind deadfall or other organic debris. Reggie Brown with OEPA-DERR was on site before I arrived and had Waldron install a siphon dam just east of a culvert at the Bossow tank battery. The remediation plan calls for 1) opening the stream channel by removing debris and leaf litter, 2) maintaining the siphon dam and recovering oil and 3) repairing the line leak.
1240231705 2009-04-14 Production Wells No Violations Crude oil recovery is continuing at the siphon dam. A majority of the oil appears to have been recovered. What remains is caught in vegetation lining the stream banks. The weather forcast is calling for rain which should help flush the system.
1260885476 2015-07-23 Production Wells No Violations Follow up to Compliance Notice #1435687915. I was on location to check the status of clean-up operations. The vault bottoms have been steamed, cleaned out and solidified. Ted Cutright (Waldron) is currently breaking up and collapsing the vaults with a skid steer mounted jack hammer. The rebar in the vault lids and walls is being cut with a chop saw. After collapsing the vaults, the holes will be backfilled with the clean gravel and fill dirt that is on location. The contaminated soil is on containment and will be removed later. I will follow-up next week. This violation continues.

1314079122 2009-04-07 Production Wells No Violations Work is continuing on recovering oil from the drainage swale. Waldron personnel are removing brush dams and washing product downstream toward the siphon dam. Northeast Ohio Oilfield Service is stopping daily to remove any product collected behind the dam.
1435687915 2015-06-26 Production Wells Violations Noted I was on location to conduct an annular disposal inspection for the Scott #2 (PN# 23224) and found the tank battery for this well and the #2 grossly impacted with oil and brine. The horizontal separator for this well is not hooked up and there is a trash pump attached to the flow line and run to a brine vault outside the east edge of the tank battery and containment. The brine vault had been over filled and oil staining stretched down the lease road to the north for 150 (3 in width) and is covered with a coating of peat. The diked area of the tank battery measured 30 x 30 and the soil inside containment was thoroughly saturated and oil staining on the dike walls is 6-8 high. Absorbent pads, old and new, were scattered and layered throughout the battery in an attempt to contain the oil. Based on the amount of saturation and the pockets of oil found throughout the tank battery, contamination has been occurring at this location for some time and this was not a one-time event. I contacted Ted Cutright (Field Supervisor) and informed him of the violations and my findings at this location. Remedial procedures will begin immediately on Monday (6/29/15) and David A. Waldron & Associates is required to: 1) Remove all equipment inside the tank battery; 2) excavate and remove the contaminated soil to an approved landfill and replace the soil with new fill; 3) properly remove and retire both brine vaults from service. The tank battery may be reconstructed after these requirements have been completed and approved.
1437751270 2015-07-22 Production Wells No Violations Follow up to Compliance Notice #1435687915. I inspected the brine vaults and found that the bottoms have been solidified with sand and concrete. The rain water that has been collecting in the excavated hole has been pumped out and the site is drying out. I spoke with Ted Cutright (Waldron) and he will be bringing in a jack-hammer tomorrow to start breaking up the vaults for burial. This clean-up continues.
1443816285 2015-10-02 Production Wells No Violations Follow-up to Compliance Notice #1435687915. My inspection today found that the contaminated soil stockpile has been removed and hauled to an approved landfill. The liner that the affected soil was on has also been removed. The affected area of the former tank battery has been remediated, reclaimed, and graded. David A Waldron & Associates has been issued plugging permits for the Scott #s 1 & 2 (PN#s 23223 & 23244) and will not be rebuilding the tank battery. This compliance notice has been satisfied and is closed. No violations.

1661201475 2015-07-08 Production Wells No Violations Follow-up to Compliance Notice #1435687915. I was on location with Ted Cutright (Waldron) and Northeast Oilfield Services. Waldron was steaming the inside of both brine vaults and Northeast was using their vac truck to collect the residual oil and sludge from inside the vaults. The Northeast driver would not remove the filter from the end of the suction hose and 4-5 of residual with debris was left in the bottom of the vaults. 40-50 barrels of residual brine and oil were recovered from the vaults. Ted was going to get another vac truck with no filter on the suction hose to finish the clean-out. The affected soil has been excavated and separated from the gravel and soil that may be re-used. Rains are in the forecast until next week making it impossible to finish reclaiming the site until it dries out. I am extending the compliance date for 30 days to finish reclamation. Progress is being made on the clean-up.
