0093430247 2014-01-16 Orphan No Violations This inspection was done to review the location to comlpete the plugging Scope of Work for the Orphan Well Program. There was no change in the status of the well from previous inspections and the Hydrogen Sulfide levels were measured at 10 ppm in winds over 15 mph.
0105732025 2014-10-14 Orphan No Violations This well under contract to be plugged for the Orphan Well Program by Glenn Davis Well Service. At the time of the inspection excavation continued on the suspected area. The last piece of 24 inside diameter clay tile was removed and a clearly defined borehole was evident about 12 below grade. The suspected borehole was about 10 in diameter and gray in color. A 6 diameter circle of black material was inside the larger borehole. A distinct bubbling sound could be heard coming from the black center area of the bore. There was a distinct smell of Hydrogen Sulfide at the location but no gas was detected at ground level. The decision was made to stop for the day and bring a 10 x 20 steel pipe to use as a riser on 10-16-14. A ring of DO NOT ENTER tape was put around the area until plugging operations continue.
0360602167 2014-11-03 Plug / Plug Back No Violations At the time of the inspection the well was cut off three feet below grade with cement observed at that level inside the 4.5 casing. The level of cement in the annular space between the 4.5 casing and the 10 riser was about two feet down. This space was filled from the surface with 3 bags of concrete mixed on location by the plugging contractor. No gas in any levels was detected at the well or the surrounding area. The identification plate was placed on the well and the remaining hole was filled in. The area was then graded and prepared for seeding.
0366389022 2014-10-24 Orphan No Violations This well under contract to be plugged for the Orphan Well Program by Glenn Davis Well Service. At the time of the inspection the well was bailed to a depth of approximately 30. There was zero ppm Hydrogen Sulfide at the wellhead prior to bailing, but as the fluid was removed from the well the gas level increased to 500 ppm Hydrogen Sulfide. Hydrologist Kyle Tharp was on location with the downhole camera. It showed the bottom of the riser at 12 with a clear bore offset to one side. Approximately 10 and 15 below that were small zones making some water. The bottom of the hole still had some muddy water that was bubbling gas. At no point did it appear as though there was any casing or rock on the sides of the borehole. Cleanout continued at 1:00 pm with a 5 cable tool bit and the hole empty to allow for bailing. A crew from OBriens Safety Services was on location to monitor gas levels during the cleanout process. Two fans were placed around the well to help dissipate all gas from the work area while cleanout continued. At 1:45, no progress had been made but the gas was increasing both in concentration and in visible volume. Hydrogen Sulfide levels at the wellhead maxed out the gas meters at 500 ppm. The decision was made to switch to a 4 bit and continue drilling on the obstruction. After drilling for another hour the mud and water coming in the hole were inhibiting any progress. The decision was made to stop drilling and come back on Monday with 4.5 casing and attempt to case the hole to the obstruction. The well was flooded, then vented through a hose that was placed ten feet up the mast. There was zero ppm Hydrogen Sulfide at the wellhead after flooding the well with fresh water. A ring of DO NOT ENTER tape was put around the area until plugging operations continue.
0637504609 2014-03-28 Orphan No Violations This inspection was done to discuss the logistics of plugging the well for the Orphan Well Program. In attendance was Fred Petersen of the Ottawa County EMA (419-734-6904) along with Ron Libben and Henry Hatt of the Erie Township Fire Department. At the time of the inspection the well was still bubbling trace amounts of Hydrogen Sulfide gas. There was a strong odor downwind from the well but no Hydrogen Sulfide gas levels were detected due to high winds. The well was located 40 behind the house at 4043 W. Lakeshore Drive inside a 10 x 10 chain link fence. The well is under a 5 x 5 sinkhole filled with water that is covered by a wooden door with a car tire placed on top of the door.
0827002840 2014-10-28 Orphan No Violations This well is under contract to be plugged for the Orphan Well Program by Glenn Davis Well Service. At the time of the inspection the crew moved the rig and equipment to the Gottron Well to start the cleanout of that well. A crew from OBriens Safety Services was on location to monitor gas levels during the cleanout process. On 10-27-14, a Total Depth of 42 was reached drilling through new rock. This appears to be an old water well so the decision was made to plug the well at that depth through the 4.5 casing. This will be done on the same day the Gottron Well gets plugged. The well was left flooded, then vented through a hose that was placed in the half tank. There was zero ppm Hydrogen Sulfide at the wellhead or the vent hose before leaving the location. Landowner Cheryl Meyers remembers a pipe sticking up in yard years ago when grandfather owned property and oil/gas smell from pipe. Inspector dug hole in middle of barren area approximately 2 feet deep and encountered deteriorated plastic covering open borehole. Strong odor of H2S and bubbles in water. There was no indication of casing at top of hole and ground appeared to be undercut below water. Placed sheet of plywood over open hole and recommended restricting access to site.

At the time of the inspection the rig was brought on location and set up on the well. The 10 riser was cut off and a 7 collar was welded to the riser for attaching the wellhead. A 7 to 2 swage with a 2 valve was put on the well. A 6 long, 2 diameter pipe was placed on top of the valve and the valve was left open to vent the gas. There was a distinct smell of Hydrogen Sulfide at the location but no gas was detected at any levels in the work area. A ring of DO NOT ENTER tape was put around the area until plugging operations continue.
1413905276 2014-10-21 Orphan No Violations This well under contract to be plugged for the Orphan Well Program by Glenn Davis Well Service. At the time of the inspection the service rig was set up on the well with a 6.25 cable tool bit. A wellhead is in place on the drill line and ready to be put on the 10 casing. A crew from OBriens Safety Services was on location to monitor gas levels during the cleanout process. Cleanout of the well started at 12:50 pm and continued till 3:00pm at a rate of around 10 an hour. The amount of bubbling in the well seemed to increase as cleanout continued and there was a distinct smell of Hydrogen Sulfide at the location, but no gas was detected at any levels in the work area. Cleanout slowed then was stopped at 4:30pm at a depth of 25. The well was vented into the half tank and a ring of DO NOT ENTER tape was put around the area until plugging operations continue.
1413990659 2014-10-22 Orphan No Violations This well under contract to be plugged for the Orphan Well Program by Glenn Davis Well Service. At the time of the inspection the 6.25 cable tool bit was replaced with a 5 bit and cleanout continued. A crew from OBriens Safety Services was on location to monitor gas levels during the cleanout process. When the wellhead was removed to change the bit, bubbling gas could be heard in the fluid at the bottom of the wellbore. A reading of 103 ppm Hydrogen Sulfide was detected from the top of the wellbore. This level dissipated instantly and no other levels were detected at any location the remainder of the day. Cleanout of the well started up again at 12:00 pm after a short delay to repair a chain on the rig that broke just after cleanout started. Cleanout slowed then an obstruction was encountered at a depth of 28. At around 1:45 pm the obstruction was broke through and the bit went two more feet in five minutes. At that point the gas from the well visibly increased and levels were detected of over 200 ppm Hydrogen Sulfide on the monitor closest to the well. The work area was immediately evacuated to the predetermined muster point. A representative of OBriens Safety Services, with an air pack and mask, took readings at the wellhead of 500 ppm Hydrogen Sulfide. The well was vented at the wellhead and into the half tank while the area was continuously monitored but no levels of Hydrogen Sulfide higher than 10 ppm outside of ten feet from the well. After an hour of venting the decision was made to flood the well with fresh water. After doing so, no levels of Hydrogen Sulfide were detected at the well so drilling continued from 3:30 pm till 5 pm to a depth of 32. The well was vented through a hose that was placed twenty feet up the mast and a ring of DO NOT ENTER tape was put around the area until plugging operations continue.
1414169093 2014-10-23 Orphan No Violations This well under contract to be plugged for the Orphan Well Program by Glenn Davis Well Service. At the time of the inspection, cleanout continued with a 5 cable tool bit and the hole filled with fresh water. A crew from OBriens Safety Services was on location to monitor gas levels during the cleanout process. Just after cleanout started, an obstruction was encountered again at a depth of 32. At that point a sinkhole approximately 5 in diameter started to develop on the Northeast side of the riser. The decision was made to stop all drilling until a downhole camera can be used to determine the best way to proceed. The well was vented through a hose that was placed twenty feet up the mast and a ring of DO NOT ENTER tape was put around the area until plugging operations continue.
1414420515 2014-10-27 Orphan No Violations This well under contract to be plugged for the Orphan Well Program by Glenn Davis Well Service. At the time of the inspection the depth of the well was tagged at 35 with the tools. This is a loss of 2 from 10-24-14. Multiple pieces of 4.5 casing were rigged up to go down hole to case off shallower pockets of water and mud that were slowing the drilling of the obstruction. The pipe went down to 25 before stopping and was driven down with minimal effort to a depth of 35 before stopping. The well was bailed down and the oil saver wellhead was placed on the 4.5 casing before drilling continued. A crew from OBriens Safety Services was on location to monitor gas levels during the cleanout process. At 10am, before any work started on the well, Hydrogen Sulfide levels were at 200 ppm at the vent hose and 1 ppm at the wellhead. Two fans were placed around the well to help dissipate all gas from the work area while cleanout continued. During drilling, gas was increasing both in concentration and in visible volume. Hydrogen Sulfide levels at the wellhead maxed out the gas meters at 500 ppm. A Total Depth of 42 was reached drilling through new rock so the decision was made to plug the well at that depth through the 4.5 casing. The well was flooded, then vented through a hose that was placed in the half tank. There was zero ppm Hydrogen Sulfide at the wellhead after flooding the well with fresh water. 1415637710 2014-10-30 Plug / Plug Back No Violations This well is under contract to be plugged for the Orphan Well Program by Davis Well Service. At the time of the inspection the well was plugged from a Total Depth of 40 to the surface with 180 sacks of 50/50 poz mix from PetroSet Cementing. A crew from OBriens Safety Services was on location to monitor gas levels. There were readings that maxed out the gas meters at 122 ppm Hydrogen Sulfide at the wellhead during the cleanout. Levels around the well and work area during plugging stayed between zero and 15 ppm of Hydrogen Sulfide.
1448566394 2014-10-16 Orphan No Violations This well under contract to be plugged for the Orphan Well Program by Glenn Davis Well Service. At the time of the inspection a 10 x 20 steel pipe was placed over the suspected borehole. The remaining hole was backfilled and the riser pipe leveled. Almost 8 of the 10 diameter pipe is above grade to allow the well to vent until a wellhead can be put in place. A ring of DO NOT ENTER tape was put around the area until plugging operations continue.
1469952360 2013-08-05 Orphan No Violations An inspection of this orphan well revealed gas bubbling within the fluid and the release of H2S from an approximate 2x2x1 depression in the ground surface. The wellbore was situated inside a cyclone style fence and located underneath a wooden door and tire that had been placed over top for protection. The well was monitored with a gas detector attached to an automatic pump, probe and filter to show an H2S concentration of 75 ppm. The well was located between the residence at 4063 (blue house) W. Lakeshore Drive at approx 50 away and a pond approx 75 away. A second residence at 4043 was approx 100 away. Note: Cheryl Meyers states that although different addresses its the same property owned with her sister Roxanne Hall and both properties were for sale. The well is also located approx 10 and 15 away from two propane tanks; approx 25 away from a septic tank directly behind house. At this time, the only overhead obstruction at the well would be a tree limb. Recommend locating the septic lines prior to plugging. A GPS reading was obtained for the well and potential entry into the well from residence at 4063. Note: Properties appear to contain public water supply; and telephone and cable lines are low in spots near entry at 4063 residence. Central Office will be notified of H2S status for this county and township.
1746370152 2014-10-10 Orphan No Violations This well under contract to be plugged for the Orphan Well Program by Glenn Davis Well Service. At the time of the inspection excavation continued on the suspected area and 3 more, 24 inside diameter, clay tile were discovered. These tile were placed vertically, one on top of the other, and filled with a chalky colored water and mud mixture. The bottom piece had a much harder material inside it with a soft spot in the middle. A distinct bubbling sound could be heard coming from the center of the soft area and Hydrogen Sulfide levels at the source exceeded 100 ppm. There was a distinct smell of Hydrogen Sulfide at the location but no gas was detected at ground level. The decision was made to stop for the day and excavate the area further to determine the source of the Hydrogen Sulfide gas at a later date. A ring of DO NOT ENTER tape was put around the area until plugging operations continue.
1966802259 2014-10-08 Orphan No Violations This well under contract to be plugged for the Orphan Well Program by Glenn Davis Well Service. At the time of the inspection a backhoe was brought on location by the plugging contractor to begin site preparation. The area of the suspected well was excavated and a 24 inside diameter clay tile was discovered. This piece of tile was placed vertically and about two feet below grade. It was filled with a chalky colored water and mud mixture. There was a distinct smell of Hydrogen Sulfide at the location but no gas was detected at any levels in the work area. The Environmental Health Department (EHD) was notified for assistance in identifying what the suspected well may have been. According to Officials from the EHD, Ottawa County, and EMA, nothing like this has been seen before in the area. The decision was made to excavate the area further to determine the source of the Hydrogen Sulfide gas. A ring of DO NOT ENTER tape was put around the area until plugging operations continue.
