0136646778 2013-08-12 Final Restoration No Violations Upon peforming a final restoration inspection on Plugged and Abandoned well 34-133-20071, I encountered this Production well and found runoff from the vault. After inpsecting the vault, I found that it was at capacity (see photos). The well card on the battery tank is incorrect and could not be found in RBDMS. The correct well number is 34-133-23005-0000, which corresponds to Lallathin #1. Lat/long in RBDMS for 23005 is for the well, and the battery tank is located at 41.07375, -81.22310, a sizeable distance away. Entry to battery tanks is along New Milford Rd approx. 3500ft north of Wilkes Rd.

The well is currently shut-in as ordered. The contamination at the tank battery has not yet been addressed but the source is turned off. I spoke with Enervest and they will begin performing the requested remedial activities the first part of next week. This violation continues.
0636357264 2015-08-14 UrbanProduction Wells No Violations I was on location to follow-up on Compliance Notice #-653100279. My inspection found that all the contaminated soil has been removed and replaced with clean fill. The dike around the tank battery has been rebuilt and the fencing has been repaired. The footprint of the tank battery has been minimized by 20 in length. A 35 barrel poly tank has been spotted in the battery, but is not plumbed into the separator as of today. The well is currently shut-in. The compliance issues have been resolved at this location. No violations.
0653100279 2015-07-30 UrbanProduction Wells Violations Noted After inspecting the wellhead for annular disposal, I inspected the tank battery for this Clinton Sandstone well. My inspection found a 10 x 30 area on the floor of the tank battery that has been grossly impacted by brine and crude oil. Based on the location of the impacted area, it is apparent that the 210 barrel tank is leaking or has leaked from its bottom and the brine vault is being used for the storage of the produced fluids. The brine vault is currently filled to capacity and has leaked brine and oil through the seams on the lid. I excavated three (3) small holes 4-6 deep and found saturated soil, gravel, and buried absorbent pads. The operator has tried, unsuccessfully, to cover up the contaminated spill area by cutting 18 out of the south side of the dike and covering over a previous spill. The fencing is now leaning over and the cemented post holes are sitting on the ground. Based on the evidence found on site, this was not a one-time occurrence and has been causing pollution and contamination for some time. Enervest Operating will, 1) Immediately shut this well in to prevent any further contamination; 2) remove the leaking 210 barrel tank; 3) excavate and remove to an approved landfill, all contaminated and affected materials; 4) empty and steam clean the brine vault and properly remove the vault from service; 5) rebuild the containment dike and equip the well with a tank capable of containing the produced fluids. Following the approval of completion of these five (5) items, the well may be placed back in production.
0731845672 2015-07-30 Temporary Abandoned Annular Disposal No Violations I was on location to inspect for the use of annular disposal. The wellhead is equipped with a Demco gate valve and produces with the aid of a plunger lift system. The 2 flow line exiting the wellhead is equipped with a Kimray motor valve. My inspection revealed a minor gas leak at the tubing slips and packing. I will contact Enervest to fix the leak. I found no evidence of annular disposal being used at this time. No violations.

2122469504 2015-08-13 UrbanProduction Wells No Violations I was on location to follow up on Compliance Notice #-653100279. Sutton Pump and Supply was on location and has steamed and removed the 210 barrel tank. The brine vault was steamed and cleaned out, and the vault was crushed and buried. Contaminated soil was found six (6) feet below grade along the edge of the brine vault and excavated. Sutton estimated 40-50 tons of affected material has been removed from inside the tank battery. The contaminated soil is being hauled to the Kimble Landfill for disposal. The footprint of the tank battery will be reduced and the containment dike will be rebuilt. I will check back tomorrow on progress.
