1517500186 2018-02-01 Final Restoration No Violations I conducted a status check inspection for Final Restoration of this plugged and abandoned well. This well was plugged and abandoned in 1972 and the plugging is detailed in a report submitted by Deputy Mine Inspector John Douglas dated 7/21/72. I reviewed aerial photos on the Portage County Auditors website from 1966, 1970, and 1975. Aerials from 1966 & 70 showed the lease road entrance, the disturbed wellhead area, and the position of the surface equipment on the west side of the lease road entrance. The 1975 photos show the surface equipment removed and no remnants of the lease road except the entrance. My inspection today found the culvert and entrance still in existence and that it is being used by the current landowner for access to the agricultural field. I inspected the area of the surface equipment and found no evidence of piping or ancillary equipment. I walked to the GPS location of the wellhead and searched a 200 radius in the field. I found no evidence of a wellhead or any ancillary equipment above grade. The entire area is cropland and is sewn with short grass. This well was plugged and abandoned over 45 years ago and no Landowner Waiver (Form 5) will be required. Final Restoration is approved. No violations.
