0043387264 2015-05-12 Plug / Plug Back No Violations I was on location to continue fishing operations in an attempt to get back inside the rotted off 4.5 production casing. Thompson Well Service ran in the hole with two different tubing and casing spears. The second spear latched onto something at 200 and pulled off without fishing anything out of the hole. Josh Thompson is going to build several large impression blocks and we will run them tomorrow.
0107943223 2018-02-02 Final Restoration No Violations I conducted a Final Restoration inspection of this plugged and abandoned Clinton Sandstone well. This well and its tubulars were severely damaged by acidic waters in the abandoned Hutson Coal Companys #4 mine that this well had penetrated. It was determined by the Inspector, during plugging operations, that borehole conditions rendered compliance with the requirements set forth in OAC 1501:9-11-03(A) impossible and an isolation plug was set below the mine floor from 251-200 to isolate the casing stub and render the well plugged and abandoned under OAC:15019-11-08(G). The following conditions and procedures were followed David A. Waldron & Associates to attain final restoration status for this well:

1. The well be properly vaulted and vented and equipped with an access port that may be monitored/checked for gas.

2. All records held by D.A. Waldron and Associates that pertain to the Montgomery #2 be mailed or delivered to the Uniontown Field Office for storage including any notes or daily records recorded by Josh Thompson and Roy Poulson during plugging operations.

3. Submission of an executed Landowner Waiver (Form 5) allowing for the vault to remain in place for perpetuity (or as long as required) and waiving the removal of the flow line and access gate at the tank battery entrance.

The above conditions have been met by David A. Waldron & Associates and Final Restoration is approved.

0289145000 2015-05-04 Plug / Plug Back No Violations I received a call from Ted Cutright (Waldron) that he has moved Thompson Well Service on location to conduct plugging operations. He told me that the 4.5 production casing had parted between 174-177. I arrived on location and inspected the casing. It was badly rotted out and full holes through the bottom 20. Josh Thompson told me that in 2000, they pulled half of the rods from the hole and they got tight so they began stripping the rods and tubing. At this time, the rods and tubing parted at 200 and the were unable to recover any of the broken off equipment and they abandoned the workover job. Josh is going to get some tools and equipment and will resume operations on Wednesday. *Well History: 1) Drilled in 1980 by Perkins Drilling for New Frontier Exploration, 2) Drilled through an abandoned coal mine and did not set mine string. A basket was set at 90 and grouted to surface. 3) In 1990, 7 joints of 4.5 casing was replaced, due to holes. When the casing was set in the slips, the 8.625 surface casing began to drop and the 13 drive pipe was welded to the surface casing to keep it from dropping. At this time, the surface casing had lost integrity.
0486755264 2015-05-21 Plug / Plug Back No Violations I was on location to witness drill-out operations in an attempt to get back on the 4.5 production casing that is rotted off in the hole. Poulson Drilling ran in the hole with a used 7.875 tri-cone bit and eight (8) 6 collars. They hit something at 167 and rotated past it. They sat down at 196 and began milling. Poulson drilled to 218 and it seemed as if we were drilling on a collar. They drilled to 225 and decided to stop and work on a plan. Operations will resume tomorrow.
0639135160 2017-12-13 Plug / Plug Back No Violations Status check of this plugged and abandoned Clinton Sandstone well. This well was unable to be plugged and abandoned in accordance with the rules outlined in OAC 1501:9-11 due to the presence of an abandoned underground mine below this land. In order to be eligible for Final Restoration, it was agreed upon that D.A. Waldron would install a vault and vent system on the 8.625 surface casing. The system was installed on 12/11/2017 by Moore Well Services and consists of a cement vault with a manhole cover and two (2) PVC vent pipes tied together and capped with a charcoal filter. I spoke with Jeff Moore and he confirmed that no natural gas was present in the wellbore during installation. The short pipe plumbed to the vent stack is venting the surface casing and the long stack is venting the vault. The top of the vault was installed above grade and four (4) steel bars have been placed around the perimeter to prevent damage by farm machinery. I have requested that all well records held by D.A. Waldron, pertaining to the Montgomery #2, be submitted to the Uniontown Field Office for permanent storage and that a Landowner Waiver (Form 5) be submitted to allow for the placement of the vault system for perpetuity. Once these steps have been taken, a Final Restoration inspection will be conducted. No violations.

1711456214 2004-03-19 Production Wells No Violations The last reported production from this well was in 1993. A plugging permit has been issued for this well. No violations exist as long as the well is plugged in a timely manner.
1909322463 2015-05-07 Plug / Plug Back No Violations On location to continue plugging operations. Continued rotating on junk/metal with a tapered mill. Pulled out and ran in with heavy pumping rods to 220 and could not latch on to anything. Will return on 5/12/15 with more rods and several spears.
1972324586 2015-05-06 Plug / Plug Back No Violations I was on location to witness plugging operations. Ted Cutright (Waldron), Thompson Well Services, and Petroset were on location. They hooked up a swivel and ran in the hole with a tapered mill on the end of 2.875 tubing and sat down on an obstruction at 147. They rotated on the obstruction while Petroset attempted to circulate the hole. The obstruction was broken through and the hole went on vacuum. Petroset pumped over 300 barrels of water and the mine void was taking all of the water. They got to 200 with the mill and rotated for an hour without getting any further. The mill was pulled and the markings seemed to indicate that we were in between casing strings or between tubing and casing. They ran in the hole with a 43 joint of 7 casing on the bottom of 4.5 casing to try and swallow up the tubing, rods, and 4.5 production casing A notch was cut in the bottom of the 7 to help cut or swallow the fish. They got the 7 casing to 200 and rotated down to 206. An impression block was run several times and did not give a good indication of what was blocking the hole. Operations shut down for the day.
2008694610 2015-05-13 Plug / Plug Back No Violations I was on location with Inspector John Fleming, Ted Cutright (Waldron), and Thompson Well Service to attempt to try and determine what is blocking the 4.5 casing at 200. A large impression block was run twice, both times showing possible jagged metal sticking up as it made a 1 sharp cut at the edge of the wax block on each run. A smaller block was run two (2) times and the second run split the wax block down the side. They pulled the 4.5 and 7 casing and the tooth cut on the bottom of the 7 was bent inwards and was the likely source of the marks and split in the impression blocks. They cut the 7 off to 6 and ran back in the hole. Two (2) more impression blocks were run and they gave us no information. Thompson milled on the obstruction for two (2) hours and another impression block was run. The block seemed to indicate that it was hitting in between casing strings, 1) between the 4.5 and 8.625 casing, or 2) between the 7 and 8.625 casing strings. It also suggested that the 4.5 or 8.625 casing was bent over or collapsed. It was decided that we needed to get collars and a flat mill to get back into the casing. Waldron is going to have a meeting and make a decision on what needs to be done.
2048578494 2015-05-20 Plug / Plug Back No Violations I was on location to check the status of plugging and fishing operations. Waldron had a meeting last week and decided to bring in Roy Poulson and his Schramm drilling rig to try and mill through the obstructions and get the well plugged. They are currently rigging up and digging a circulating pit. Ted Cutright brought in the pit liner and lined the pit and J-Hawk began hauling in loads of freshwater for the operations. Roy and I discussed the current state of the well and what strategies they are going to employ to get the well plugged. They will begin operations tomorrow.
