0228515308 2007-04-11 Production Wells No Violations There was no evidence or free oil or a sheen at the release site. I contacted Great Lakes and asked them to remove all of the sorbent pads and booms and schedule the reclamation as soon as possible. The violation has been corrected.
0291327986 2017-07-24 Production Wells No Violations Status check of this Clinton Sandstone well. The wellhead is equipped and operating with the aid of an electric powered pumping unit. Gauges indicate 98psi on the 2.375 tubing and 105psi on the 4.5 production casing. The required ID is posted on the pumping unit. The well produces to a diked tank battery to the southeast along the lease road and contains: one (1) 210 barrel tank, one (1) 50 barrel poly tank, a horizontal separator, and the associated meter run. The battery is fenced, locked, and diked and the required ID is posted at the battery. I found no violations at this time. Note: GPS coordinates in the Emergency Well Locator plot this wellhead 700 east of the actual location of the well.

1112966037 2005-03-28 Production Wells Violations Noted Chief Don Harris with the Garrettsville Fire Department reported a pipeline leak at 7311 SR 303. The pipeline was determined to be the salesline from the Morgan #2 tank battery. I contacted John Fleming with Great Lakes who said that they were aware of the problem and that it occurred when the ground was snow covered. They planned on completing the clean-up as soon as weather permitted. We agreed that now was a good time.
1167755126 2006-12-30 Production Wells Violations Noted At approximately 11:45 am, I received a call from Kurt Kollar with the Ohio EPA reporting a pipeline leak at 7018 State Route 303 in Freedom Township. The leak was reported to EPA by the Garrettsville Fire Chief. The adjoining property is owned by Gary Cochran (330-398-1028). The release occurred sometime during the night of 12/29 and resulted in a gas odor complaint to the fire department. The leak was not located until the morning. The spill included natural gas and crude oil (approximately 3 or 4 bbl). The oil impacted an area approximately 100 long x 20 wide directly west of the pipeline. Great Lakes, W. Pol Contracting and Spencer Water Service personnel were on site to contain and remediate the spill. The impacted soil and vegetation were removed and transported to a staging area established at Great Lakes facility on SR 44. The ultimate disposal location will be the Petrocell facility in Lodi. This is the third failure on this section of pipeline in the past two years. After the second leak, Great Lakes told me that the line would be incerted with plastic. It was not. The system was ordered shut in until the sales line was replaced and witnessed.
1168276553 2007-01-02 Production Wells No Violations The sorbent pads and booms are saturated with water and a small amount of oil. I spoke to Great Lakes about changing the sorbent materials. All of the impacted soil has been moved to a staging area in their yard on SR 44.
1191149532 2007-01-18 Production Wells No Violations The spill remediation portion of the NOV should be resolved by the end of the month. Restoration will not be completed until the weather permits in the spring.
2013009412 2006-09-08 Production Wells No Violations I received a call from Kurt Kollar with OEPA relaying a leaking pipeline problem at 7311 SR 303 just west of SR 88. A similar complaint was received in March 2005 which Great Lakes corrected. This second release involves the same salesline. I stopped and talked to Woody McDaniels and Carl Day and requested that they replace or incert this section of pipeline. They agreed to do this next week. They will also grade and seed the area that was disturbed when the second leak was repaired. If the work is completed as promised, a NOV will not be issued.
