0132239175 2008-06-12 Production Wells No Violations I received a call from Alan Coogan requesting an inspection of this well. The last reported production was in 2001. The Knights of Columbus facility has not had domestic gas for several years. The tank battery is overgrown but is not leaking and the tanks have been emptied. I called Moore Well and spoke with Chuck V. He stated that they intended to sell the well to the K of C or to plug and abandon. The issue should be resolved this summer.

1320868118 2011-11-09 Orphan No Violations I inspected the well site area for gas leaks. The vent pipe was working. The orange fence is around the well area. This complaint continues.

1337960467 2012-05-25 Orphan No Violations My inspection today was with Mike Dinsmore Oil and Gas Inspector and Bill Liston representative for the land owner. Previous inspections have found that this plugged well is leaking. The well is currently vault and vented by Moore Well Services Inc. Moore well service has also put up safety fence around the wells location. I informed the land owner that the well has been plugged correctly (see plug report,) and the well has had final restoration approved (see RBDMS report.) The well is to be referred to the idle and orphan program for remedial action. Bill was given the Land Owner Release form to complete and return to the division. The well is in an open field, 150 feet from a water well and 150 feet from a play ground that is under construction. I informed Bill, that the well at this time is not a priority safety concern. The well is vented 10 feet into the air. I advised Bill to notify Mike if there is a noticeable change in the well, examples: Vegetation scalding, increase in gas venting. At the time of inspection there was no visible gas venting on the surface of the ground or from the vent. High winds prevented any air readings from being taken.
1340368235 2012-05-25 Orphan No Violations On location with DNR Inspector Tom Hill to meet with Bill Liston (church representative - 330-842-2811), and follow up on a complaint of a leaking, plugged and abandoned well behind the church. Well head area is surrounded by orange safety fence with a 10 PVC vent pipe sticking out of the ground. No visible leakage was observed. Mr. Liston said that when the ground is saturated, there is visible bubbling around the enclosed area. I told Mr. Liston that I would start paperwork to get the well placed in the Idle & Orphan Well Program in order to have the issue addressed. I left him with a Right of Entry form to be filled out by the church and to be returned to me.

1443448386 2016-05-03 Orphan No Violations On site to follow up on the original Orphan well complaint received by the Division in 2011. The Chapel at Tinker Creek church filed a complaint with the Division on 10-18-2011 reporting that bubbling was coming through the ground in the area of a plugged well. Inspector Roberts conducted the initial inspection and Inspector Dinsmore conducted the follow up inspection. My inspection today showed a PVC vent coming from the ground. Well casing is not visible The vent is approximately 10 high. A gas reading is not possible due to the height of the vent. There is not evident oil or gas leakage in the grass around the well. The area is heavily vegetated. Orange plastic safety fence is around the well and vent. The well is 35 yards SE from the church playground and 55 yards east from the church fellowship building and 105 yards west of the church. The well was drilled in 1979 and is a Clinton well. The well was plugged on 8/29/2008.
1476728252 2016-10-14 Orphan No Violations My inspection found a 2 vent stack plumbed up to 8 above the ground. The area around the well is protected by 4 ft high orange safety fence. The well was not leaking at the time of inspection. The orphan paperwork is in the process of being completed for this well.
