0836976564 2016-05-18 Plug / Plug Back No Violations I met with Bill Minton today at Knox Energy to discuss how we would plug the well. They have pulled out the rods and 1.5 tubing from the well but the 2-78 is cemented inside the 4.5. Bill is contacting Appalachian to see when we can get them out there to run a bond log on the well.

1268523356 2016-05-20 Plug / Plug Back No Violations I was on location today to witness the plugging of this well. Knox Energy has there service rig on the well to plug it. Huffman and Bowers will be the company used for the cement. The rods and tubing were pulled from the well but they were unable to pull the 2-7/8. They thought it was on a packer and it was not releasing. After looking at the permit we found out that this well was plugged back from the Rose Run to the Clinton. When they did that they did not get a good cement job on the 4 so they ran the 2-7/8 and cemented it back to surface. We had Appalachian come out and run a bond log to see if we could find a free spot. After we pumped the bottom plug to surface we ran back in and shot 10 holes at the Big Lime then another 10 holes at the Berea. Knox Energy then ran 1.5 tubing down to the Big Lime to set the plug. Once that plug was set we pulled up to the Berea and set that plug. We pumped some extra sacks of class A cement at the Berea to get up to the 8. The 1.5 tubing was pulled from the well so we could shoot off the 2-7/8 . The tubing was shot at 410ft but it would not move. The cement was still around the pipe. We then pulled up to 200ft and shot the 2-7/8 off. They were able to pull it from the well. We then pulled tension on the 4.5 and backed it off with power tongs. 200ft of 4.5 casing was retrieved from the well. A 200ft cement plug topped the well off at the surface.
