1723464867 2003-04-24 Production Wells Violations Noted CASE 03-013 I received a call from Mike Loper, the GE contract pumper (330-495-3939), reporting a pipeline leak near the Hinman #1 well. The leak was associated with the sales line from this well and resulted in both crude oil and brine impacting a nearby unnamed tributary to Eckart Ditch. The total spill volume was estimated between 1 and 1.5 barrels. Brian Kelly with the USEPA-NRC (734-692-7684 or 734-740-9019 cell) was notified of the stream impact. The NRC number for this release is 643082. Rob Sommers and Aaron Gula with General Electric, also mobilized to the site. The initial response included, shutting the well in, placing booms and sorbent pads along the stream course until oil was no linger visible, approximately 500 feet, and recovering impacted soil and vegetation near the spill point and down stream. D&M Contracting from Randolph, Ohio was retained by GE to repair the 2 sales line and to remediate the spill. The response equipment included a mini track hoe, rolloff boxes and off road vehicles. The sales line was exposed and three holes were located and clamped off. A siphon dam was constructed between the last hole and the stream bed. All impacted soil was placed into lined rolloff boxes and transported to a nearby GE facility for temporary storage until disposal could be arranged. In all, 3 rolloff boxes with an approximate total of 30 cubic yards of material was removed. Future action will include removing impacted vegetation downstream, boom and pad replacement and site monitoring.
2020701675 2003-05-01 Production Wells Violations Noted Spill remediation is continuing nicely. The siphon dam is preventing any free oil from reaching the creek and a majority of the impacted vegetation along the creek bank has been removed. I will continue to monitor the site.
