0192052975 2018-05-30 Orphan No Violations Orphan well field work; accompanied by DOGRM staff Kris Roser and Jason Blocker: The purpose of this field visit was to dig down to expose the 10.75 diameter casing and run a downhole camera. Upon exposing the casing, a slight natural gas odor was noted, however no ambient gas levels were detected on the Ventis 4-gas meter. Upon running the downhole camera, a fluid level of 61 was observed with crude oil on top of the fluid column and natural gas bubbling through the fluid column. A weighted line was run into the wellbore which sat down at a depth of 990. A plastic water meter tub was placed around the casing to act as a riser to keep it open at grade. A table was placed on top of the riser to keep people or pets from falling into the well. Photos were taken of the field work done.
0697518439 2016-08-23 Orphan No Violations Orphan well status inspection: Upon inspection, it is observed that this well is still idle and orphan. The casing and cement which were originally uncovered and exhumed by the landowner are now back in the hole and are being used to cover the well. Part of the damaged steel casing is sticking up 10 above ground level at the well. The landowner, Mr. Melvin Baker, states he covered the well back up for safety purposes. There are several fruit trees around the well which were recently planted and would likely need to be removed to perform the plugging procedure. The Bakers house burned down last year and a new house stands in its place. Photos were taken of the location.
1289986253 2014-07-23 Orphan No Violations I met with Cindy to show her this well for the orphan program. We talked with Mel Baker to let him know how the process goes and what the state would possibly due to replug the well.
1405599976 2014-07-16 Orphan No Violations I was contacted by Mel Baker from New Straitsville. Mel removed what he thought was a rock from his garden but ended up being a 10 piece of pipe with concrete in it. When he pulled it out from the ground he uncovered a well. I went out to inspect the well. There is fluid down the hole approximately 35ft. We ran a tape measure with a weight on it down to see if we could hit the plug. The weight set down at 93ft. I was able to find a plug report in our system from 1916. I will contact Cindy and prepare the well for the idle orphan program.
1406230978 2014-07-23 Orphan No Violations My inspection this day with Perry Co. Inspector Justin Smith was in response to a complaint filed by landowner, Melvin Baker, of a large open hole in his garden. We found an open hole, approximately 12-14 in diameter, where he had encountered some steel casing while tilling his garden. A 3 long piece of 10 diameter surface casing was lying in the grass with a cement plug in the top of it. It was apparent that this casing had rusted off below the ground and, according to the landowner, was easily pulled out when disturbed. Records show that there was 160 of 10 diameter casing installed in this well when it was drilled to a total depth of 3330, in the Clinton Sandstone. Records also show the well had been plugged, but Mr. Baker ran a measuring tape down to 93 before hitting any resistance and the tape had oil on it when removed. By looking down the well, you can see the 10 casing on the wall of the hole and water @ 40 down, with surface water steadily dripping down. Pictures taken, SEM is 204.
1529421849 2018-05-18 Orphan No Violations On location to take photos of the well location and potential access routes. The area upslope of the well is saturated with water due to a spring which, according to the landowner, flows nearly year-round and the area stays very wet. The spring may need to be addressed before putting a service rig over the well.
1641257636 2018-05-01 Orphan No Violations Orphan well status inspection, accompanied by DOGRM Staff members Nate Lane, Paul Carder, Rob Lowe, and Lucas Smith: The purpose of this inspection is to familiarize the technical sections with this well so they can perform their individual assessments for the Perry #2 Orphan Well Plugging Project.
1815227831 2014-07-21 Orphan No Violations I returned to this well to take pictures of it for the orphan program.
