0074105426 2017-07-28 Plug / Plug Back No Violations I returned to this well today to witness the plugging of it. The rig crew ran there tools back in the well this morning to tag the plug. They tagged the cement at 1130 feet so they are running the tubing back in the well to set more cement on top of it. Artex decided to put another 200 feet of cement at the bottom of the surface plug. They then pulled the tubing up to set a 222 foot surface plug. Steve Minor picked the surface plug footage.
0339568508 2017-07-27 Plug / Plug Back No Violations I was back on location today to witness the plugging operations of this well. Today Formation cementing was back out on location to set the Clinton plug from (2877-2677), the Big lime plug from (2034-1834) and the bottom of the surface plug from (1107-907). We had circulation on each plug but the fluid level settled on the last so Steve Minor from Artex Oil Company decided to shut down for the day and tag the plug in the morning. Plugging operations will continue tomorrow.
0434461286 2016-10-11 Casing String Witnessed No Violations I was on location tonight to witness the cement job on the 8-5/8 diameter surface casing. Kilbarger #5 drilled the hole to 1039 feet. The 8-5/8 casing was ran to a depth of 1022 feet. Formation Cementing was the company used to cement the casing to surface. After pumping the cement we had 21 barrels of returns. The cement stayed at the surface and did not settle.
1118336474 2016-10-27 Completion Testing No Violations I was on location today to witness the acid job on this well. Artex Oil Company had Producers Service Corp come out to do the job.
1226735046 2016-12-06 Preliminary Restoration No Violations I was on location today to do a preliminary restoration on this well. The location has been reclaimed and all the production equipment is hooked up and capable of production. The preliminary inspection passed its inspection.
1328994056 2017-07-25 Plug / Plug Back No Violations I was on location today to witness the plugging of this well. Bishop Well Service ran the 2 inch diameter tubing back to bottom to set the bottom plug. They had Formation Cementing come out today to pump the bottom plug through the 2 inch tubing. A 30 sack Class A cement plug was pumped through the tubing for the bottom plug. Plugging will continue tomorrow.
1476963397 2016-10-17 Casing String Witnessed No Violations I was on location today to witness the production string being cemented. Formation Cementing used 175 sacks of Class A cement with 200 pounds of gel and 50 pounds of L.C.M Flake.
1660093313 2017-07-21 Drill / Deepen / Reopen No Violations I returned to this well to check on the rig crew to see how the job was going. They are currently still swabbing fluid back at this time. The well is making a lot of water at this point. They plan on swabbing the well for a few more days.
1726732143 2017-07-26 Plug / Plug Back No Violations I was back on location today for the plugging of this well. The rig crew had to run the tubing back to bottom to reset the bottom plug. When the rig crew ran there 4.5 inch ripping tools down to rip the casing they found the formation had drank most of the bottom plug so they wanted to add more cement to it. They had Formation Cementing come back out and pump 30 more sacks of Class A cement on top of the plug. The rig crew then puled the tubing back out of the well so they could rip the 4.5 inch diameter casing. Before ripping the pipe they tagged the plug at 4480 feet. After tagging the bottom plug the rig crew was able to find free pipe at 3677 feet. They ripped the 4.5 inch casing at this point. They will finish plugging the well tomorrow.
1771670543 2017-07-18 Work Over Reconditioning No Violations I went out to this well today to see how the frack job went. The company had decided to try and frack this well to see if the production would come up. They had Producers Service Corp come out to do the work. I was not on location to witness the actual job but Joe the company man from Artex had paperwork for me when I arrived on location. He stated that the shut in pressure was 468psi and after a 2 hour shut in the pressure is 340psi. The well is currently flowing back on its own with little sand returns. The well was fracked from 4765 to 4798 feet by spotting acid and then using gel. There was a mechanical bridge plug set just below 4798 feet to separate the bottom zone. The rig crew will be driving the mechanical plug to bottom when they complete the well.
1785585976 2016-10-26 Drill / Deepen / Reopen No Violations I stopped by this well today to see how everything was going. The service rig has moved in on location and they are running the tubing in the well so they can spot the acid through it in the morning.

The well now has rods with 2 inch diameter tubing in it. The pumping unit is hooked up to the well and the flow lines are ran to the production tank. As soon as the rest of the location is put back and reclaimed I will be able to do the preliminary restoration for it.
