0643662406 2018-05-22 Plug / Plug Back No Violations I was on location to witness the plugging and abandonment of this Clinton Sandstone well. Hagen Well Service (HWS) ran inside the 4.5 casing, and tagged the top of the bottom plug at 4300. They loaded the 4.5 casing with water, ran in the casing with the knife tool, and parted the casing at a depth of 3200. 72 joints of 9.5lb/ft., Range-3 casing was recovered from the hole and taken for scrap. HWS ran 95 joints of 1.5 tubing back inside the 7.875 open hole to a depth of 2980 and packed in the wellhead for the day.
0950204130 2018-02-07 Production Wells No Violations Status check inspection of this Clinton Sandstone well that is scheduled to be plugged and abandoned in the next week. My inspection found the wellhead equipped with 1.5 tubing and equipped with a motor valve, logic controls, and a plunger lift system. The Demco gate valve is closed and the 4.5 production casing flow line ball valve is closed. This well produces to a diked tank battery to the north and is shared with the Timberlake #2 (PN#22526) owned by Pin Oak Energy. The battery contains one (1) 100 barrel tank (Pin Oak), a concrete brine vault, a horizontal separator, and two meter runs. The required ID is posted at the wellhead and at the tank battery. No violations.
1005371081 2018-07-30 Final Restoration No Violations I was on location to check the Final Restoration status of this plugged and abandoned, Clinton Sandstone well. The well was plugged on 5/23/18, the surface equipment has been removed, and the flow line between the wellhead and the tank battery was pulled. The remaining equipment at the tank battery is for production of the Timberlake #2 (PN#22576). The area of the former wellhead is in an agricultural field and grass has established in the area and is surrounded by corn. No landowner waiver is required for this well. Final Restoration is approved. No violations.
1273578104 2018-05-24 Plug / Plug Back No Violations I was on location to check the level and integrity of the top plug that was set yesterday (5/23/18) in this Clinton Sandstone well. My inspection found the fluid level five (5) feet below the top of the 8.625 surface casing head. I tagged the top of the cement with a weight and reel tape at 9.5 below the casing head. J.R. Smail personnel topped the hole off with five (5) bags of Quikrete, cut the casing head off, and attached the ID plate. The casing was buried and the area was graded. Final restoration is due by mid-November. No violations.
1540173248 2018-06-18 Final Restoration No Violations I was on location to check the status of Final Restoration of this recently plugged and abandoned well. The areas impacted during plugging operations have been graded and the flow line from the wellhead to the tank battery has been removed at the request of the landowner. All equipment for this well has been removed and the tank battery and lease road remain for the production of the Timberlake #2 (PN#22526). I will conduct a follow-up inspection to check for stability in the cornfield prior to approving Final Restoration. No violations.
1962592845 2018-05-23 Plug / Plug Back No Violations I was on location to witness the plugging and abandonment of this Clinton Sandstone well. Hagen Well Service and Petroset Cementing spotted cement plugs through 1.5 (ID) tubing to cover and isolate the top of the Big Lime, the Berea Sandstone/Ohio Shale interval, and the Glacial Deposits. See plugging report for more details on this job. No violations.
2055317187 2018-05-21 Plug / Plug Back No Violations I was on location to witness the plugging and abandonment of this Clinton Sandstone well. Petroset Cementing killed the well with 50 barrels of water pumped down the 1.5 tubing. They then mixed and pumped 7.3 barrels (30 sacks) of Class A cement slurry. The plug was spotted inside the 4.5 production casing, across the Clinton Sandstone, from 4710 to 4260. Hagen Well Service pulled the tubing from the well and operations were shut down for the day.
